Methods: A total of 200 patients who used intravenous indwelling needles during hospital treatment from July to October 2022 were selected and randomly divided into an observation group and a control group, with 100 patients in each group. Patients in the control group received routine nursing methods, while those in the observation group were managed using the safety risk management method of the infusion project team. The tube blocking rate and tube removal rate were compared between the two groups. Results: The one-time puncture success rate and the standard implementation rate of intravenous indwelling needle use in the observation group were significantly higher than those in the control group. The total incidence of complications in the observation group was significantly lower than that in the control group, and patient satisfaction in the observation group was notably higher, with statistically significant differences (P < 0.05). Conclusion: The application of an infusion project team in the safety risk management of indwelling needle use is effective and has positive impacts on improving the one-time puncture success rate, the standard implementation rate of intravenous indwelling needle use, and patient satisfaction. BACKGROUND Arthroscopic rotator cuff repair is a common surgical treatment for rotator cuff injuries(RCIs).Although this procedure has certain clinical advantages,it requires rehabilitation management interventions to...BACKGROUND Arthroscopic rotator cuff repair is a common surgical treatment for rotator cuff injuries(RCIs).Although this procedure has certain clinical advantages,it requires rehabilitation management interventions to ensure therapeutic efficacy.AIM To investigate the effect of integrated traditional Chinese medicine and Western medicine(TCM-WM)under the multidisciplinary team(MDT)model on the postoperative recovery of patients undergoing arthroscopic surgery for RCIs.METHODS This study enrolled 100 patients who underwent arthroscopic rotator cuff repair for RCIs at the Seventh People’s Hospital of Shanghai University of Traditional Chinese Medicine between June 2021 and May 2024.They were divided into a control group(n=48)that received routine rehabilitation treatment and an experimental group(n=52)that received TCM-WM under the MDT model(e.g.,acupuncture,TCM traumatology and orthopedics,and rehabilitation).The results of the Constant–Murley Shoulder Score(CMS),Visual Analogue Scale(VAS),Shoulder Pain and Disability Index(SPADI),muscular strength evaluation,and shoulder range of motion(ROM)assessments were analyzed.RESULTS After treatment,the experimental group showed significantly higher CMS scores in terms of pain,functional activity,shoulder joint mobility,and muscular strength than the baseline and those of the control group.The experimental group also exhibited significantly lower VAS and SPADI scores than the baseline and those of the control group.In addition,the experimental group showed significantly enhanced muscular strength(forward flexor and external and internal rotator muscles)and shoulder ROM(forward flexion,abduction,and lateral abduction)after treatment compared with the control group.CONCLUSION TCM-WM under the MDT model improved shoulder joint function,relieved postoperative pain,promoted postoperative functional recovery,and facilitated the recovery of muscular strength and shoulder ROM in patients with RCIs who underwent arthroscopic rotator cuff repair.展开更多

BACKGROUND Severe pneumonia is a common severe respiratory infection worldwide,and its treatment is challenging,especially for patients in the intensive care unit(ICU).AIM To explore the effect of communication and co...BACKGROUND Severe pneumonia is a common severe respiratory infection worldwide,and its treatment is challenging,especially for patients in the intensive care unit(ICU).AIM To explore the effect of communication and collaboration between nursing teams on the treatment outcomes of patients with severe pneumonia in ICU.METHODS We retrospectively analyzed 60 patients with severe pneumonia who were treated at the ICU of the hospital between January 1,2021 and December 31,2023.We compared and analyzed the respiratory mechanical indexes[airway resistance(Raw),mean airway pressure(mPaw),peak pressure(PIP)],blood gas analysis indexes(arterial oxygen saturation,arterial oxygen partial pressure,and oxygenation index),and serum inflammatory factor levels[C-reactive protein(CRP),procalcitonin(PCT),cortisol(COR),and high mobility group protein B1(HMGB1)]of all patients before and after treatment.RESULTS Before treatment,there was no significant difference in respiratory mechanics index and blood gas analysis index between 2 groups(P>0.05).However,after treatment,the respiratory mechanical indexes of patients in both groups were significantly improved,and the improvement of Raw,mPaw,plateau pressure,PIP and other indexes in the combined group after communication and collaboration with the nursing team was significantly better than that in the single care group(P<0.05).The serum CRP and PCT levels of patients were significantly decreased,and the difference was statistically significant compared with that of nursing group alone(P<0.05).The levels of serum COR and HMGB1 before and after treatment were also significantly decreased between the two groups.CONCLUSION The communication and collaboration of the nursing team have a significant positive impact on respiratory mechanics indicators,blood gas analysis indicators and serum inflammatory factor levels in the treatment of severe pneumonia patients in ICU.展开更多
To facilitate the implementation of controlled donation after circulatory death(cDCD)programs even in hospitals not equipped with a local Extracorporeal Membrane Oxygenation(ECMO)team(Spokes),some countries and Italia...To facilitate the implementation of controlled donation after circulatory death(cDCD)programs even in hospitals not equipped with a local Extracorporeal Membrane Oxygenation(ECMO)team(Spokes),some countries and Italian Regions have launched a local cDCD network with a ECMO mobile team who move from Hub hospitals to Spokes for normothermic regional perfusion(NRP)implantation in the setting of a cDCD pathway.While ECMO teams have been clearly defined by the Extracorporeal Life Support Organization,regarding composition,responsibilities and training programs,no clear,widely accepted indications are to date available for NRP teams.Although existing NRP mobile networks were developed due to the urgent need to increase the number of cDCDs,there is now the necessity for transplantation medicine to identify the peculiarities and responsibility of a NRP team for all those centers launching a cDCD pathway.Thus,in the present manuscript we summarized the character-istics of an ECMO mobile team,highlighting similarities and differences with the NRP mobile team.We also assessed existing evidence on NRP teams with the goal of identifying the characteristic and essential features of an NRP mobile team for a cDCD program,especially for those centers who are starting the program.Differences were identified between the mobile ECMO team and NRP mobile team.The common essential feature for both mobile teams is high skills and experience to reduce complications and,in the case of cDCD,to reduce the total warm ischemic time.Dedicated training programs should be developed for the launch of de novo NRP teams.展开更多

Purpose:Our institution developed the subspecialized medical team(SMT)mode in radiology reporting workflows in July 2022 to strengthen profes-sionalism in radiology residency training.This study aimed to investigate w...Purpose:Our institution developed the subspecialized medical team(SMT)mode in radiology reporting workflows in July 2022 to strengthen profes-sionalism in radiology residency training.This study aimed to investigate whether adopting the SMT mode facilitated radiology resident training.Method:This observational study was conducted in October 2022.An online questionnaire was distributed among first-to third-year radiology residents to determine their levels of perceived stress,anxiety,competency in image interpretation,and learning effectiveness in using the SMT mode and other approaches.Responses were on 4 or 5 point Likert scales,and multiple-choice responses were used for semi-open questions.The turnaround time for radi-ology reports was measured for both residents and senior radiologists.Sta-tistical significance(p<0.05)was determined using chi-square or Fisher's exact tests,one-way analysis of variance,pairwise t-tests,and the Kruskal-Wallis H test.Results:In total,71 residents completed the questionnaire survey.The turnaround times for radiology reports were collected for these residents and 23 senior radiologists.Residents'stress and anxiety scores,perceived levels of competency in image interpretation,and time spent writing reports were significantly increased in the SMT mode.In contrast,the time senior radiol-ogists spent reviewing a single report was decreased for the majority of report types after 1 year of implementing the SMT mode.Conclusion:These findings suggest adopting the SMT mode could facilitate the training of radiology residents.However,residents'mental health and well-being should also be considered.展开更多

目的:探究多学科诊疗模式(multidisciplinary team,MDT)在慢性难愈合创面诊疗中的应用成效,为优化此类疾病的临床诊疗提供新思路。方法:回顾性分析2015年1月至2023年10月在北京大学第三医院接受手术治疗的慢性难愈合创面患者的临床资料...目的:探究多学科诊疗模式(multidisciplinary team,MDT)在慢性难愈合创面诊疗中的应用成效,为优化此类疾病的临床诊疗提供新思路。方法:回顾性分析2015年1月至2023年10月在北京大学第三医院接受手术治疗的慢性难愈合创面患者的临床资料,共纳入患者456例,包括男性290例,女性166例,平均年龄(49.4±16.9)岁。根据是否进行术前MDT讨论,将患者分为MDT讨论组和非MDT讨论组。MDT总体执行流程包括:启动与医务处备案、收集资料与初次MDT讨论、告知患者诊疗方案并严格执行、病情变化需再次MDT讨论。比较两组患者的一般临床资料、麻醉风险分级、合并症(高血压、糖尿病、冠心病)、慢性难愈合创面的发生病因与部位等资料的差异。治疗效果的主要观察与结局指标包括入院后达到创面愈合所需的手术次数、创面愈合后的复发率、围手术期并发症(肺部感染、严重心血管事件、静脉血栓栓塞症、脑卒中及谵妄等)的发生率、患者满意度评分等。结果:MDT讨论组患者189例,非MDT讨论组患者267例,两组患者的年龄、性别、体重指数、美国麻醉医师协会(American Society of Anesthesiologists,ASA)分级、合并症、慢性难愈合创面的病因与部位等临床资料差异无统计学意义(P>0.05)。MDT讨论组与非MDT讨论组患者获得创面愈合所需的平均手术次数分别为(2.1±1.1)次与(2.8±1.6)次,差异有统计学意义(P<0.001),这种差异在糖尿病溃疡、外伤或手术后感染、放疗后不愈合等3个病因造成的慢性难愈合创面中也有统计学意义(P<0.05)。非MDT讨论组患者在创面痊愈后的复发率是18.0%,稍高于MDT讨论组的14.3%,但差异无统计学意义(P>0.05)。在围手术期并发症方面,非MDT讨论组的发生率也较MDT讨论组高(3.7%vs.2.6%),但差异无统计学意义(P>0.05)。患者满意度方面,MDT讨论组的评分显著高于非MDT讨论组,差异有统计学意义(96.5 vs.91.1,P=0.028)。结论:多学科诊疗模式能够显著减少慢性难愈合创面患者的住院手术次数,提升治愈效率,提高患者满意度,是优化慢性难愈合创面临床诊疗成效的推荐模式。展开更多

文摘With the advancement of the“Double First-Class”initiative,universities are increasingly focusing on attracting top-tier talent to enhance faculty quality and strengthen academic disciplines.The blind competition for high-end talent among universities reflects an overshadowing of academic values by economic considerations,neglecting the growth patterns of top talent and the development laws of higher education.This study investigates and reflects on the real-world challenges of faculty mobility in universities,aiming to provide effective guidance for managing university faculty teams.
文摘In the new era,the team of college counselors,as an indispensable and important component of the higher education system,bears multiple responsibilities such as ideological and political education,daily student management,mental health counseling,and employment and entrepreneurship guidance.With the rapid development of society and the diversified transformation of student groups,the construction of the counselor team is facing new challenges and opportunities.In-depth research on the optimization path of the construction of the college counselor team is not only significant for improving the overall quality and work efficiency of the counselor team but also directly related to the completion effect of the fundamental task of cultivating morality and talents in colleges and universities,as well as the achievement of the goal of cultivating more high-quality talents for the country.Based on this and the author’s work experience,this paper first elaborates on the necessity of the construction of the college counselor team,and then conducts a deep analysis of its optimization path and strategy,aiming to provide a useful reference for the professionalization and professional development of the college counselor team,thereby promoting the sustainable development of higher education and the comprehensive growth of students.

文摘Purpose:Our institution developed the subspecialized medical team(SMT)mode in radiology reporting workflows in July 2022 to strengthen profes-sionalism in radiology residency training.This study aimed to investigate whether adopting the SMT mode facilitated radiology resident training.Method:This observational study was conducted in October 2022.An online questionnaire was distributed among first-to third-year radiology residents to determine their levels of perceived stress,anxiety,competency in image interpretation,and learning effectiveness in using the SMT mode and other approaches.Responses were on 4 or 5 point Likert scales,and multiple-choice responses were used for semi-open questions.The turnaround time for radi-ology reports was measured for both residents and senior radiologists.Sta-tistical significance(p<0.05)was determined using chi-square or Fisher's exact tests,one-way analysis of variance,pairwise t-tests,and the Kruskal-Wallis H test.Results:In total,71 residents completed the questionnaire survey.The turnaround times for radiology reports were collected for these residents and 23 senior radiologists.Residents'stress and anxiety scores,perceived levels of competency in image interpretation,and time spent writing reports were significantly increased in the SMT mode.In contrast,the time senior radiol-ogists spent reviewing a single report was decreased for the majority of report types after 1 year of implementing the SMT mode.Conclusion:These findings suggest adopting the SMT mode could facilitate the training of radiology residents.However,residents'mental health and well-being should also be considered.
文摘Objective:This study assessed the role of the attending medical team in the cost control of the cholecystectomy DRG components.Methods:The association between team structure,workflow,and treatment outcomes was analyzed using a mixed-methods approach combining quantitative data and qualitative interviews from 628 patients.Results:Inter-professional teamwork significantly affected length of stay,treatment costs,and recurrence rates,with experienced teams performing better in terms of emergency response and collaborative efficiency.Patient satisfaction was generally high,indicating that good teamwork enhances treatment outcomes.Significance:The study highlights the importance of optimizing team configuration to improve the quality,efficiency,and cost control of healthcare.

文摘This study is dedicated to exploring the role of scientific research team construction and collaborative management mechanisms in applied undergraduate universities in promoting diversified talent training and collaborative innovation models.By forming an interdisciplinary scientific research team and recording in detail the team’s performance,problems encountered,and solutions during the collaborative innovation process,the study found that interdisciplinary team construction significantly improved scientific research timeliness,which was 29.33%higher than that of traditional single-disciplinary teams.Therefore,the construction of interdisciplinary scientific research teams and collaborative management mechanisms are effective ways for applied undergraduate universities to promote diversified talent training and collaborative innovation.
文摘目的:探究多学科诊疗模式(multidisciplinary team,MDT)在慢性难愈合创面诊疗中的应用成效,为优化此类疾病的临床诊疗提供新思路。方法:回顾性分析2015年1月至2023年10月在北京大学第三医院接受手术治疗的慢性难愈合创面患者的临床资料,共纳入患者456例,包括男性290例,女性166例,平均年龄(49.4±16.9)岁。根据是否进行术前MDT讨论,将患者分为MDT讨论组和非MDT讨论组。MDT总体执行流程包括:启动与医务处备案、收集资料与初次MDT讨论、告知患者诊疗方案并严格执行、病情变化需再次MDT讨论。比较两组患者的一般临床资料、麻醉风险分级、合并症(高血压、糖尿病、冠心病)、慢性难愈合创面的发生病因与部位等资料的差异。治疗效果的主要观察与结局指标包括入院后达到创面愈合所需的手术次数、创面愈合后的复发率、围手术期并发症(肺部感染、严重心血管事件、静脉血栓栓塞症、脑卒中及谵妄等)的发生率、患者满意度评分等。结果:MDT讨论组患者189例,非MDT讨论组患者267例,两组患者的年龄、性别、体重指数、美国麻醉医师协会(American Society of Anesthesiologists,ASA)分级、合并症、慢性难愈合创面的病因与部位等临床资料差异无统计学意义(P>0.05)。MDT讨论组与非MDT讨论组患者获得创面愈合所需的平均手术次数分别为(2.1±1.1)次与(2.8±1.6)次,差异有统计学意义(P<0.001),这种差异在糖尿病溃疡、外伤或手术后感染、放疗后不愈合等3个病因造成的慢性难愈合创面中也有统计学意义(P<0.05)。非MDT讨论组患者在创面痊愈后的复发率是18.0%,稍高于MDT讨论组的14.3%,但差异无统计学意义(P>0.05)。在围手术期并发症方面,非MDT讨论组的发生率也较MDT讨论组高(3.7%vs.2.6%),但差异无统计学意义(P>0.05)。患者满意度方面,MDT讨论组的评分显著高于非MDT讨论组,差异有统计学意义(96.5 vs.91.1,P=0.028)。结论:多学科诊疗模式能够显著减少慢性难愈合创面患者的住院手术次数,提升治愈效率,提高患者满意度,是优化慢性难愈合创面临床诊疗成效的推荐模式。
