Rationale:Vanishing lung syndrome is rare and can be associated with a history of smoking and marijuana use.The occurrence of giant bullae can also be linked to infections,particularly tuberculosis in tropical countri...Rationale:Vanishing lung syndrome is rare and can be associated with a history of smoking and marijuana use.The occurrence of giant bullae can also be linked to infections,particularly tuberculosis in tropical countries.Patient concerns:A 26-year-old male complained of weakness,severe vomiting,and reduced breathlessness when lying on the left side.He had a history of pulmonary tuberculosis two years ago.Diagnosis:Symptomatic bradycardia in tuberculosis-related giant bullae.Interventions:The patient was recommended to undergo an elective bullectomy,but he decided not to proceed with the procedure.Atropine sulfate was administered to alleviate symptoms of bradycardia,while a standardized anti-tuberculosis regimen were started for the next six months.Outcomes:Following 7 days of intensive care treatment involving antituberculosis medications and atropine sulfate,the patient achieved hemodynamic stability,opting against bullectomy despite residual symptoms of dyspnea.Subsequent six months of antituberculosis therapy notably alleviated symptoms without requiring bullectomy.Lessons:Increasing intrathoracic pressure can also be caused mechanically by giant bullae.Cardiac symptoms in vanishing lung syndrome are reversible and can be alleviated once the underlying cause is addressed.In this case,symptomatic bradycardia was reduced only with tuberculosis treatment without bullectomy intervention。展开更多

The majority of adult patients with VBDS suffer from primary biliary cirrhosis (PBC) and primary sclerosing cholangitis (PSC). Ursodeoxycholic acid (UDCA), a hydrophilic dihydroxy bile acid, is the only drug currently approved for the treatment of patients with PBC, and anticholestatic effects have been reported for several other cholestatic syndromes. Despite the extensive investigation of the patho genetic mechanisms of the disease, its etiology hasn'been clarified and several theories exist. The syndrome can affect one or multiple bones of the patient, includ ing the skull, the upper and lower extremities, the spine and pelvis. The clinical presentation of a patient suffer ing from vanishing bone disease includes, pain, func tional impairment and swelling of the affected region although asymptomatic cases have been reported, as well as cases in which the diagnosis was made after a pathologic fracture. In this short review we summarize the theories regarding the etiology as well as the clini cal presentation, the diagnostic approach and treat ment options of this rare disease.展开更多

Given any entropy solution of non-isentropic gas dynamics which consists of two different families of shocks interacting at some positive time, we show that such solution is the vanishing viscosity limit of a family of smooth global solutions for a viscous system of conservation law. It is an acquired condition characterized by progressive destruction and loss of the intra-hepatic bile ducts leading to cholestasis. Prognosis is variable and partially dependent upon the etiology of bile duct injury. Irreversible bile duct loss leads to significant ductopenia, biliary cirrhosis, liver failure, and death. If biliary epithelial regeneration occurs, clinical recovery may occur over a period of months to years. VBDS has been described in a number of cases of patients with Hodgkin’s lymphoma (HL) where it is thought to be a paraneoplastic phenomenon. This case describes a 25-year-old man found on liver biopsy to have VBDS. Given poor response to medical treatment, the patient underwent transplant evaluation at that time and was found to have classical stage IIB HL. Early recognition of this underlying cause or association of VBDS, including laboratory screening, and physical exam for lymphadenopathy are paramount to identifying potential underlying VBDS-associated malignancy. Here we review the literature of HL-associated VBDS and report a case of diagnosed HL with biopsy proven VBDS.展开更多

The presentation of this infection can sometimes...Gastrointestinal involvement in cytomegalovirus (CMV) infection is well documented among immunocompromised patients and is also observed in immunocompetent individuals. The presentation of this infection can sometimes mimic those of other diseases, thus making accurate diagnosis difficult. We herein report a rare case of an immunocompetent elderly adult with gastrointestinal CMV infection that presented as a vanishing tumor at the cecum. A 76-year old man initially presented with lower abdominal pain. Colonoscopy revealed a tumorous lesion with irregular ulceration observed at the ileocecal valve. Histological findings of a biopsy specimen revealed intranuclear inclusions which were positive for CMV on immunohistochemical staining. However, this tumorous lesion disappeared within 7 wk from the initialhospital visit without any further treatment. Inflammatory pseudotumors associated with CMV infection should be considered as a differential diagnosis of tumorous lesions in the colon, even in immunocompetent adults.展开更多

A 14-yearold child admitted to the department of Neurology at September 2009 with a history of subacute onset of fever, anorexia, vomiting, blurring of vision and right hemiparesis since one month. Magnetic resonance imaging(MRI) of the brain revealed presence of intraaxial large mass(25 mm × 19 mm) in the left temporal lobe and the brainstem which showed hypointense signal in T1 W and hyperintense signals in T2 W and fluid attenuated inversion recovery(FLAIR) images and homogenously enhanced with gadolinium(Gd). It was surrounded by vasogenic edema with mass effect. Intravenous antibiotics, mannitol(2 g/12 h per 2 d) and dexamethasone(8 mg/12 h) were given to relief manifestations of increased intracranial pressure. Whole craniospinal radiotherapy(brain = 4000 CGy/20 settings per 4 wk; Spinal = 2600/13 settings per 3 wk) was given based on the high suspicion of neoplastic lesion(lymphoma or glioma). Marked clinical improvement(up to complete recovery) occurred within 15 d. Tapering of the steroid dose was done over the next 4 mo. Follow up with MRI after 3 mo showed small lesion in the left antero-medial temporal region with hypointense signal in T1 W and hyperintense signals in T2 W and FLAIR images but did not enhance with Gd. At August 2012, the patient developed recurrent generalized epilepsy. His electroencephalography showed the presence of left temporal focus of epileptic activity. MRI showed the same lesion as described in the follow up. The diffusion weighted images were normal. The seizures frequency was decreased with carbamazepine therapy(300 mg/12 h). At October 2014, single voxel proton(1H) MR spectroscopy(MRS) showedreduced N-acetyl-aspartate(NAA)/creatine(Cr), choline(Cho)/Cr, NAA/Cho ratios consistent with absence of a neoplasm and highly suggested presence of gliosis. A solitary brain mass in a child poses a considerable diagnostic difficulty. MRS provided valuable diagnostic differentiation between tumor and pseudotumor lesions.展开更多

To check previous findings of the most common complications among pregnancies with vanishing embryo(VE) in another actual study retropective and in group of patiens with selective embryo reduction (SER) Methods We...To check previous findings of the most common complications among pregnancies with vanishing embryo(VE) in another actual study retropective and in group of patiens with selective embryo reduction (SER) Methods We defined vanishing phenomenon as the spontaneous loss of one or more embryos after visualizing heart activity at the first trimester of pregnancy. Selective embryo reduction was performed between 8th-12th pregnancy week, through vaginal punction and aspiration of embryonic mass. Results Vanishing embryo was observed in 86 patients (18.0%). In 61 patients (70.9%) this phenomenom happened before 9th pregnancy week. The incidence of VE increased with higher number of gestational sacs initially visualized (P〈0.03). First trimester bleeding was more common among pregnancies with VE than in the control (P〈0.005). The incidence of pregnancy induced hypertension was lower in pregnancies with VE than in the controls (P〈0.03). In contrast, preterm spontaneous rupture of membranes was higher, although without statistical significance. Gestational age at delivery, mode of delivery and birth weight was similar in the group of VE and the controls (P=NS). Conclusion All these informations may be useful in counselling patients on the prognosis and outcome of pregnancies achieved by oocyte donation.展开更多

A lot of real problems are effected by a delayed history information. In this paper we investigate some properties of cordial Volterra integral operators influenced by a vanishing delay. It is shown that to replicate all eigenfunctions , or , the vanishing delay must be a proportional delay. For such a linear delay, the spectrum, eigenvalues and eigenfunctions of the operators and the existence, uniqueness and solution spaces of solutions are presented. For a nonlinear vanishing delay, we show a necessary and sufficient condition such that the operator is compact, which also yields the existence and uniqueness of solutions to CVIEs with the vanishing delay.展开更多

文摘In this article, we focus to study about modified objective function approach for multiobjective optimization problem with vanishing constraints. An equivalent η-approximated multiobjective optimization problem is constructed by a modification of the objective function in the original considered optimization problem. Furthermore, we discuss saddle point criteria for the aforesaid problem. Moreover, we present some examples to verify the established results.
文摘Gastrointestinal involvement in cytomegalovirus (CMV) infection is well documented among immunocompromised patients and is also observed in immunocompetent individuals. The presentation of this infection can sometimes mimic those of other diseases, thus making accurate diagnosis difficult. We herein report a rare case of an immunocompetent elderly adult with gastrointestinal CMV infection that presented as a vanishing tumor at the cecum. A 76-year old man initially presented with lower abdominal pain. Colonoscopy revealed a tumorous lesion with irregular ulceration observed at the ileocecal valve. Histological findings of a biopsy specimen revealed intranuclear inclusions which were positive for CMV on immunohistochemical staining. However, this tumorous lesion disappeared within 7 wk from the initialhospital visit without any further treatment. Inflammatory pseudotumors associated with CMV infection should be considered as a differential diagnosis of tumorous lesions in the colon, even in immunocompetent adults.
文摘We describe clinical, diagnostic features and follow up of a patient with a vanishing brain lesion. A 14-yearold child admitted to the department of Neurology at September 2009 with a history of subacute onset of fever, anorexia, vomiting, blurring of vision and right hemiparesis since one month. Magnetic resonance imaging(MRI) of the brain revealed presence of intraaxial large mass(25 mm × 19 mm) in the left temporal lobe and the brainstem which showed hypointense signal in T1 W and hyperintense signals in T2 W and fluid attenuated inversion recovery(FLAIR) images and homogenously enhanced with gadolinium(Gd). It was surrounded by vasogenic edema with mass effect. Intravenous antibiotics, mannitol(2 g/12 h per 2 d) and dexamethasone(8 mg/12 h) were given to relief manifestations of increased intracranial pressure. Whole craniospinal radiotherapy(brain = 4000 CGy/20 settings per 4 wk; Spinal = 2600/13 settings per 3 wk) was given based on the high suspicion of neoplastic lesion(lymphoma or glioma). Marked clinical improvement(up to complete recovery) occurred within 15 d. Tapering of the steroid dose was done over the next 4 mo. Follow up with MRI after 3 mo showed small lesion in the left antero-medial temporal region with hypointense signal in T1 W and hyperintense signals in T2 W and FLAIR images but did not enhance with Gd. At August 2012, the patient developed recurrent generalized epilepsy. His electroencephalography showed the presence of left temporal focus of epileptic activity. MRI showed the same lesion as described in the follow up. The diffusion weighted images were normal. The seizures frequency was decreased with carbamazepine therapy(300 mg/12 h). At October 2014, single voxel proton(1H) MR spectroscopy(MRS) showedreduced N-acetyl-aspartate(NAA)/creatine(Cr), choline(Cho)/Cr, NAA/Cho ratios consistent with absence of a neoplasm and highly suggested presence of gliosis. A solitary brain mass in a child poses a considerable diagnostic difficulty. MRS provided valuable diagnostic differentiation between tumor and pseudotumor lesions.

A computation example for N = 4 is also given.

文摘To check previous findings of the most common complications among pregnancies with vanishing embryo(VE) in another actual study retropective and in group of patiens with selective embryo reduction (SER) Methods We defined vanishing phenomenon as the spontaneous loss of one or more embryos after visualizing heart activity at the first trimester of pregnancy. Selective embryo reduction was performed between 8th-12th pregnancy week, through vaginal punction and aspiration of embryonic mass. Results Vanishing embryo was observed in 86 patients (18.0%). In 61 patients (70.9%) this phenomenom happened before 9th pregnancy week. The incidence of VE increased with higher number of gestational sacs initially visualized (P〈0.03). First trimester bleeding was more common among pregnancies with VE than in the control (P〈0.005). The incidence of pregnancy induced hypertension was lower in pregnancies with VE than in the controls (P〈0.03). In contrast, preterm spontaneous rupture of membranes was higher, although without statistical significance. Gestational age at delivery, mode of delivery and birth weight was similar in the group of VE and the controls (P=NS). Conclusion All these informations may be useful in counselling patients on the prognosis and outcome of pregnancies achieved by oocyte donation.

文摘The rapid development of image processing techniques has made it extremely easy to alter the content of images or create newimages.So photographs,which appear in magazines,social media,and political attacks,can no longer be trusted.A novel and effective technique is proposed in this paper to expose image forgery using inconsistent reflection vanishing point(RVP).More specifically,the definition of error distance is given,sin^2()-based function is proposed to normalize error distance,and a reasonable threshold value is set to detect image forgery.The experimental data and results are presented to demonstrate the accuracy and effectiveness of the technique.
文摘Cordial Volterra integral equations (CVIEs) from some applications models associated with a noncompact cordial Volterra integral operator are discussed in the recent years. A lot of real problems are effected by a delayed history information. In this paper we investigate some properties of cordial Volterra integral operators influenced by a vanishing delay. It is shown that to replicate all eigenfunctions , or , the vanishing delay must be a proportional delay. For such a linear delay, the spectrum, eigenvalues and eigenfunctions of the operators and the existence, uniqueness and solution spaces of solutions are presented. For a nonlinear vanishing delay, we show a necessary and sufficient condition such that the operator is compact, which also yields the existence and uniqueness of solutions to CVIEs with the vanishing delay.
