Improvement of Activities of Daily Living (ADL) for Patients Treated in Skilled Nursing Facilities (SNFs)

Background: Self-care tasks or activities of daily living (ADL)’s performance are often disrupted after an older adult sustains an injury or illness, which can be a determining factor for discharge placement after hospitalization, or it can determine the level of assistance required after discharge from a skilled nursing facility (SNF). We believe that comprehensive rehabilitation can improve a patient’s overall functioning during their short term stay in SNFs. The purpose of this study is to determine if an individual’s level of independence improved after admission to a skilled nursing facility and procurement of rehabilitation services. Methods: This study retrospectively reviewed 4612 nursing home patients in California who underwent rehabilitative services at skilled nursing facilities to determine how the patients responded to their environments, therapy interventions, and any other additional supportive measures. The records of patients who were admitted for short-term rehabilitation were reviewed in a blinded fashion, looking specifically at Section GG of the Minimum Data Set (MDS). Self-Care Section GG scores recorded by rehabilitative staff provide objective data and measure patients’ levels of assistance that were required. Each activity is scored from 1, being completely dependent, to 6, being entirely independent, with varying levels of assistance with scores in between. The admission scores versus the discharge scores can be compared to determine if a patient improved their level of functioning upon discharge. Results: Utilizing the Pearson’s correlation coefficient, a strong correlation of improvement in ADL performance on Section GG between the admission and discharge scores was identified, implying significant improvement in functional independence upon discharge. The average percent improvement for Managed Care patients was ~35.4% and ~39.4% for Medicare patients. Conclusions: The results support the benefits of rehabilitation services in skilled nursing facilities, and the data suggests that admission scores can be used as a predictive tool for functional outcomes. 展开更多

Objective: To assess the outcome of the application of active learning during practicum among nursing students using clinical assessment and evaluation scores as a measurement. Methods: Nursing students were instructed on the basics of active learning prior to the initiation of their clinical experience. The participants were divided into 5groups of nursing students ( n = 56) across three levels (years 2-4) in a public academic institute of a bachelor degree program in Macao. Final clinical evaluation was averaged and compared between groups with and without intervention. Results: These nursing students were given higher appraisals in verbal and written comments than previous students without interventian. The groups with the invention achieved higher clinical assessment and evaluation scores on average than comparable groups without the active learning intervention. One group of sophomore nursing students (year 2) did not receive as high of evaluations as the other groups, receiving an average score of above 80. Conclusions" Nursing students must engage in active learning to demonstrate that they are willing to gain knowledge of theory, nursing skills and communication skills during the clinical practicum. 展开更多

Objective:This paper aims to explore the effect of individualized nursing intervention on patients with active tuberculosis(ATB)and latent tuberculosis infection(LTBI).Methods:The nursing study started in January 2020 and ended in January 2023.A total of 60 patients with ATB and LTBI were included,and they were divided into two groups according to the intervention schemes selected for control testing,each with 30 cases.The intervention program selected for group A was routine care,and for group B was individualized nursing.The proportion of adverse reactions,changes in the level of lung items,self-management outcomes and satisfaction were evaluated and compared.Results:After evaluating the proportion of adverse reactions,the total proportion of ATB and LTBI in group B was lower than that in group A(P<0.05).Based on the evaluation and testing of the expiratory flow(EF),expiratory volume(EV),and vital capacity(VC)after the intervention,these levels in group B showed higher outcomes than those in group A(P<0.05).The scores in terms of living habits,sleep,diet,and compliance in group B were higher than those in group A(P<0.05).The total proportion of the satisfaction of ATB and LTBI patients in group B was higher than that in group A(P<0.05).Conclusion:After the intervention of individualized nursing measures in patients with ATB and LTBI,it was found that it can not only play a positive role in the prevention and control of adverse reactions,but also improve their lung function,and promote their self-management,with good satisfaction level,thus it has high research and clinical application values. 展开更多

Objective:Entrustable professional activities(EPAs)is a newly emerging concept.This paper aims at analyzing the concept of"EPAs"to clarify the meaning and to identify its components for optimum understanding of the concept for use in nursing education and future research.Method:Three databases-ProQuest,Ovid,and PubMed-were retrieved,and Walker and Avant's concept analysis approach was used to analyze the concept of EPAs.Results:Based on the analysis,three attributes of EPAs were defined,namely (1)EPAs should be restricted to a limited number of tasks;(2)reflect the experience curve;and (3)include acceptance of responsibility and accountability.The antecedent of EPAs is a holistic and integrated assessment approach.This will eventually lead to anticipated consequences such as a mastery of core competencies and an increase in the level of autonomy.Conclusion:The study may contribute to a reduction in the ambiguity of this concept to better apply it in nursing practice. 展开更多

Objective: The purpose of this study was to clarify the collaborative activities and mutual recognition between community comprehensive care unit nurses (Ns) and care managers (CM) in supporting the discharge of the elderly from the hospital. Methods: A total of 300 nurses working in community comprehensive care wards and 360 care managers working in B City in A Prefecture were surveyed using an anonymous self-administered questionnaire. Results: The highest percentage of responses regarding necessary collaborative activities with multiple professions in supporting hospital discharge were the same for Ns and CMs. The items regarding practice with the highest percentages were “relationship as a team” for Ns, and “user-centered awareness” for CM. While these professionals were willing to share information about their patients’ lives after discharge, the percentage of those explaining their expertise was low. It is thought that collaborative activities focusing on these aspects would lead to more appropriate discharge support. 展开更多

The aim of this paper was to describe the routine of Brazilian nursing students, identifying the activities that are part of their daily life, highlighting how much time they dedicate for physical activity practice in their leisure time. The participants of this cross-sectional study were 141 nursing students from a public university in the State of S<span style="white-space:nowrap;">&#227;</span>o Paulo. Data were collected using a sociodemographic questionnaire, three closed questions about physical activities and one open question in which the students described their daily life activities. For analysis, we used the chi-squared-test and content analysis. Although academic, household and work activities take up most of the students’ week, some dedicate a portion of their daily live to enjoy leisure periods with physical activities, sedentary behavior and sociocultural activities. Some options of activities seem to be related to Brazilian cultural aspects that are pointed out in this paper. Some recommendations were made, aiming at improving the health of students as well as the importance of a healthier school environment. 展开更多

The Influence of Sound Awareness on the Level of Sound Generated during Nursing Activity in a Pseudo-Ward

Sound generated in a ward can be classified into 1) generated by medical staff, 2) generated by patients and their visitors, and 3) others, such as from in-hospital broadcasting microphones and nurse calls. Among these sounds, the incidence from medical staff, in particular, is reportedly high. The study objective is to investigate whether sound awareness is effective in regulating the sound environment even in a busy situation, such as in a real clinical setting, and to examine the extent to which sound awareness affects sound level. Nursing students were asked to perform a series of nursing activities in a pseudo-ward, and the changes in the sound level generated during the nursing activities with or without time and sound awareness were examined. Under varying experimental conditions, the sound and time levels associated with the nursing activities were measured in the following order: condition 1, without sound or time awareness;condition 2, with time awareness but without sound awareness;and condition 3, with both sound and time awareness. The time to perform nursing activities was longer with sound awareness. However, when aware of time only, the sound level from nursing activities rose by 2.3 dB, whereas when aware of both time and sound, the sound level dropped by 3.0 dB. With both time and sound awareness, there is a distinct drop in the sound level from nursing activities, such as wagon handling, handling of items (trays, bowls), working at the sink, and opening and closing the microwave oven door. These results suggest that even in a pseudo-clinical setting it is possible to regulate the environmental sound through the environmental sound awareness of the medical staff, resulting in a drop in the sound level generated while performing nursing activities. 展开更多

BACKGROUND Rehabilitation nursing is considered an indispensable part of the cerebral infarction treatment system.The hospital–community–family trinity rehabilitation nursing model can provide continuous nursing services across hospitals,communities,and families for patients.AIM To explore the application of a hospital–community–family rehabilitation nursing model combined with motor imagery therapy in patients with cerebral infarction.METHODS From January 2021 to December 2021,88 patients with cerebral infarction were divided into a study(n=44)and a control(n=44)group using a simple random number table.The control group received routine nursing and motor imagery therapy.The study group was given hospital–community–family trinity rehabilitation nursing based on the control group.Motor function(FMA),balance ability(BBS),activities of daily living(BI),quality of life(SS-QOL),activation status of the contralateral primary sensorimotor cortical area to the affected side,and nursing satisfaction were evaluated before and after intervention in both groups.RESULTS Before intervention,FMA and BBS were similar(P>0.05).After 6 months’intervention,FMA and BBS were significantly higher in the study than in the control group(both P<0.05).Before intervention,BI and SS-QOL scores were not different between the study and control group(P>0.05).However,after 6months’intervention,BI and SS-QOL were higher in the study than in the control group(P<0.05).Before intervention,activation frequency and volume were similar between the study and the control group(P>0.05).After 6 months’intervention,the activation frequency and volume were higher in the study than in the control group(P<0.05).The reliability,empathy,reactivity,assurance,and tangibles scores for quality of nursing service were higher in the study than in the control group(P<0.05).CONCLUSION Combining a hospital–community–family trinity rehabilitation nursing model and motor imagery therapy enhances the motor function and balance ability of patients with cerebral infarction,improving their quality of life. 展开更多

Studies have indicated that lack of regular physical exercise and not adhering to a health-promot- ing lifestyle among student nurses are barriers to their encouraging patients to exercise. On the other hand, nurses who personally embrace physical activity are more likely to encourage their patients to improve their health through physical activity or diet. As a result, promoting an active and healthy lifestyle among student nurses is important because this has a crucial role in their subsequently encouraging healthy lifestyles among their patients. The aims of this study were to determine the physical activity levels and health-promoting lifestyles among student nurses in Malaysia, and to investigate whether there was a correlation between the physical activity levels of student nurses and their lifestyle habits. Further investigation was also carried out to correlate lifestyle habits with physical activity during weekdays and at the weekends. This study involved 189 student nurses who were pursuing Diplomas and Bachelor Degrees in local government universities. The Yamax Digi-Walker pedometer was used to measure physical activity, while the Health-Promoting Lifestyle Profile (HPLP) II questionnaire was used to assess the health-pro- moting lifestyles of the participants. The HPLP II consisted of six sub-scales which included health responsibility, physical activity, nutrition, spiritual growth, interpersonal relations, and stress management. According to the results on physical activity, the student nurses had a mean of 8275.97 ± 2210.174 steps per day and were thus categorised as somewhat active. Additionally, the student nurses were found to be more active during weekends (M = 8389.75, SD = 2689.12) as compared to weekdays (M = 8230.46, SD = 2353.97). According to previous studies, both step counts are classified in the “somewhat active” category. Overall, the results for HPLP II showed that student nurses students in Malaysia had a positive health-promoting lifestyle with a score of 2.58 ± 0.34 on a 4-point Likert scale. The highest score was reported in spiritual growth, with a score of 2.92 ± 0.43. Meanwhile, physical activity was observed to be the lowest, with a score of 1.74 ± 0.39. The results found no correlation between physical activity and overall health-promoting lifestyle among student nurses in Malaysia (r = 0.06, n = 189, p < 0.37). However, there was a significant relationship between physical activity (measured by the pedometer) and physical activity habit (measured through the questionnaire) among the student nurses (r = 0.24, n = 189, p < 0.01). According to the findings, when the participants indicated in the questionnaire that they were active physically, the pedometer readings also showed a corresponding similarity in terms of their daily physical activities. 展开更多

This is often in conflict with the demands that tutors in the Bachelor of Nursing programme are required to have academic ability and clinical skills whilst at the same time, engage in meaningful research activity. This does not occur. A study of the literature finds a commonality of this dichotomy throughout the world. A question is raised as to the impact of the increasing age of the nursing workforce;does this have an impact on the develop-ment of a research culture? It is suggested that the joint project approach adopted by the WITT (Western Institute of Technology at Taranaki) School of Nursing will assist in overcoming some of the identi-fied issues and begin the development of a research culture within the School that will be ongoing. 展开更多

What made these elderly athletes different from others? Method: This was a retrospective quantitative study. The health profile data on a total of 125 elderly participants were collected. 36 and 29 were the winners and losers of the Hong Kong Athletic Gala for the Elderly between 2008 and 2010, respectively. The remaining 60 were non-participants. The non-participants came from six elderly centers in different districts in Hong Kong. Health profile parameters were measured using the Health-Promoting Lifestyle Profile II, which is used to measure healthy behaviors such as physical activity level, nutrition, stress management, health responsibility, interpersonal relations and spiritual growth. Results: About 41.7% (n = 15) and 80.0% (n = 48) of the elderly (mean age = 74 ± 9.0) in the winner and non-participant groups with chronic diseases. There were significant 展开更多

目的探讨影像科护士利用主动防范风险程序优化肺动脉血管成像检查方法及效果,分析主动防范风险护理程序提高肺动脉血管成像检查图像质量中的应用效果以及对提高检查一次性成功率及图像质量的价值。方法选取2021年1—12月在常州市中医医... 目的探讨影像科护士利用主动防范风险程序优化肺动脉血管成像检查方法及效果,分析主动防范风险护理程序提高肺动脉血管成像检查图像质量中的应用效果以及对提高检查一次性成功率及图像质量的价值。方法选取2021年1—12月在常州市中医医院影像科行肺动脉血管成像检查的98例患者作为研究对象,依据组间基线资料具有可比性原则分为对照组和观察组,各49例。对照组给予常规护理,观察组给予主动防范风险护理程序。比较两组患者心率变化、扫描时间、一次性成功率、图像质量以及不良反应发生情况。结果检查前5 min,观察组心率低于对照组,扫描时间短于对照组,一次性成功率(87.76%)则高于对照组(69.39%),差异有统计学意义(P<0.05)。观察组患者图像质量合格率(93.88%)高于对照组(79.59%),差异有统计学意义(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论主动防范风险护理程序在肺动脉血管成像检查患者中应用,可有效控制患者心率水平。在缩短检查扫描时间的同时,提高图像质量和一次性检查成功率,对优化检查效率与效果有积极作用。 展开更多

目的探讨基于护理质量敏感指标构建量化活动护理对普外科术后预防低钾血症发生率的影响。方法选取2017年9月至2021年3月普外科腹腔镜腹部手术患者400例为研究对象,按照时间顺序,将2019年9月至2021年3月普外科手术患者200例作为观察组,将... 目的探讨基于护理质量敏感指标构建量化活动护理对普外科术后预防低钾血症发生率的影响。方法选取2017年9月至2021年3月普外科腹腔镜腹部手术患者400例为研究对象,按照时间顺序,将2019年9月至2021年3月普外科手术患者200例作为观察组,将2017年9月至2019年8月普外科手术患者200例作为对照组。对照组给予常规护理,观察组给予基于护理质量敏感指标构建量化活动护理。比较2组术后恢复情况,及2组血钾浓度、疼痛评分变化情况,比较干预前后2组运动能力,比较2组低钾血症发生率情况,比较2组护理满意度。结果观察组住院时间、肛门排气时间及恢复正常饮食时间比对照组缩短(P<0.05)。与术后6 h比较,术后24、48、72 h,2组血钾浓度升高,疼痛评分降低,观察组比对照组更明显(P<0.05)。干预后,2组活动步数增多,活动距离增远,步速变快,观察组更明显(P<0.05)。观察组低钾血症发生率低于对照组,满意度高于对照组(P<0.05)。结论基于护理质量敏感指标构建量化活动护理可加快普外科患者术后恢复速度,减轻痛苦,降低术后低钾血症发生风险,极大提高患者满意度。 展开更多

目的:探讨术后早期活动量化护理干预对原发性肝癌切除术患者的影响。方法:选取2022年8月—2024年10月在广西玉林市红十字会医院接受原发性肝癌切除术的370例患者为研究对象,随机分为常规组(实施常规术后护理干预)和干预组(在常规组基础... 目的:探讨术后早期活动量化护理干预对原发性肝癌切除术患者的影响。方法:选取2022年8月—2024年10月在广西玉林市红十字会医院接受原发性肝癌切除术的370例患者为研究对象,随机分为常规组(实施常规术后护理干预)和干预组(在常规组基础上实施术后早期活动量化护理干预),每组185例。对比两组胃肠功能恢复情况、首次下床活动时间、并发症发生情况、生活质量评分。结果:干预组肠鸣音恢复时间,首次排气、排便、下床活动时间早于常规组(P<0.001);干预组并发症总发生率低于常规组(P=0.032);干预后,两组症状、躯体功能、社会功能、心理功能评分均升高,且干预组高于常规组(P<0.05)。结论:术后早期活动量化护理干预应用在原发性肝癌切除术患者中,能加快患者胃肠功能恢复进程,促进患者早期下床活动,降低并发症发生率,提高生活质量。 展开更多

目的:观察个体化运动康复护理在永久性人工心脏起搏器植入术后患者中的应用效果。方法:回顾性分析2021年7月至2023年4月该院收治的65例行永久性人工心脏起搏器植入患者的临床资料,根据护理方法不同将其分为观察组32例和对照组33例。对... 目的:观察个体化运动康复护理在永久性人工心脏起搏器植入术后患者中的应用效果。方法:回顾性分析2021年7月至2023年4月该院收治的65例行永久性人工心脏起搏器植入患者的临床资料,根据护理方法不同将其分为观察组32例和对照组33例。对照组实施常规护理,观察组在对照组基础上实施个体化运动康复护理,比较两组护理前后心功能指标[左心室射血分数(LVEF)、左心室舒张末期内径(LVEDD)]水平、6 min步行试验(6MWT)距离、日常生活能力[改良Barthel指数(MBI)]评分和生命质量[中国心血管病人生活质量评定问卷(CQQC)]评分。结果:护理后,两组LVEF水平均高于护理前,且观察组高于对照组,两组LVEDD水平均低于护理前,且观察组低于对照组,差异有统计学意义(P<0.05);护理后,两组6MWT距离均长于护理前,且观察组长于对照组,两组MBI、CQQC评分均高于护理前,且观察组高于对照组,差异有统计学意义(P<0.05)。结论:在常规护理基础上实施个体化运动康复护理可改善永久性人工心脏起搏器植入术后患者心功能指标水平,延长6MWT距离,提高生命质量和日常活动能力评分,效果优于单纯常规护理。 展开更多

目的:构建符合我国国情的高级实践护士(APN)核心置信职业行为(EPAs)框架。方法:通过文献研究法、半结构化访谈法和专家咨询法确定我国APN核心EPAs指标及描述说明,通过专家会议法确定EPAs指标对应的核心胜任力、置信级别和里程碑,从而形... 目的:构建符合我国国情的高级实践护士(APN)核心置信职业行为(EPAs)框架。方法:通过文献研究法、半结构化访谈法和专家咨询法确定我国APN核心EPAs指标及描述说明,通过专家会议法确定EPAs指标对应的核心胜任力、置信级别和里程碑,从而形成我国APN核心EPAs框架。结果:对27名专家进行2轮专家咨询,问卷有效回收率分别为90.00%和100.00%,专家权威系数分别为0.891和0.926,指标重要程度得分的肯德尔协调系数分别为0.252和0.313(均P<0.001),相关性得分的肯德尔协调系数分别为0.197和0.254(均P<0.001)。最终确定5个领域、20个EPAs指标及描述说明,并确定完成各项指标需要具备的核心胜任力、在不同年限的预期置信级别和培养过程中的里程碑。结论:本研究形成的我国APN核心EPAs框架具有科学性和可行性,未来可为我国APN的培养与评价提供参考。 展开更多
