Sepsis was induced...To study the redistribution of endothelin- 1 (ET- 1) receptors in two subcellular organelles , the sarcolemmal membrane and the light vesicle, of rat heart during the progression of sepsis. Methods. Sepsis was induced by cecal ligation and puncture (CLP). ET1 receptor was assayed by using [125I]- ET1 binding. Marker enzyme activities, protein yield, and dry- to- wet weight ratio of cardiac membranes were measured. Results. Septic rat heart exhibited two distinct phases: an initial hyperdynamic phase( 9h after CLP; early stage of sepsis) followed by a hypodynamic (18h after CLP, late stage of sepsis) phase. [125I]- ET1 binding study showed that during early stage of sepsis, the Bmax of ET1 receptors was increased by 30% in sarcolemma but decreased by 19% in light vesicles, while during late stage of sepsis, the Bmax was decreased by 24% in sarcolemma but increased by 38% in light vesicles.The total binding of sarcolemma and light vesicles was increased by 25% during early stage of sepsis but decreased by 17% during late stage of sepsis. Conclusions. These data indicated that ET1 receptors in the rat heart were externalized from light vesicles to sarcolemmal membranes during early hyperdynamic phase while internalized from surface membranes to intracellular compartment during late hypodynamic phase of sepsis.展开更多

Cases utilizing ‘‘Active Greeting Technique(AGT)", a mother-and-child catheter based technique to facilitate retrograde wire externalization, were extracted from Chronic Total Occlusion Club, China(CTOCC) database. AGT was performed by deep intubation a mother-and-child catheter(GuidezillaTMextension, 4 or 5 Fr inner catheter, and etc.) in combination with either reverse controlled antegrade or retrograde subintimal tracking(CART) technique or retrograde wire crossing technique. A total of 111 patients with 112 CTO lesions treated with this technique were retrospectively analyzed. Reverse CART technique and retrograde wire crossing technique were performed in 90.2% and 9.8% of all procedures. The utilization of GuidezillaTMextension, 4 Fr, and 5 Fr inner catheter accounted for 94.6%, 3.6%, and 1.8%, respectively. Externalization of retrograde wire was successful in all cases. No procedural complications were adjudicated to AGT. Complications independent of AGT included two target vessel perforations and two collateral perforations. No in-hospital major adverse cardiac events were found. AGT is a feasible and safe technique that facilitates retrograde wire externalization.展开更多

BACKGROUND Pain in the back or pelvis or fear of back pain may affect the timing or cocontraction of the core muscles.In both static and dynamic movements,the Sahrmann core stability test provides an assessment of cor...BACKGROUND Pain in the back or pelvis or fear of back pain may affect the timing or cocontraction of the core muscles.In both static and dynamic movements,the Sahrmann core stability test provides an assessment of core muscle activation and a person's ability to stabilize the lumbopelvic complex.Preparatory cues and images can be used to increase the activation of these muscles.To attain optimal movement patterns,it will be necessary to determine what cueing will give the most effective results for core stability.AIM To investigate the effects of external and internal cues on core muscle activation during the Sahrmann five-level core stability test.METHODS Total 68 participants(21.83±3.47 years)were randomly allocated to an external(n=35)or internal cue group(n=33).Participants performed the Sahrmann fivelevel core stability test without a cue as baseline and the five-level stability exercises with an internal or external cue.External cue group received a pressure biofeedback unit(PBU),and the internal cue group received an audio cue.A Delsys Trigno^(TM)surface electromyography unit was used for muscle activation from the rectus abdominis,external oblique,and transverse abdominis/internal oblique muscles.RESULTS Linear mixed effects model analysis showed that cueing had a significant effect on core muscle activation(P=0.001);however,there was no significant difference between cue types(internal or external)(P=0.130).CONCLUSION Both external and internal cueing have significant effects on core muscle activation during the Sahrmann five-level core stability test and the PBU does not create higher muscle activation than internal cueing.展开更多

To ad- dress this problem, we propose an effective sound image externalization approach. Specifically, we consider several important factors related to sound propagation, which include image-source model based early reflections with distance decay, wall absorption and air absorption, late reverberation and other dynamic factors like head movement. We apply our sound image externalization approach to a headphone based real-time 3D audio system. Subjective listening tests show that the sound image externalization performance is significantly improved and the sound source direction is preserved as well. A/B preference test further shows that, as compared with a recent popular approach, the proposed approach is mostly preferred by the listeners.展开更多

This study developed an animal model with internal and external urethral sphincter insufficiency by bypassing the sphincter without major damage so that the animal under study can return to normal life after the study...This study developed an animal model with internal and external urethral sphincter insufficiency by bypassing the sphincter without major damage so that the animal under study can return to normal life after the study.There is a need for a reliable,applicable,and reproducible animal model for studying urinary incontinency disease due to incorrect sphincter function.Seven adult male dogs were used for this study.The urethral sphincter was bypassed by inserting a catheter between the bladder neck and the distal sphincter.The animals'physical condition was closely monitored for 9 weeks,and standard urodynamic and radiologic studies were performed before and 1–2 months after surgery.The animals were killed at 9 weeks after surgery for pathological assessment.Catheter placement caused complete incontinence in the animal,with urodynamic assessments indicating that the animal was unable to control urination and radiological assessments indicating an empty bladder with a residual volume of 50±10 cc.Tissue analysis did not show significant histological damage and inflammation.The study shows that by bypassing the urethral sphincter,which is a reliable and reproducible method,an animal model of urinary incontinence can be developed,which can be used in various studies such as assessing the adequacy of artificial sphincter function.The animals under study did not have any permanent defect,so they were able to return to their normal life.展开更多

Exis...In the age of information explosion and artificial intelligence, sentiment analysis tailored for the tobacco industry has emerged as a pivotal avenue for cigarette manufacturers to enhance their tobacco products. Existing solutions have primarily focused on intrinsic features within consumer reviews and achieved significant progress through deep feature extraction models. However, they still face these two key limitations: (1) neglecting the influence of fundamental tobacco information on analyzing the sentiment inclination of consumer reviews, resulting in a lack of consistent sentiment assessment criteria across thousands of tobacco brands;(2) overlooking the syntactic dependencies between Chinese word phrases and the underlying impact of sentiment scores between word phrases on sentiment inclination determination. To tackle these challenges, we propose the External Knowledge-enhanced Cross-Attention Fusion model, CITSA. Specifically, in the Cross Infusion Layer, we fuse consumer comment information and tobacco fundamental information through interactive attention mechanisms. In the Textual Attention Enhancement Layer, we introduce an emotion-oriented syntactic dependency graph and incorporate sentiment-syntactic relationships into consumer comments through a graph convolution network module. Subsequently, the Textual Attention Layer is introduced to combine these two feature representations. Additionally, we compile a Chinese-oriented tobacco sentiment analysis dataset, comprising 55,096 consumer reviews and 2074 tobacco fundamental information entries. Experimental results on our self-constructed datasets consistently demonstrate that our proposed model outperforms state-of-the-art methods in terms of accuracy, precision, recall, and F1-score.展开更多

When acute fluid collection persists for 4 - 6 weeks and is encapsulated by a fibrous wall, it is classified as a pancreatic pseudocyst. While PPC is generally asymptomatic in many patients, it can manifest with persistent abdominal pain, dyspepsia, intra-cystic infection, and potentially lead to gastrointestinal obstruction in some cases. Although smaller PPCs may resolve spontaneously, larger PPCs tend to be refractory to absorption and often necessitate surgical intervention to prevent complications such as intracystic hemorrhage. Objective: To explore the efficacy of percutaneous catheterization with negative pressure in the treatment of large pancreatic pseudocysts. Methods: The cases of large pancreatic pseudocysts treated in our hospital from 2004 to 2022 were retrospectively collected, and the general condition, operation time, drainage time, feeding time, postoperative complications, hospital stay, cost and follow-up of the patients were analyzed. Results: A total of 132 patients with large pancreatic pseudocysts were collected. The average operation time was 32.4 ± 2.1 min;The retention time of the drainage tube was 30 ± 1.8 days in the percutaneous negative pressure drainage group;The postoperative feeding time was no fasting after local anesthesia drainage;Postoperative complications (bleeding, infection, pancreatic leakage, recurrence, anastomotic leakage, etc.): Two of the 132 patients had recurrent cysts, which were cured by re-puncture and negative pressure drainage. The length of hospital stay was 6 ± 1.1 days;The cost was 11,200 ± 1300 yuan;Follow-up: The follow-up time ranged from 1 to 3 years, and the patients had no discomfort. Conclusion: Percutaneous catheterization and negative pressure drainage can effectively treat large pancreatic pseudocysts. Compared with other treatment methods, it is simple and effective, the postoperative recovery of patients is faster, the physical damage is less, the hospital stay is shorter, and the cost is lower.展开更多
[Objectives]To investigate the efficacy and mechanisms of action of the Dangshen Xiangfu Chinese herbal patch through the application of network pharmacology.[Methods]The HIT 2.0 database and the TCMSP database were u...[Objectives]To investigate the efficacy and mechanisms of action of the Dangshen Xiangfu Chinese herbal patch through the application of network pharmacology.[Methods]The HIT 2.0 database and the TCMSP database were utilized to identify and gather the active ingredients and corresponding action targets of the Dangshen Xiangfu Chinese herbal patch.Cytoscape software was utilized to create network diagrams and analyze the primary active ingredients and action targets of the herbal patch formulation.Gene Ontology(GO)functional analysis and Kyoto Encyclopedia of Genes and Genomes(KEGG)enrichment analysis of the identified action targets were conducted using the DAVID database.The SymMap database was utilized to analyze the network information pertaining to traditional Chinese medicine(TCM),syndromes and the associated symptoms of the medicinal plants found in the Chinese herbal patch.[Results]The network pharmacological analysis suggested that the primary Chinese herbal ingredients of the Dangshen Xiangfu Chinese herbal patch may include Radix Glycyrrhizae Preparata,Fructus Aurantii,Cyperi Rhizoma,Radix Codonopsis,etc.The primary active ingredients may include quercetin,luteolin,kaempferol,fisetin,nobiletin,naringenin,isosinensetin,baicalin,etc.These ingredients exhibited a protective effect on the intestinal tract and were advantageous in the regulation of abnormal intestinal function.The primary targets of action included PTGS2,CASP3,HSP90AA1,PTGS1,AKT1,and MMP9.The active ingredients of the Dangshen Xiangfu Chinese herbal patch may exert a regulatory effect on gastrointestinal inflammation by interacting with the pertinent targets within the PI3K-Akt signaling pathway,the IL-17 signaling pathway,and other related pathways in the body.The principal symptoms associated with this condition may include qi stagnation,qi deficiency,and food accumulation.[Conclusions]This study offers a framework for analyzing the efficacy and mechanisms of action of external preparations derived from medicinal plants.展开更多

The purpose of this study is to analyze the clinical effect of external application of Granulation-Promoting Jade and Red paste in the treatment of diabetic foot.This study was carried out in Pingdingshan Traditional ...The purpose of this study is to analyze the clinical effect of external application of Granulation-Promoting Jade and Red paste in the treatment of diabetic foot.This study was carried out in Pingdingshan Traditional Chinese Medicine Hospital from December 2023 to December 2024.A total of 80 patients were selected as research objects,all of whom were diagnosed diabetic foot patients.They were treated and divided into two groups based on different methods,the group names were“experimental group”and“control group”.Patients in the control group were treated with Vaseline on the basis of external treatment,and patients in the experimental group were treated with Granulation-Promoting Jade and Red paste on the basis of external treatment,and the clinical effects of different treatment methods were compared and analyzed.The results showed that the effective rate in the experimental group was significantly higher(95.00%)compared to the control group(75.00%)(P<0.05).Furthermore,the experimental group exhibited notably shorter wound healing time and granulation tissue growth time,indicating faster and better recovery(P<0.05).Additionally,post-treatment Wagner grading revealed that the proportion of patients in advanced grades(4,5,and 6)was significantly lower in the experimental group,whereas a higher proportion of patients achieved lower grades(0,1,and 2),demonstrating improved clinical outcomes(P<0.05).In conclusion,Granulation-Promoting Jade and Red Paste shows a significant therapeutic effect in treating diabetic foot,effectively promoting wound healing,improving clinical symptoms,and enhancing overall recovery.展开更多
This paper aims to enhance the compression capacity of underwater cylindrical shells by adopting the corrugated sandwich structure of cuttlebone.The cuttlebone suffers uniaxial external compression,while underwater cy...This paper aims to enhance the compression capacity of underwater cylindrical shells by adopting the corrugated sandwich structure of cuttlebone.The cuttlebone suffers uniaxial external compression,while underwater cylindrical shells are in a biaxial compressive stress state.To suit the biaxial compressive stress state,a novel bidirectional corrugated sandwich structure is proposed to improve the bearing capacity of cylindrical shells.The static and buckling analysis for the sandwich shell and the unstiffened cylindrical shell with the same volume-weight ratio are studied by numerical simulation.It is indicated that the proposed sandwich shell can effectively reduce the ratio between circumferential and axial stress from 2 to 1.25 and improve the critical buckling load by about 1.63 times.Numerical simulation shows that optimizing and adjusting the structural parameters could significantly improve the advantage of the sandwich shell.Then,the hydrostatic pressure tests for shell models fabricated by 3D printing are carried out.According to the experimental results,the overall failure position of the sandwich shell is at the center part of the sandwich shell.It has been found the average critical load of the proposed sandwich shell models exceeds two times that of the unstiffened shell models.Hence,the proposed bio-inspired bidirectional corrugated sandwich structure can significantly enhance the pressure resistance capability of cylindrical shells.展开更多
In order to obtain the transient characteristics of a low-speed centrifugal pump during the start-up and shutdown stages,dedicated experimental tests were conducted with eight different valve opening conditions.The Pe...In order to obtain the transient characteristics of a low-speed centrifugal pump during the start-up and shutdown stages,dedicated experimental tests were conducted with eight different valve opening conditions.The Pearson correlation coefficient was used to reveal the linear correlation between variables.According to the results,the stable rotational speed decreases with increasing valve opening(rotational speed decreases from approximately 1472 to 1453 r/min),while the stable shaft power exhibits an increasing trend(shaft power increases from approximately 0.242 to 0.390 kW).The stable time and zeroing time of each parameter during start-up and shutdown processes vary,with the flow zeroing time significantly increasing with the relative flow,reaching up to 10.468 s,while the shaft power zeroing time is roughly between 1.219 and 1.375 s.The results demonstrate that with increasing valve opening,the stable and zeroing time of flow significantly increase(|r|greater than 0.95),while the stable and zeroing time of rotational speed,power,and head display a smaller sensitivity on the valve opening(|r|less than 0.6).展开更多
The effect of slow shot speed on externally solidified crystal(ESC),porosity and tensile property in a newly developed high-pressure die-cast Al-Si alloy was investigated by optical microscopy(OM),scanning electron mi...The effect of slow shot speed on externally solidified crystal(ESC),porosity and tensile property in a newly developed high-pressure die-cast Al-Si alloy was investigated by optical microscopy(OM),scanning electron microscopy(SEM)and laboratory computed tomography(CT).Results showed that the newly developed AlSi9MnMoV alloy exhibited improved mechanical properties when compared to the AlSi10MnMg alloy.The AlSi9MnMoV alloy,which was designed with trace multicomponent additions,displays a notable grain refining effect in comparison to the AlSi10MnMg alloy.Refining elements Ti,Zr,V,Nb,B promote heterogeneous nucleation and reduce the grain size of primaryα-Al.At a lower slow shot speed,the large ESCs are easier to form and gather,developing into the dendrite net and net-shrinkage.With an increase in slow shot speed,the size and number of ESCs and porosities significantly reduce.In addition,the distribution of ESCs is more dispersed and the net-shrinkage disappears.The tensile property is greatly improved by adopting a higher slow shot speed.The ultimate tensile strength is enhanced from 260.31 MPa to 290.31 MPa(increased by 11.52%),and the elongation is enhanced from 3.72%to 6.34%(increased by 70.52%).展开更多
BACKGROUND Primary liver cancer is a malignant tumor with a high recurrence rate that significantly affects patient prognosis.Postoperative adjuvant external radiation therapy(RT)has been shown to effectively prevent ...BACKGROUND Primary liver cancer is a malignant tumor with a high recurrence rate that significantly affects patient prognosis.Postoperative adjuvant external radiation therapy(RT)has been shown to effectively prevent recurrence after liver cancer resection.However,there are multiple RT techniques available,and the differ-ential effects of these techniques in preventing postoperative liver cancer re-currence require further investigation.AIM To assess the advantages and disadvantages of various adjuvant external RT methods after liver resection based on overall survival(OS)and disease-free survival(DFS)and to determine the optimal strategy.METHODS This study involved network meta-analyses and followed the PRISMA guidelines.The data of qualified studies published before July 10,2023,were collected from PubMed,Embase,the Web of Science,and the Cochrane Library.We included relevant studies on postoperative external beam RT after liver resection that had OS and DFS as the primary endpoints.The magnitudes of the effects were determined using risk ratios with 95%confidential intervals.The results were analyzed using R software and STATA software.RESULTS A total of 12 studies,including 1265 patients with hepatocellular carcinoma(HCC)after liver resection,were included in this study.There was no significant heterogeneity in the direct paired comparisons,and there were no significant differences in the inclusion or exclusion criteria,intervention measures,or outcome indicators,meeting the assumptions of heterogeneity and transitivity.OS analysis revealed that patients who underwent stereotactic body radiotherapy(SBRT)after resection had longer OS than those who underwent intensity modulated radiotherapy(IMRT)or 3-dimensional conformal RT(3D-CRT).DFS analysis revealed that patients who underwent 3D-CRT after resection had the longest DFS.Patients who underwent IMRT after resection had longer OS than those who underwent 3D-CRT and longer DFS than those who underwent SBRT.CONCLUSION HCC patients who undergo liver cancer resection must consider distinct advantages and disadvantages when choosing between SBRT and 3D-CRT.IMRT,a RT technique that is associated with longer OS than 3D-CRT and longer DFS than SBRT,may be a preferred option.展开更多

文摘To study the redistribution of endothelin- 1 (ET- 1) receptors in two subcellular organelles , the sarcolemmal membrane and the light vesicle, of rat heart during the progression of sepsis. Methods. Sepsis was induced by cecal ligation and puncture (CLP). ET1 receptor was assayed by using [125I]- ET1 binding. Marker enzyme activities, protein yield, and dry- to- wet weight ratio of cardiac membranes were measured. Results. Septic rat heart exhibited two distinct phases: an initial hyperdynamic phase( 9h after CLP; early stage of sepsis) followed by a hypodynamic (18h after CLP, late stage of sepsis) phase. [125I]- ET1 binding study showed that during early stage of sepsis, the Bmax of ET1 receptors was increased by 30% in sarcolemma but decreased by 19% in light vesicles, while during late stage of sepsis, the Bmax was decreased by 24% in sarcolemma but increased by 38% in light vesicles.The total binding of sarcolemma and light vesicles was increased by 25% during early stage of sepsis but decreased by 17% during late stage of sepsis. Conclusions. These data indicated that ET1 receptors in the rat heart were externalized from light vesicles to sarcolemmal membranes during early hyperdynamic phase while internalized from surface membranes to intracellular compartment during late hypodynamic phase of sepsis.

文摘Although retrograde approach has greatly improved the success rate of percutaneous coronary intervention(PCI) for coronary chronic total occlusion(CTO), retrograde wire externalization still remains challenging and time-consuming in some cases. Cases utilizing ‘‘Active Greeting Technique(AGT)", a mother-and-child catheter based technique to facilitate retrograde wire externalization, were extracted from Chronic Total Occlusion Club, China(CTOCC) database. AGT was performed by deep intubation a mother-and-child catheter(GuidezillaTMextension, 4 or 5 Fr inner catheter, and etc.) in combination with either reverse controlled antegrade or retrograde subintimal tracking(CART) technique or retrograde wire crossing technique. A total of 111 patients with 112 CTO lesions treated with this technique were retrospectively analyzed. Reverse CART technique and retrograde wire crossing technique were performed in 90.2% and 9.8% of all procedures. The utilization of GuidezillaTMextension, 4 Fr, and 5 Fr inner catheter accounted for 94.6%, 3.6%, and 1.8%, respectively. Externalization of retrograde wire was successful in all cases. No procedural complications were adjudicated to AGT. Complications independent of AGT included two target vessel perforations and two collateral perforations. No in-hospital major adverse cardiac events were found. AGT is a feasible and safe technique that facilitates retrograde wire externalization.

文摘High pressure die casting(HPDC)AlSi10Mn Mg alloy castings are widely used in the automobile industry.Mg can optimize the mechanical properties of castings through heat treatment,while the release of thermal stress arouses the deformation of large integrated die-castings.Herein,the development of non-heat treatment Al alloys is becoming the hot topic.In addition,HPDC contains externally solidified crystals(ESCs),which are detrimental to the mechanical properties of castings.To achieve high strength and toughness of non-heat treatment die-casting Al-Si alloy,we used AlSi9Mn alloy as matrix with the introduction of Zr,Ti,Nb,and Ce.Their influences on ESCs and mechanical properties were systematically investigated through three-dimensional reconstruction and thermodynamic simulation.Our results reveal that the addition of Ti increased ESCs'size and porosity,while the introduction of Nb refined ESCs and decreased porosity.Meanwhile,large-sized Al_3(Zr,Ti)phases formed and degraded the mechanical properties.Subsequent introduction of Ce resulted in the poisoning effect and reduced mechanical properties.

文摘The paper presents experimental investigation results of crack pattern change in cement pastes caused by external sulfate attack(ESA).To visualize the formation and development of cracks in cement pastes under ESA,an X-ray computed tomography(X-ray CT)was used,i e,the tomography system of Zeiss Xradia 510 versa.The results indicate that X-CT can monitor the development process and distribution characteristics of the internal cracks of cement pastes under ESA with attack time.In addition,the C3A content in the cement significantly affects the damage mode of cement paste specimens during sulfate erosion.The damage of ordinary Portland cement(OPC)pastes subjected to sulfate attack with high C3A content are severe,while the damage of sulfate resistant Portland cement(SRPC)pastes is much smaller than that of OPC pastes.Furthermore,a quadratic function describes the correlation between the crack volume fraction and development depth for two cement pastes immermed in sulfate solution.
文摘BACKGROUND Pain in the back or pelvis or fear of back pain may affect the timing or cocontraction of the core muscles.In both static and dynamic movements,the Sahrmann core stability test provides an assessment of core muscle activation and a person's ability to stabilize the lumbopelvic complex.Preparatory cues and images can be used to increase the activation of these muscles.To attain optimal movement patterns,it will be necessary to determine what cueing will give the most effective results for core stability.AIM To investigate the effects of external and internal cues on core muscle activation during the Sahrmann five-level core stability test.METHODS Total 68 participants(21.83±3.47 years)were randomly allocated to an external(n=35)or internal cue group(n=33).Participants performed the Sahrmann fivelevel core stability test without a cue as baseline and the five-level stability exercises with an internal or external cue.External cue group received a pressure biofeedback unit(PBU),and the internal cue group received an audio cue.A Delsys Trigno^(TM)surface electromyography unit was used for muscle activation from the rectus abdominis,external oblique,and transverse abdominis/internal oblique muscles.RESULTS Linear mixed effects model analysis showed that cueing had a significant effect on core muscle activation(P=0.001);however,there was no significant difference between cue types(internal or external)(P=0.130).CONCLUSION Both external and internal cueing have significant effects on core muscle activation during the Sahrmann five-level core stability test and the PBU does not create higher muscle activation than internal cueing.
文摘3D audio effects can provide immersive auditory experience, but we often face the so-called in-head localization (IHL) problem in headphone sound reproduction. To ad- dress this problem, we propose an effective sound image externalization approach. Specifically, we consider several important factors related to sound propagation, which include image-source model based early reflections with distance decay, wall absorption and air absorption, late reverberation and other dynamic factors like head movement. We apply our sound image externalization approach to a headphone based real-time 3D audio system. Subjective listening tests show that the sound image externalization performance is significantly improved and the sound source direction is preserved as well. A/B preference test further shows that, as compared with a recent popular approach, the proposed approach is mostly preferred by the listeners.

文摘The metamaterial based on external meshing gears(MEG)is designed based on the principle of external meshing gear transmission.Based on the meshing transmission principle of external meshing gears and planetary gear trains,the internal and external gear rings are designed.Based on the internal and external gear rings,the metamaterial based on inner and outer planetary gear trains(MIP)is designed to study the shear modulus,Young's modulus,and amplitude-frequency characteristics of the metamaterial based on gears at different angles.The effects of the number of planetary gears on the physical characteristics of the MIP are studied.The results show that the MEG can be continuously adjusted by adjusting the shear modulus and Young's modulus due to its meshing characteristics.With the same number of gears,the adjustment range of the MIP is larger than the adjustment range of the MEG.When the number of planetary gears increases,the adjustment range of the MIP decreases.Moreover,when the metamaterial based on gears rotates,the harmonic response changes with the change of the angle.
文摘Objective To study the redistribution of ET1 receptors in two subcellular organelles, the sarcolemmal membrane and the light vesicle, of rat heart during the progress of septic shock Methods Male Sprague Dawley rats weighing from 270 to 320?g were randomly divided into three groups: control, early sepsis, and late sepsis Each group included six rats Sepsis was induced by cecal ligation and puncture (CLP) Control rats were sham operated After operation for 9 hours or 18 hours, animals of the three groups were anesthetized with sodium pentobarbital (60?mg/kg IP) and the hearts were removed for preparation of sarcolemma and light vesicle Hemodynamic parameters were determined with polygraph via femoral artery and intraventricular cannula ET1 receptor was assayed by [ 125 I] ET1 binding Results Heart rate, cardiac output and left ventricular +dp/dt max undergo biphasic changes: an increase in early phase of sepsis (9?h after CLP) followed by a decrease in late phase of sepsis (18?h after CLP) Mean arterial blood pressure and left ventricular dp/dt max remained relatively unaltered during early phase of sepsis but was decreased during late phase of sepsis Although septic rat heart exhibited biphasic cardiodynamic changes, myocardial function showed signs of progressive deterioration during the development of sepsis, as indicated by a progressive elevation of LVEDP [ 125 I] ET1 bindings to cardiac membranes exhibited a saturable process with a single component binding characteristic for all three experimental groups In sarcolemmal membrane fraction, the maximum binding capacity (B max ) calculated from scatchard plot was increased 30% ( P <0 01) during early phase of sepsis but decreased 24% ( P <0 01) during late phase of sepsis The affinity [the reciprocal of the dissociation contant (Kd)] for [ 125 I] ET1 binding in sarcolemmal membranes remained unaffected during early and late phases of sepsis In light vesicle fraction, the B max for [ 125 I] ET1 binding was decreased by 19% ( P <0 05) during early phase of sepsis but increased by 38% ( P <0 01) during late phase of sepsis The affinity for [ 125 I] ET1 binding in light vesicles was unaltered in early and late phases of sepsis It should be mentioned that the sum of Bmax of sarcolemmal and light vesicle fractions was increased by 25% ( P <0 01) during early phase of sepsis but was decreased by 17% ( P <0 01) during late phase of sepsis Conclusions These data indicated that a biphasic intracellular redistribution of ET1 receptor in the heart might contribute to the development of the initial hyperdynamic and subsequent hypodynamic state during sepsis
文摘The estimated annual incidence of out-of-hospital cardiac arrest(OHCA)is approximately 120 cases per 100000 inhabitants in western countries.Although the rates of bystander cardiopulmonary resuscitation(CPR)and use of automated external defibrillator are increasing,the likelihood of survival to hospital discharge is no more than 8%.To date,various devices and methods have been utilized in the initial CPR approach targeting to improve survival and neurological outcomes in OHCA patients.The aim of this review is to discuss strategies that facilitate resuscitation,increase the chance to achieve return to spontaneous circulation and improve survival to hospital discharge and neurological outcomes in the prehospital setting.
文摘This study developed an animal model with internal and external urethral sphincter insufficiency by bypassing the sphincter without major damage so that the animal under study can return to normal life after the study.There is a need for a reliable,applicable,and reproducible animal model for studying urinary incontinency disease due to incorrect sphincter function.Seven adult male dogs were used for this study.The urethral sphincter was bypassed by inserting a catheter between the bladder neck and the distal sphincter.The animals'physical condition was closely monitored for 9 weeks,and standard urodynamic and radiologic studies were performed before and 1–2 months after surgery.The animals were killed at 9 weeks after surgery for pathological assessment.Catheter placement caused complete incontinence in the animal,with urodynamic assessments indicating that the animal was unable to control urination and radiological assessments indicating an empty bladder with a residual volume of 50±10 cc.Tissue analysis did not show significant histological damage and inflammation.The study shows that by bypassing the urethral sphincter,which is a reliable and reproducible method,an animal model of urinary incontinence can be developed,which can be used in various studies such as assessing the adequacy of artificial sphincter function.The animals under study did not have any permanent defect,so they were able to return to their normal life.

文摘In the age of information explosion and artificial intelligence, sentiment analysis tailored for the tobacco industry has emerged as a pivotal avenue for cigarette manufacturers to enhance their tobacco products. Existing solutions have primarily focused on intrinsic features within consumer reviews and achieved significant progress through deep feature extraction models. However, they still face these two key limitations: (1) neglecting the influence of fundamental tobacco information on analyzing the sentiment inclination of consumer reviews, resulting in a lack of consistent sentiment assessment criteria across thousands of tobacco brands;(2) overlooking the syntactic dependencies between Chinese word phrases and the underlying impact of sentiment scores between word phrases on sentiment inclination determination. To tackle these challenges, we propose the External Knowledge-enhanced Cross-Attention Fusion model, CITSA. Specifically, in the Cross Infusion Layer, we fuse consumer comment information and tobacco fundamental information through interactive attention mechanisms. In the Textual Attention Enhancement Layer, we introduce an emotion-oriented syntactic dependency graph and incorporate sentiment-syntactic relationships into consumer comments through a graph convolution network module. Subsequently, the Textual Attention Layer is introduced to combine these two feature representations. Additionally, we compile a Chinese-oriented tobacco sentiment analysis dataset, comprising 55,096 consumer reviews and 2074 tobacco fundamental information entries. Experimental results on our self-constructed datasets consistently demonstrate that our proposed model outperforms state-of-the-art methods in terms of accuracy, precision, recall, and F1-score.
文摘Bcakground: Pancreatic pseudocyst (PPC) is a common complication arising from acute or chronic pancreatitis, trauma, or pancreatic duct obstruction. When acute fluid collection persists for 4 - 6 weeks and is encapsulated by a fibrous wall, it is classified as a pancreatic pseudocyst. While PPC is generally asymptomatic in many patients, it can manifest with persistent abdominal pain, dyspepsia, intra-cystic infection, and potentially lead to gastrointestinal obstruction in some cases. Although smaller PPCs may resolve spontaneously, larger PPCs tend to be refractory to absorption and often necessitate surgical intervention to prevent complications such as intracystic hemorrhage. Objective: To explore the efficacy of percutaneous catheterization with negative pressure in the treatment of large pancreatic pseudocysts. Methods: The cases of large pancreatic pseudocysts treated in our hospital from 2004 to 2022 were retrospectively collected, and the general condition, operation time, drainage time, feeding time, postoperative complications, hospital stay, cost and follow-up of the patients were analyzed. Results: A total of 132 patients with large pancreatic pseudocysts were collected. The average operation time was 32.4 ± 2.1 min;The retention time of the drainage tube was 30 ± 1.8 days in the percutaneous negative pressure drainage group;The postoperative feeding time was no fasting after local anesthesia drainage;Postoperative complications (bleeding, infection, pancreatic leakage, recurrence, anastomotic leakage, etc.): Two of the 132 patients had recurrent cysts, which were cured by re-puncture and negative pressure drainage. The length of hospital stay was 6 ± 1.1 days;The cost was 11,200 ± 1300 yuan;Follow-up: The follow-up time ranged from 1 to 3 years, and the patients had no discomfort. Conclusion: Percutaneous catheterization and negative pressure drainage can effectively treat large pancreatic pseudocysts. Compared with other treatment methods, it is simple and effective, the postoperative recovery of patients is faster, the physical damage is less, the hospital stay is shorter, and the cost is lower.

文摘[Objectives]To investigate the efficacy and mechanisms of action of the Dangshen Xiangfu Chinese herbal patch through the application of network pharmacology.[Methods]The HIT 2.0 database and the TCMSP database were utilized to identify and gather the active ingredients and corresponding action targets of the Dangshen Xiangfu Chinese herbal patch.Cytoscape software was utilized to create network diagrams and analyze the primary active ingredients and action targets of the herbal patch formulation.Gene Ontology(GO)functional analysis and Kyoto Encyclopedia of Genes and Genomes(KEGG)enrichment analysis of the identified action targets were conducted using the DAVID database.The SymMap database was utilized to analyze the network information pertaining to traditional Chinese medicine(TCM),syndromes and the associated symptoms of the medicinal plants found in the Chinese herbal patch.[Results]The network pharmacological analysis suggested that the primary Chinese herbal ingredients of the Dangshen Xiangfu Chinese herbal patch may include Radix Glycyrrhizae Preparata,Fructus Aurantii,Cyperi Rhizoma,Radix Codonopsis,etc.The primary active ingredients may include quercetin,luteolin,kaempferol,fisetin,nobiletin,naringenin,isosinensetin,baicalin,etc.These ingredients exhibited a protective effect on the intestinal tract and were advantageous in the regulation of abnormal intestinal function.The primary targets of action included PTGS2,CASP3,HSP90AA1,PTGS1,AKT1,and MMP9.The active ingredients of the Dangshen Xiangfu Chinese herbal patch may exert a regulatory effect on gastrointestinal inflammation by interacting with the pertinent targets within the PI3K-Akt signaling pathway,the IL-17 signaling pathway,and other related pathways in the body.The principal symptoms associated with this condition may include qi stagnation,qi deficiency,and food accumulation.[Conclusions]This study offers a framework for analyzing the efficacy and mechanisms of action of external preparations derived from medicinal plants.

文摘The topic of improving the mechanical stability of external cavity diode lasers(ECDLs)has recently attracted widespread attention and interest.The use of corner-cube-array(CCA)-based resonators provides a potential solution for this purpose,although continuous oscillation at super large incident angle remains challenging.In this work,we employ the CCA resonator to generate continuous oscillation within±20°angular misalignment of cavity mirror in experiment.On the basis of retroreflection theory,the retroreflectivity of a CCA is analyzed by using optical simulation software.Notably,the experiment verifies the advantage of using a CCA over a plane mirror in laser resonator,thereby providing a promising approach for ECDLs.The threshold characteristic curves measured at different incident angles in the experiment verify that the CCA possesses an obvious anti-angle misalignment performance.This research introduces an alternative solution of using CCA resonator instead of parallel plane cavity,thereby realizing an adjustment-free ECDL with enhanced mechanical stability.

文摘The purpose of this study is to analyze the clinical effect of external application of Granulation-Promoting Jade and Red paste in the treatment of diabetic foot.This study was carried out in Pingdingshan Traditional Chinese Medicine Hospital from December 2023 to December 2024.A total of 80 patients were selected as research objects,all of whom were diagnosed diabetic foot patients.They were treated and divided into two groups based on different methods,the group names were“experimental group”and“control group”.Patients in the control group were treated with Vaseline on the basis of external treatment,and patients in the experimental group were treated with Granulation-Promoting Jade and Red paste on the basis of external treatment,and the clinical effects of different treatment methods were compared and analyzed.The results showed that the effective rate in the experimental group was significantly higher(95.00%)compared to the control group(75.00%)(P<0.05).Furthermore,the experimental group exhibited notably shorter wound healing time and granulation tissue growth time,indicating faster and better recovery(P<0.05).Additionally,post-treatment Wagner grading revealed that the proportion of patients in advanced grades(4,5,and 6)was significantly lower in the experimental group,whereas a higher proportion of patients achieved lower grades(0,1,and 2),demonstrating improved clinical outcomes(P<0.05).In conclusion,Granulation-Promoting Jade and Red Paste shows a significant therapeutic effect in treating diabetic foot,effectively promoting wound healing,improving clinical symptoms,and enhancing overall recovery.

文摘This paper aims to enhance the compression capacity of underwater cylindrical shells by adopting the corrugated sandwich structure of cuttlebone.The cuttlebone suffers uniaxial external compression,while underwater cylindrical shells are in a biaxial compressive stress state.To suit the biaxial compressive stress state,a novel bidirectional corrugated sandwich structure is proposed to improve the bearing capacity of cylindrical shells.The static and buckling analysis for the sandwich shell and the unstiffened cylindrical shell with the same volume-weight ratio are studied by numerical simulation.It is indicated that the proposed sandwich shell can effectively reduce the ratio between circumferential and axial stress from 2 to 1.25 and improve the critical buckling load by about 1.63 times.Numerical simulation shows that optimizing and adjusting the structural parameters could significantly improve the advantage of the sandwich shell.Then,the hydrostatic pressure tests for shell models fabricated by 3D printing are carried out.According to the experimental results,the overall failure position of the sandwich shell is at the center part of the sandwich shell.It has been found the average critical load of the proposed sandwich shell models exceeds two times that of the unstiffened shell models.Hence,the proposed bio-inspired bidirectional corrugated sandwich structure can significantly enhance the pressure resistance capability of cylindrical shells.

文摘In order to obtain the transient characteristics of a low-speed centrifugal pump during the start-up and shutdown stages,dedicated experimental tests were conducted with eight different valve opening conditions.The Pearson correlation coefficient was used to reveal the linear correlation between variables.According to the results,the stable rotational speed decreases with increasing valve opening(rotational speed decreases from approximately 1472 to 1453 r/min),while the stable shaft power exhibits an increasing trend(shaft power increases from approximately 0.242 to 0.390 kW).The stable time and zeroing time of each parameter during start-up and shutdown processes vary,with the flow zeroing time significantly increasing with the relative flow,reaching up to 10.468 s,while the shaft power zeroing time is roughly between 1.219 and 1.375 s.The results demonstrate that with increasing valve opening,the stable and zeroing time of flow significantly increase(|r|greater than 0.95),while the stable and zeroing time of rotational speed,power,and head display a smaller sensitivity on the valve opening(|r|less than 0.6).

文摘The effect of slow shot speed on externally solidified crystal(ESC),porosity and tensile property in a newly developed high-pressure die-cast Al-Si alloy was investigated by optical microscopy(OM),scanning electron microscopy(SEM)and laboratory computed tomography(CT).Results showed that the newly developed AlSi9MnMoV alloy exhibited improved mechanical properties when compared to the AlSi10MnMg alloy.The AlSi9MnMoV alloy,which was designed with trace multicomponent additions,displays a notable grain refining effect in comparison to the AlSi10MnMg alloy.Refining elements Ti,Zr,V,Nb,B promote heterogeneous nucleation and reduce the grain size of primaryα-Al.At a lower slow shot speed,the large ESCs are easier to form and gather,developing into the dendrite net and net-shrinkage.With an increase in slow shot speed,the size and number of ESCs and porosities significantly reduce.In addition,the distribution of ESCs is more dispersed and the net-shrinkage disappears.The tensile property is greatly improved by adopting a higher slow shot speed.The ultimate tensile strength is enhanced from 260.31 MPa to 290.31 MPa(increased by 11.52%),and the elongation is enhanced from 3.72%to 6.34%(increased by 70.52%).

文摘BACKGROUND Primary liver cancer is a malignant tumor with a high recurrence rate that significantly affects patient prognosis.Postoperative adjuvant external radiation therapy(RT)has been shown to effectively prevent recurrence after liver cancer resection.However,there are multiple RT techniques available,and the differ-ential effects of these techniques in preventing postoperative liver cancer re-currence require further investigation.AIM To assess the advantages and disadvantages of various adjuvant external RT methods after liver resection based on overall survival(OS)and disease-free survival(DFS)and to determine the optimal strategy.METHODS This study involved network meta-analyses and followed the PRISMA guidelines.The data of qualified studies published before July 10,2023,were collected from PubMed,Embase,the Web of Science,and the Cochrane Library.We included relevant studies on postoperative external beam RT after liver resection that had OS and DFS as the primary endpoints.The magnitudes of the effects were determined using risk ratios with 95%confidential intervals.The results were analyzed using R software and STATA software.RESULTS A total of 12 studies,including 1265 patients with hepatocellular carcinoma(HCC)after liver resection,were included in this study.There was no significant heterogeneity in the direct paired comparisons,and there were no significant differences in the inclusion or exclusion criteria,intervention measures,or outcome indicators,meeting the assumptions of heterogeneity and transitivity.OS analysis revealed that patients who underwent stereotactic body radiotherapy(SBRT)after resection had longer OS than those who underwent intensity modulated radiotherapy(IMRT)or 3-dimensional conformal RT(3D-CRT).DFS analysis revealed that patients who underwent 3D-CRT after resection had the longest DFS.Patients who underwent IMRT after resection had longer OS than those who underwent 3D-CRT and longer DFS than those who underwent SBRT.CONCLUSION HCC patients who undergo liver cancer resection must consider distinct advantages and disadvantages when choosing between SBRT and 3D-CRT.IMRT,a RT technique that is associated with longer OS than 3D-CRT and longer DFS than SBRT,may be a preferred option.
