基于纳支法不同频率电针治疗上运动神经源性肠功能障碍临床研究

目的:探讨电针刺激特定穴位不同的针刺时间、频率参数对上运动神经源性肠功能障碍患者的临床干预效应。方法:将60例患者随机分为A、B、C与D组,每组15例,常规针刺取穴支沟、大肠俞、足三里、次髎、气海、长强、天枢与上巨虚;针刺得气后... 目的:探讨电针刺激特定穴位不同的针刺时间、频率参数对上运动神经源性肠功能障碍患者的临床干预效应。方法:将60例患者随机分为A、B、C与D组,每组15例,常规针刺取穴支沟、大肠俞、足三里、次髎、气海、长强、天枢与上巨虚;针刺得气后电针治疗仪连接,以天枢、大肠俞为刺激电极,足三里、次髎为参考电极;A组:连续波5 Hz,辰时(7-9时)电针20 min;B组:连续波5 Hz,非辰时电针20 min;C组:连续波100 Hz,辰时(7-9时)电针20 min;D组:连续波100 Hz,非辰时电针20 min。各组针刺留针30 min。比较各组治疗前后国际脊髓损伤肠功能基础数据集评分、布里斯托尔大便分型(Bristol分型)、大肠标志物在肠内残余数量和生活综合评定量表(GQOL-74量表)。结果:与治疗前比较,各组患者治疗后国际脊髓损伤肠功能基础数据集评分、Bristol分型、大肠标志物在肠内残余数量与GQOL-74量表均有改善(P<0.05),其中国际脊髓损伤肠功能基础数据集评分、大肠标志物在肠内残余数量和GQOL-74量表C组患者改善程度均优于A组、B组和D组(P<0.05),A组与D组均优于B组(P<0.05),A组与D组疗效差异无统计学意义(P>0.05)。在Bristol分型中,C组改善程度均优于A组、B组和D组(P<0.05),A组、B组和D组疗效差异无统计学意义(P>0.05)。结论:在上运动神经源性肠功能障碍治疗上,高频辰时电针疗效最佳,能够有效缓解患者排便时间长、大便干燥与排便困难等症状,改善结肠传输功能,提高患者生活质量,疗效明确,值得临床推广。 展开更多

目的探讨肠溶阿司匹林对老年性肠运动功能异常性腹泻的治疗效果。方法对本院2007年1月至2011年7月期间本院收治的86例老年肠运动功能异常性腹泻患者的临床资料进行回顾性分析,将其随机分为观察组和对照组,对照组给予整肠生胶囊2粒每次,... 目的探讨肠溶阿司匹林对老年性肠运动功能异常性腹泻的治疗效果。方法对本院2007年1月至2011年7月期间本院收治的86例老年肠运动功能异常性腹泻患者的临床资料进行回顾性分析,将其随机分为观察组和对照组,对照组给予整肠生胶囊2粒每次,每天3次口服,观察组在应用整肠生胶囊的基础上加用肠溶阿司匹林每次0.1g,每天3次口服,统计治愈率及不良反应发生率,评价治疗效果。结果观察组43例,2d后治愈30例,治疗2d时的治愈率为69.77%,对照组43例,2d后治愈18例,治疗2d时治愈率为41.86%,观察组治愈率明显高于对照组,P<0.01,具有统计学意义,两组均未发生明显不良反应。结论肠溶阿司匹林能够减慢肠蠕动,促进营养物质的吸收,并且具有抗炎作用,能够调节菌群失调,抑制肠道炎症,与地衣芽孢杆菌制剂整肠生胶囊联合使用,对老年肠运动功能异常性腹泻具有良好的治疗效果,并且无明显的不良反应。 展开更多

目的评价地衣芽孢杆菌活菌胶囊联合阿司匹林肠溶片治疗老年肠运动功能异常性腹泻患者的临床效果。方法选取开封市人民医院2018年1月至2019年12月老年肠运动功能异常性腹泻患者122例,依据随机数表法分为对照组和研究组,各61例。给予对照... 目的评价地衣芽孢杆菌活菌胶囊联合阿司匹林肠溶片治疗老年肠运动功能异常性腹泻患者的临床效果。方法选取开封市人民医院2018年1月至2019年12月老年肠运动功能异常性腹泻患者122例,依据随机数表法分为对照组和研究组,各61例。给予对照组地衣芽孢杆菌活菌胶囊治疗,给予研究组地衣芽孢杆菌活菌胶囊联合阿司匹林肠溶片治疗。对比两组临床疗效、症状(排便次数、大便性状、肠鸣音)改善情况、不良反应发生状况以及治疗前后肠道菌群(大肠埃希菌、粪肠球菌、双歧杆菌、乳酸杆菌)数量。结果研究组总有效率[96.72%(59/61)]高于对照组[72.13%(44/61)](P<0.05);研究组肠鸣音、大便性状、排便次数恢复时间短于对照组(P<0.05);研究组不良反应发生率[3.28%(2/61)]与对照组[6.56%(4/61)]比较,差异无统计学意义(P>0.05);治疗后研究组大肠埃希菌、粪肠球菌数量少于对照组,双歧杆菌、乳酸杆菌数量多于对照组(P<0.05)。结论地衣芽孢杆菌活菌胶囊联合阿司匹林肠溶片应用于老年肠运动功能异常性腹泻效果显著,可改善临床症状,调节肠道菌群,安全性高。 展开更多

目的:了解中国优秀运动员参加大赛前的肺功能状况与相关因素的影响。方法:对参加2006年多哈亚运会的中国国家队285名运动员(共4个项群、27个运动项目)进行了系统肺功能检测与相关因素问诊。结果:(1)全体受检运动员中出现肺通气功能异常... 目的:了解中国优秀运动员参加大赛前的肺功能状况与相关因素的影响。方法:对参加2006年多哈亚运会的中国国家队285名运动员(共4个项群、27个运动项目)进行了系统肺功能检测与相关因素问诊。结果:(1)全体受检运动员中出现肺通气功能异常者78名,小气道功能减弱者59名,两项共137名,占受检总数的48.1%,尤以耐力类项群和对抗类项群居多,且VC、FVC、FEV1/FVC%各项群间存在显著性差异(P<0.01)。(2)既往患肺炎、支气管炎运动员的FEV1/FVC%、MEF50%及MEF25%显著低于既往其他疾病组和无既往疾病组(P<0.05),且肺通气功能异常及小气道功能减弱者百分比也高于另两组运动员。结论:中国优秀运动员的肺功能异常主要与项群特点有关,既往肺炎、支气管炎病史也对其具有一定提示作用。 展开更多

Is there an acute exercise-induced physiological/biochemical threshold which triggers increased speed of cognitive functioning? A meta-analytic investigation 被引量:3

Purpose: The purpose of this study was to examine, using meta-analytic measures, the evidence regarding the optimal exercise intensity at which improvements in speed of cognitive function are triggered. Specifically, it was hypothesized that the catecholamine, lactate, and ventilatory thresholds is the point at which significant improvements in speed of cognitive function are observed. Methods: We compared mean effect sizes for threshold studies and for those studies where exercise intensity was classed as moderate (40%-- 79% VO2 or equivalent) but in which the thresholds were not measured. Results: Random effects meta-analysis showed significant, moderate, mean effect sizes for studies at the threshold (g = 0.58, Z = 2.98, p 〈 0.003) and for those during moderate intensity exercise but in which the threshold was not measured ( g = 0.54, Z = 5.0l, p 〈 0.001). There was no significant difference between mean effect sizes, which suggests that the thresholds are unlikely to represent a trigger point. Conclusion: Moderate intensity exercise, even below the thresholds, can induce improved speed of cognition, possibly due to a combination of increased peripheral catecholamine concentrations inducing vagal/nucleus tractus solitarii pathway activation and central increases due to perceptions of stress. 展开更多

Objective: To study the effects of highly selective vagotomy plus resection of antral mucosa (HSV+RAM) or highly selective vagotomy (HSV) alone on the motility function of the pyloric antrum. Methods: 48 patients with duodenal ulcer were studied. 18 dogs were employed as experimental animals. 20 patients were operated on with HSV and 28 with HSV+RAM. The frequency of gastric evacuation and the amplitude of electrogastrography were determined 4 to 6 months after operation. 18 dogs were divided into the control group, HSV group and HSV+RAM group. The time of gastric evacuation, antral myoelectric activity and antral pressure were determined in the dogs 4 to 6 months after operation. The preoperative findings of the patients and the control dogs served as the control. Results: After operation, barium meal revealed that the shape of the stomach and duodenum was normal and the gastric peristalsis was clearly visible in human patients and experimental dogs. In the HSV+RAM group of dogs, the initial evacuation time was (5.0+0.06) min and the time of complete evacuation was (4.0+0.4) h after food-taking, which were similar to those of the control and the HSV group of dogs (P>0.05). The frequency of the antral myoelectric action potential was (3.11+0.65) cycles/min in the dog HAS+RAM group and the frequency of electrogastrography was (3.25+0.75) cycles/min in the human HSV+RAM group, which were significantly lower than those of the control and the dog and human HSV groups (P<0.05). Injection of pentagastrin in dogs and food-taking in human beings significantly increased the antral pressure and the amplitude and frequency of electrogastrography. Conclusion: The findings of this study suggest that the motility function of the reconstructed pyloric antrum in the HSV+RAM group of both the experimental dogs and human patients approaches to the normal even though there is a decrease of antral myoelectric frequency. It is suggested that HSV+RAM should be the first choice for the surgical management of duodenal ulcer. 展开更多

Background: Exercise-associated menstrual dysfunction (EAMD) is a common health problem in female athletes as a part of female athlete triad (FAT), a condition related to low energy availability. In this study, we explored the possibility that carbohydrate supplements can improve the status of EAMD and prevent exercise-induced ovarian injury in a FAT rat model. This research aimed to provide experimental evidence with regard to the relationship of energy intervention and EAMD. Methods: Forty-five female Sprague-Dawley rats (2 months old) were randomly divided into five experimental groups: control group (C), 9-week exercise as model for EAMD (E), post-EAMD recovery group (R), oligosaccharide intervention group (O), and glucose intervention group (G). All rats were sacrificed at the end of 9 weeks. Serum samples were collected for measuring gonadotropin releasing hormone, follicle stimulating hormone, luteinizing hormone, 1713-estradiol and progesterone levels. The ovaries were taken for investigation of exercise- and carbohydrate-induced follicular subcellular structure changes. Results: Exercise induced irregular menstrual cycles and ovary subcellular structural damages, such as swollenness of mitochondria in rats from groups E and R. Both glucose and oligosaccharide supplements restored well-differentiated mitochondria in the ovarian follicular cells, and a significant improvement of endoplasmic reticulum and Golgi in swollenness in theca cells in groups O and G compared to groups C, E, and R. There was no difference in mitochondria subcellular structural changes between groups O and G. Group E showed attenuation of serum levels of 17β-estradiol and progesterone compared to C. There were no differences of 17β-estradiol serum levels among groups O, G, and R, while group G showed a lower level of progesterone than C. Conclusion: Female adult rats with 9-week continuous exercise can cause menstrual dysregulation as a model for EAMD. Post-EAMD intervention with glucose and oligosaccharide intake can normalize the menstrual cycle, restore the follicular subcellular structure, and reverse the exercise-induced reduction of ovary sex hormones. It suggests a positive feedback of hypothalamus-pituitary-ovary axis might be involved in the molecular mechanisms of energy intake in treating EAMD. 展开更多

Objective:To investigate the potential mechanism of electroacupuncture(EA)at bilateral Fengchi(GB20)in treating cerebral ischemia-reperfusion injury and to provide a scientific basis for future experimental research and clinical applications.Methods:Forty male specific-pathogen-free Sprague-Dawley rats were randomly divided into four groups:a normal group,a normal with EA group,a model group,and a model with EA group,with 10 rats in each group.The normal group received no intervention.The normal with EA group received EA at bilateral Fengchi(GB20).The model group underwent middle cerebral artery occlusion(MCAO)using the suture.The model with EA group underwent MCAO and received EA at bilateral Fengchi(GB20).Cerebral blood flow was monitored using a laser Doppler cerebral blood flow meter.Neurologic damage was assessed using the neurologic deficit score,and motor ability was observed using the CatWalk gait system.The expression of glial fibrillary acidic protein(GFAP)and neuronal nuclei(NeuN)protein,the neuron markers,was detected by Western blotting.The protein expression levels of GFAP and NeuN,as well as the number of positive cells in the motor cortex,were detected using immunofluorescence.Results:Compared to the normal group,the cerebral blood flow values in the model group and the model with EA group decreased by more than 50%during the modeling process(P<0.01)and returned to pre-modeling levels after reperfusion(P>0.05).The neurologic deficit score increased(P<0.05),the average motor velocity decreased(P<0.05),GFAP protein expression and the number of positive cells in the motor cortex increased(P<0.05),and the NeuN protein expression and the number of positive cells decreased(P<0.05)in the model group.Compared to the model group,the neurologic deficit score decreased(P<0.05),the average motor velocity accelerated(P<0.05),GFAP and NeuN protein expression and the number of positive cells in the motor cortex increased(P<0.01)in the model with EA group.Conclusion:EA at bilateral Fengchi(GB20)can reduce neuronal loss and increase GFAP and NeuN protein expression in the motor cortex of rats after ischemia-reperfusion,improve the motor function after ischemic stroke,and accelerate the recovery of balance and stability of the affected limbs. 展开更多

OBJECTIVE: To observe long-term functional recovery after contralateral C7 transfer. METHODS: From August 1986 to July 2000, 224 patients with brachial plexus avulsion injuries were treated with contralateral C7 transfer in our department. Thirty-two patients were followed up for over 2 years for evaluation of the following items: 1 influence on healthy limb function; 2 sensory and motor recovery of the recipient nerves in the affected limb; and 3 coordination between the healthy and affected limbs. RESULTS: There was no impairment of healthy limb function. Functional recovery of the recipient area reached > or =M3 in 8 patients (8/10, 80%) after musculocutaneous nerve neurotization, > or =M3 in 4 patients (4/6, 66%) after radial nerve neurotization, > or = M3 in 7 patients (7/14, 50%) and > or = M3 in 12 patients (85.7%) after median nerve neurotization, and > or = M3 in 1 patients (1/2, 50%) after thoracodorsal nerve neurotization. Synchronic contraction of the affected limb with the healthy limb occurred within 2-3 years in 12 patients, within 5 years in 13 patients, and over 5 years in 7 patients. CONCLUSION: Contralateral C7 transfer is an ideal procedure for the treatment of brachial plexus root avulsion injury. Selection of the whole root or the posterior division as neurotizer and a staged operation are the major factors influencing treatment outcome. 展开更多

OBJECTIVE:To investigate the effects of different frequencies of electro-acupuncture at Shuigou(GV 26) on the latent period and wave amplitude of motor evoked potentials(MEPs) in rats with focal cerebral infarction.METHODS:Fifty healthy male Wistar rats were randomly divided into five groups:controls,model,2 Hz Shuigou,50 Hz Shuigou and 100 Hz Shuigou.There were 10 rats in each group.Using a modification of a technique for middle cerebral artery occlusion,focal cerebral ischemic injury was induced in all rats except those in the control group.The rats in the control group received no treatment.After behavioral deficit had been evaluated using the Zausinger 6-point neurological function score,therats in the Shuigou groups underwent acupuncture and continuous wave stimulation at a frequency of 2 Hz,50 Hz or 100 Hz(intensity 1 mA) for 10 min twice daily for 3 days.The control and model groups underwent no intervention.Zausinger 6-point neurological function score and MEPs were measured 72 h after the start of treatment.RESULTS:The neurological function scores of the three Shuigou groups were significantly higher than those of the model group(P<0.05).There was no significant difference between sides in the latency and amplitude of MEPs in the model group(P> 0.05).The latency on the affected side in the model group was significantly longer than that in the control group(P<0.05) and the amplitude on affected side was significantly reduced(P<0.01).After 3 days of electro-acupuncture,the latency on the affected side in the 2 Hz Shuigou group was significantly shortened(P<0.05) and the amplitude was significantly increased(P<0.05) compared with the model group.CONCLUSION:Low frequency electro-acupuncture at Shuigou(GV 26) can promote recovery of motor function after focal cerebral ischemic injury in rats. 展开更多
