We used three reverse adipofascial cross finger flaps raised at the same time from 2 fingers to reconstruct adjacent fingers of the patient. The patient recovered well postoperatively and had good range of movement of the fingers. This avoided the complications of the use of regional or distal flaps. To our knowledge, this is the first case reported in which three reverse adipofascial cross fingers flaps are raised at the same time, two of them from an injured finger, to cover three raw areas on two fingers of a patient.展开更多

Methods Nine cases were replanted using the routine method and the artery-vein bridge grafting method respectively. Results All 9 cases survived. At 1 year postoperation, the length of the replanted finger was comparable to that of the healthy side with satisfactory appearance. The average finger function increased 30% when compared with pre-operation one. Conclusion For the amputated complex tissue mass of fingers, routine replantation should be carried out if there was one or two digital proper arteries. It is significant to develop a robot hand with high rigidity by a 6-DOF parallel manipulator(PM).However,the existing6-DOF PMs include spherical joint which has less capability of pulling force bearing,less rotation...It is significant to develop a robot hand with high rigidity by a 6-DOF parallel manipulator(PM).However,the existing6-DOF PMs include spherical joint which has less capability of pulling force bearing,less rotation range and lower precision under alternately heavy loads.A novel 6-DOF PM with three planar limbs and equipped with three fingers is proposed and its kinematics and statics are analyzed systematically.A 3-dimension simulation mechanism of the proposed manipulator is constructed and its structure characteristics is analyzed.The kinematics formulae for solving the displacement,velocity,acceleration of the platform,the active legs and the fingers are established.The statics formulae are derived for solving the active forces of PM and the finger mechanisms.An analytic example is given for solving the kinematics and statics of proposed manipulator and the analytic solved results are verified by the simulation mechanism.It is proved from the error analysis of analytic solutions and simulation solutions that the derived analytic formulae are correct and provide the theoretical and technical foundations for its manufacturing,control and application.展开更多
BACKGROUND Sodium glucose cotransporter 2(SGLT2)inhibitors use has been associated with toe amputations and non-healing ulcers and gangrene mostly of lower extremities.There are no case reports about association of Em...BACKGROUND Sodium glucose cotransporter 2(SGLT2)inhibitors use has been associated with toe amputations and non-healing ulcers and gangrene mostly of lower extremities.There are no case reports about association of Empagliflozin with finger ulcers or gangrene.This is the first case report of Empagliflozin(Jardiance)an SGLT2 inhibitor causing gangrene of fingers and second case in literature about any SGLT2 inhibitor causing gangrene of upper extremity.CASE SUMMARY A 76-year-old man with type 2 diabetes mellitus sustained minimal trauma to both middle fingers,which started healing.He was started on empagliflozin a week later for management of type 2 diabetes mellitus and started developing gangrene to both middle finger tips along with neuropathic pain which worsened over the course of next four months.Investigations were negative for vascular insufficiency,infection and vasculitis and imaging of hand was normal.Discontinuation of empagliflozin slowed progression of gangrene and caused symptomatic improvement with reduction in neuropathic pain.CONCLUSION This case report suggests possible association of empagliflozin and finger gangrene and recommends that more research and awareness among clinicians is needed in this area.展开更多

At first, ergonomic consideration in design is reachable; this means the operator' s fingertip must be able to reach the operating component. This is generally no question because human ann has much more degrees of freedom required to position his arms, hands and fingers in the three-dimension space. However, some times we need the finger operate with a fixed wrist. For exm'nple in the case in the typing, the reachable workspace of the finger must take into account in such situation. Finger contacting is the most familiar operation mode of the man-machine system, and the index fmger takes on the primary operation tasks. From viewpoint of ergonomic engineering, the operation component should be placed within the workspace of the fingertip to reduced or eliminate the movement of palm and ann should to the greatest extent during finger manipulation. Therefore the research of the workspace of ginger is significant to the ergonomic design of the operation device. In this paper, the reachable workspace and workspace under direction restrain of contacting for the index finger are determined using serial mechanism model and the Penalty Function Method based on geometric measurement of human body. Background:Hand injuries are very common in sports,such as skiing and ball sports.One of the major reasons causing hand and finger deformity is due to ligament and tendon injury.The aim of this study was to investiga...Background:Hand injuries are very common in sports,such as skiing and ball sports.One of the major reasons causing hand and finger deformity is due to ligament and tendon injury.The aim of this study was to investigate if the high-resolution 3T magnetic resonance imaging (MRI) can demonstrate the complex anatomy of the fingers and thumb,especially the tendons and ligaments,and provide the accurate diagnosis of clinically important fingers and thumbs deformity due to ligamentous and tendinous injuries during sport activities.Methods:Sixteen fresh un-embalmed cadaveric hands were harvested from eight cadavers.A total of 20 healthy volunteers' hands and 44 patients with fingers or thumb deformity due to sports-related injuries were included in this study.All subjects had MR examination with T1-weighted images and proton density-weighted imaging with fat suppression (PD FS) in axial,coronal,and sagittal plane,respectively.Subsequently,all 16 cadaveric hands were sliced into 2-mm thick slab with a band saw (six in coronal plane,six in sagittal plane,and four in axial plane).The correlation of anatomic sections and the MRI characteristics of tendons of fingers and the ulnar collateral ligament (UCL) at the metacarpal phalangeal joint (MCPJ) of thumb between 20 healthy volunteers and 44 patients (confirmed by surgery) were analyzed.Results:The normal ligaments and tendons in 16 cadaveric hands and 20 volunteers' hands showed uniform low-signal intensity on all the sequences of the MRI.Among 44 patients with tendinous and ligamentous injuries in the fingers or thumb,12 cases with UCL injury at MCPJ of the thumb (Stener lesion =8 and non-Stener lesion=4),6 cases with the central slip injury,12 cases with terminal tendon injury,and 14 cases with flexor digitorum profundus injury.The ligaments and tendons disruption manifested as increased signal intensity and poor definition,discontinuity,and heterogeneous signal intensity of the involved ligaments and tendons.Conclusions:Sports injury-related fingers and thumb deformity are relatively common.MRI is an accurate method for evaluation of the anatomy and pathologic conditions of the fingers and thumb.It is a useful tool for accurate diagnosis of the sports-related ligaments and tendons injuries in hand.展开更多

The interest of this s...Purpose: Distal finger amputations pose a therapeutic problem with the distal fragment quality. Reimplantation remains the reference treatment for functional and aesthetic recovery of the hand. The interest of this study is to propose the reposition flap as an alternative to different hedging techniques in the proximal stump, in many situations where revascularization is impossible. It consists in osteosynthesis of the bone fragment and its coverage by a pedicled local flap. Methods: The technique of reposition flap was evaluated retrospectively between 2003 and 2016 through a study of 13 patients compiled in Nabeul orthopedic department. For each patient, the sensitivity, the pulp trophicity, the interphalangeal mobility, the digital length, the appearance of the nail and radiological consolidation were evaluated. Results: The reposition flap keeps more than 80% of the length of p3. This procedure improves nail aesthetics in comparison with the regularizations. There is no significant difference in sensitivity of the pulp or of the mobility of the distal inter-phalangeal (DIP) joint as a function of the technique studied. However there is a significant difference in average test of the Quick Dash (350 against 500 for regularizations). Conclusion: The reposition flap seems to be a good alternative to regularization in the context of trans-p3 fingers amputations, in which the distal fragment is not revascularizable. It allows better aesthetic and functional results.展开更多

Background With the increasing prominence of hand and finger motion tracking in virtual reality(VR)applications and rehabilitation studies,data gloves have emerged as a prevalent solution.In this study,we developed an...Background With the increasing prominence of hand and finger motion tracking in virtual reality(VR)applications and rehabilitation studies,data gloves have emerged as a prevalent solution.In this study,we developed an innovative,lightweight,and detachable data glove tailored for finger motion tracking in VR environments.Methods The glove design incorporates a potentiometer coupled with a flexible rack and pinion gear system,facilitating precise and natural hand gestures for interaction with VR applications.Initially,we calibrated the potentiometer to align with the actual finger bending angle,and verified the accuracy of angle measurements recorded by the data glove.To verify the precision and reliability of our data glove,we conducted repeatability testing for flexion(grip test)and extension(flat test),with 250 measurements each,across five users.We employed the Gage Repeatability and Reproducibility to analyze and interpret the repeatable data.Furthermore,we integrated the gloves into a SteamVR home environment using the OpenGlove auto-calibration tool.Conclusions The repeatability analysis revealed an aggregate error of 1.45 degrees in both the gripped and flat hand positions.This outcome was notably favorable when compared with the findings from assessments of nine alternative data gloves that employed similar protocols.In these experiments,users navigated and engaged with virtual objects,underlining the glove's exact tracking of finger motion.Furthermore,the proposed data glove exhibited a low response time of 17-34 ms and back-drive force of only 0.19 N.Additionally,according to a comfort evaluation using the Comfort Rating Scales,the proposed glove system is wearable,placing it at the WL1 level.展开更多
BACKGROUND Hepatocellular carcinoma(HCC)is one of the most prevalent and aggressive forms of liver cancer,with high morbidity and poor prognosis due to late diagnosis and limited treatment options.Despite advances in ...BACKGROUND Hepatocellular carcinoma(HCC)is one of the most prevalent and aggressive forms of liver cancer,with high morbidity and poor prognosis due to late diagnosis and limited treatment options.Despite advances in understanding its molecular mechanisms,effective biomarkers for early detection and targeted therapy remain scarce.Zinc finger protein 71(ZNF71),a zinc-finger protein,has been implicated in various cancers,yet its role in HCC remains largely unexplored.This gap in knowledge underscores the need for further investigation into the ZNF71 of potential as a diagnostic or therapeutic target in HCC.AIM To explore the expression levels,clinical relevance,and molecular mechanisms of ZNF71 in the progression of HCC.METHODS The study evaluated ZNF71 expression in 235 HCC specimens and 13 noncancerous liver tissue samples using immunohistochemistry.High-throughput datasets were employed to assess the differential expression of ZNF71 in HCC and its association with clinical and pathological features.The impact of ZNF71 on HCC cell line growth was examined through clustered regularly interspaced short palindromic repeat knockout screens.Co-expressed genes were identified and analyzed for enrichment using LinkedOmics and Sangerbox 3.0,focusing on significant correlations(P<0.01,correlation coefficient≥0.3).Furthermore,the relationship between ZNF71 expression and immune cell infiltration was quantified using TIMER2.0.RESULTS ZNF71 showed higher expression in HCC tissues vs non-tumorous tissues,with a significant statistical difference(P<0.05).Data from the UALCAN platform indicated increased ZNF71 levels across early to mid-stage HCC,correlating with disease severity(P<0.05).High-throughput analysis presented a standardized mean difference in ZNF71 expression of 0.55(95%confidence interval[CI]:0.34-0.75).The efficiency of ZNF71 mRNA was evaluated,yielding an area under the curve of 0.78(95%CI:0.75-0.82),a sensitivity of 0.63(95%CI:0.53-0.72),and a specificity of 0.82(95%CI:0.73-0.89).Diagnostic likelihood ratios were positive at 3.61(95%CI:2.41-5.41)and negative at 0.45(95%CI:0.36-0.56).LinkedOmics analysis identified strong positive correlations of ZNF71 with genes such as ZNF470,ZNF256,and ZNF285.Pathway enrichment analyses highlighted associations with herpes simplex virus type 1 infection,the cell cycle,and DNA replication.Negative correlations involved metabolic pathways,peroxisomes,and fatty acid degradation.TIMER2.0 analysis demonstrated positive correlations of high ZNF71 expression with various immune cell types,including CD4^(+)T cells,B cells,regulatory T cells,monocytes,macrophages,and myeloid dendritic cells.CONCLUSION ZNF71 is significantly upregulated in HCC,correlating with the disease’s clinical and pathological stages.It appears to promote HCC progression through mechanisms involving the cell cycle and metabolism and is associated with immune cell infiltration.These findings suggest that ZNF71 could be a novel target for diagnosing and treating HCC.展开更多

文摘Background:Hand injuries are very common in sports,such as skiing and ball sports.One of the major reasons causing hand and finger deformity is due to ligament and tendon injury.The aim of this study was to investigate if the high-resolution 3T magnetic resonance imaging (MRI) can demonstrate the complex anatomy of the fingers and thumb,especially the tendons and ligaments,and provide the accurate diagnosis of clinically important fingers and thumbs deformity due to ligamentous and tendinous injuries during sport activities.Methods:Sixteen fresh un-embalmed cadaveric hands were harvested from eight cadavers.A total of 20 healthy volunteers' hands and 44 patients with fingers or thumb deformity due to sports-related injuries were included in this study.All subjects had MR examination with T1-weighted images and proton density-weighted imaging with fat suppression (PD FS) in axial,coronal,and sagittal plane,respectively.Subsequently,all 16 cadaveric hands were sliced into 2-mm thick slab with a band saw (six in coronal plane,six in sagittal plane,and four in axial plane).The correlation of anatomic sections and the MRI characteristics of tendons of fingers and the ulnar collateral ligament (UCL) at the metacarpal phalangeal joint (MCPJ) of thumb between 20 healthy volunteers and 44 patients (confirmed by surgery) were analyzed.Results:The normal ligaments and tendons in 16 cadaveric hands and 20 volunteers' hands showed uniform low-signal intensity on all the sequences of the MRI.Among 44 patients with tendinous and ligamentous injuries in the fingers or thumb,12 cases with UCL injury at MCPJ of the thumb (Stener lesion =8 and non-Stener lesion=4),6 cases with the central slip injury,12 cases with terminal tendon injury,and 14 cases with flexor digitorum profundus injury.The ligaments and tendons disruption manifested as increased signal intensity and poor definition,discontinuity,and heterogeneous signal intensity of the involved ligaments and tendons.Conclusions:Sports injury-related fingers and thumb deformity are relatively common.MRI is an accurate method for evaluation of the anatomy and pathologic conditions of the fingers and thumb.It is a useful tool for accurate diagnosis of the sports-related ligaments and tendons injuries in hand.
文摘Purpose: Distal finger amputations pose a therapeutic problem with the distal fragment quality. Reimplantation remains the reference treatment for functional and aesthetic recovery of the hand. The interest of this study is to propose the reposition flap as an alternative to different hedging techniques in the proximal stump, in many situations where revascularization is impossible. It consists in osteosynthesis of the bone fragment and its coverage by a pedicled local flap. Methods: The technique of reposition flap was evaluated retrospectively between 2003 and 2016 through a study of 13 patients compiled in Nabeul orthopedic department. For each patient, the sensitivity, the pulp trophicity, the interphalangeal mobility, the digital length, the appearance of the nail and radiological consolidation were evaluated. Results: The reposition flap keeps more than 80% of the length of p3. This procedure improves nail aesthetics in comparison with the regularizations. There is no significant difference in sensitivity of the pulp or of the mobility of the distal inter-phalangeal (DIP) joint as a function of the technique studied. However there is a significant difference in average test of the Quick Dash (350 against 500 for regularizations). Conclusion: The reposition flap seems to be a good alternative to regularization in the context of trans-p3 fingers amputations, in which the distal fragment is not revascularizable. It allows better aesthetic and functional results.

文摘PrPSc,a misfolded,aggregation-prone isoform of the cellular prion protein(PrPC),is the infectious prion agent responsible for fatal neurodegenerative diseases of humans and other mammals.PrPSccan adopt different pathogenic conformations(prion strains),which can be resistant to potential drugs,or acquire drug resistance,posing challenges for the development of effective therapies.Since PrPCis the obligate precursor of any prion strain and serves as the mediator of prion neurotoxicity,it represents an attractive therapeutic target fo r prion diseases.In this minireview,we briefly outline the approaches to target PrPCand discuss our recent identification of Zn(Ⅱ)-Bn PyP,a PrPC-targeting porphyrin with an unprecedented bimodal mechanism of action.We argue that in-depth understanding of the molecular mechanism by which Zn(Ⅱ)-Bn PyP targets PrPCmay lead toward the development of a new class of dual mechanism anti-prion compounds.

文摘Background With the increasing prominence of hand and finger motion tracking in virtual reality(VR)applications and rehabilitation studies,data gloves have emerged as a prevalent solution.In this study,we developed an innovative,lightweight,and detachable data glove tailored for finger motion tracking in VR environments.Methods The glove design incorporates a potentiometer coupled with a flexible rack and pinion gear system,facilitating precise and natural hand gestures for interaction with VR applications.Initially,we calibrated the potentiometer to align with the actual finger bending angle,and verified the accuracy of angle measurements recorded by the data glove.To verify the precision and reliability of our data glove,we conducted repeatability testing for flexion(grip test)and extension(flat test),with 250 measurements each,across five users.We employed the Gage Repeatability and Reproducibility to analyze and interpret the repeatable data.Furthermore,we integrated the gloves into a SteamVR home environment using the OpenGlove auto-calibration tool.Conclusions The repeatability analysis revealed an aggregate error of 1.45 degrees in both the gripped and flat hand positions.This outcome was notably favorable when compared with the findings from assessments of nine alternative data gloves that employed similar protocols.In these experiments,users navigated and engaged with virtual objects,underlining the glove's exact tracking of finger motion.Furthermore,the proposed data glove exhibited a low response time of 17-34 ms and back-drive force of only 0.19 N.Additionally,according to a comfort evaluation using the Comfort Rating Scales,the proposed glove system is wearable,placing it at the WL1 level.

文摘BACKGROUND Hepatocellular carcinoma(HCC)is one of the most prevalent and aggressive forms of liver cancer,with high morbidity and poor prognosis due to late diagnosis and limited treatment options.Despite advances in understanding its molecular mechanisms,effective biomarkers for early detection and targeted therapy remain scarce.Zinc finger protein 71(ZNF71),a zinc-finger protein,has been implicated in various cancers,yet its role in HCC remains largely unexplored.This gap in knowledge underscores the need for further investigation into the ZNF71 of potential as a diagnostic or therapeutic target in HCC.AIM To explore the expression levels,clinical relevance,and molecular mechanisms of ZNF71 in the progression of HCC.METHODS The study evaluated ZNF71 expression in 235 HCC specimens and 13 noncancerous liver tissue samples using immunohistochemistry.High-throughput datasets were employed to assess the differential expression of ZNF71 in HCC and its association with clinical and pathological features.The impact of ZNF71 on HCC cell line growth was examined through clustered regularly interspaced short palindromic repeat knockout screens.Co-expressed genes were identified and analyzed for enrichment using LinkedOmics and Sangerbox 3.0,focusing on significant correlations(P<0.01,correlation coefficient≥0.3).Furthermore,the relationship between ZNF71 expression and immune cell infiltration was quantified using TIMER2.0.RESULTS ZNF71 showed higher expression in HCC tissues vs non-tumorous tissues,with a significant statistical difference(P<0.05).Data from the UALCAN platform indicated increased ZNF71 levels across early to mid-stage HCC,correlating with disease severity(P<0.05).High-throughput analysis presented a standardized mean difference in ZNF71 expression of 0.55(95%confidence interval[CI]:0.34-0.75).The efficiency of ZNF71 mRNA was evaluated,yielding an area under the curve of 0.78(95%CI:0.75-0.82),a sensitivity of 0.63(95%CI:0.53-0.72),and a specificity of 0.82(95%CI:0.73-0.89).Diagnostic likelihood ratios were positive at 3.61(95%CI:2.41-5.41)and negative at 0.45(95%CI:0.36-0.56).LinkedOmics analysis identified strong positive correlations of ZNF71 with genes such as ZNF470,ZNF256,and ZNF285.Pathway enrichment analyses highlighted associations with herpes simplex virus type 1 infection,the cell cycle,and DNA replication.Negative correlations involved metabolic pathways,peroxisomes,and fatty acid degradation.TIMER2.0 analysis demonstrated positive correlations of high ZNF71 expression with various immune cell types,including CD4^(+)T cells,B cells,regulatory T cells,monocytes,macrophages,and myeloid dendritic cells.CONCLUSION ZNF71 is significantly upregulated in HCC,correlating with the disease’s clinical and pathological stages.It appears to promote HCC progression through mechanisms involving the cell cycle and metabolism and is associated with immune cell infiltration.These findings suggest that ZNF71 could be a novel target for diagnosing and treating HCC.

文摘This paper investigates load-pull measurement of AlGaN/GaN high electron mobility transistors(HEMTs) at different numbers of gate fingers.Scalable small-signal models are extracted to analyze the relationship between each model's parameters and the number of device's gate fingers.The simulated S-parameters from the small-signal models are compared with the reflection coefficients measured from the load-pull measurement system at X-band frequencies of 8.8 and 10.4 GHz.The dependency between the number of device's gate fingers and load-pull characterization is presented.
