In countries with intermediate or high endemicity for HBV, exacerbations of chronic hepatitis B may be the first presentation of HBV infection. Some of these patients may be diagnosed mistakenly as having acute hepatitis B. Accurate diagnosis in these cases is very important for deciding whether to start treatment or not, because acute hepatitis B does not require therapy, while exacerbation of chronic hepatitis may benefit from it. Clinical and routine laboratory findings cannot help distinguishing between these two conditions. Therefore, several assays have been proposed for this purpose during the last few years. The presence of high levels of anti-HBe antibodies, HBsAg and HBV DNA are typical of chronic disease, whereas high titers of IgM anti-HBc, together with their high avidity index, characterize acute HBV infection. Starting from the description of a patient with acute hepatitis B-who recently came to our observation-we critically review the currently available assays that may help distinguishing between the different conditions and lead to the optimal management of each patient.展开更多

We present a female pa...To show that brucellosis may trigger autoimmune hepatitis(AIH), in addition to nonspecific liver involvement and toxic hepatitis, due to a class effect of tetracycline family used for treatment. We present a female patient admitted to our hospital due to partially improved fatigue and elevated liver enzymes following doxycycline and streptomycin usage for brucellosis. Brucellosis is endemic in our country, Turkey. It may involve any organ in the body. Liver is frequently involved. Doxycycline used for treatment occasionally may lead to hepatotoxicity. AIH is a necroinflammatory disease of the liver. Certain drugs (e.g. Minocycline), toxins, and viruses (hepatitis B, hepatitis C, EBV, etc.) can trigger AIH. Only one case of AIH probably caused by doxycycline and brucellosis was reported. We discuss the relationship between brucellosis, AIH, and hepatotoxicity of doxycycline. Autoimmune hepatitis(AIH) has rarely been described as an autoimmune paraneoplastic syndrome of thymoma.This case is the seventh case of AIH revealed by cholestasis few years after the diagnosis of thymoma and the fir...Autoimmune hepatitis(AIH) has rarely been described as an autoimmune paraneoplastic syndrome of thymoma.This case is the seventh case of AIH revealed by cholestasis few years after the diagnosis of thymoma and the first case treated with chemotherapy alone.We report in this paper a new approach to this rare severe condition.A 29 year-old man presented with chest pain and dyspnea with a history of thymoma surgically removed 4 years ago.CT scan showed the recurrence of an anterior mediastinal mass.Biology showed elevated liver enzymes and profound cholestasis.No sign of viral or toxic hepatitis or bile duct abnormalities were observed.Autoimmune antibodies,except for the anti-nuclear antibody,were negative.Liver biopsy showed active chronic AIH.The patient was diagnosed with recurrent thymoma with AIH and underwent 6 cycles of chemotherapy.A complete response on thymoma and cholestasis was obtained after 10 months of follow-up.Steroids and immunosuppressors are the standard treatment for AIH.The effect of chemotherapy as a specific treatment of this paraneoplastic syndrome needs to be considered.展开更多
AIM:To study the coincidence of celiac disease, we tested its serological markers in patients with various liver diseases.METHODS:Large-scale screening of serum antibodies against tissue transglutaminase (tTG), and de...AIM:To study the coincidence of celiac disease, we tested its serological markers in patients with various liver diseases.METHODS:Large-scale screening of serum antibodies against tissue transglutaminase (tTG), and deamidated gliadin using enzyme-linked immunosorbent assay and serum antibodies against endomysium using immunohistochemistry, in patients with various liver diseases (n = 962) and patients who underwent liver transplantation (OLTx, n = 523) was performed. The expression of tTG in liver tissue samples of patients simultaneously suffering from celiac disease and from various liver diseases using immunohistochemistry was carried out. The final diagnosis of celiac disease was confirmed by histological analysis of small-intestinal biopsy. RESULTS:We found that 29 of 962 patients (3%) with liver diseases and 5 of 523 patients (0.8%) who underwent OLTx were seropositive for IgA and IgG anti-tTG antibodies. However, celiac disease was biopsy-diagnosed in 16 patients:4 with autoimmune hepatitis type Ⅰ, 3 with Wilson's disease, 3 with celiac hepatitis, 2 with primary sclerosing cholangitis, 1 with primary biliary cirrhosis, 1 with Budd-Chiari syndrome, 1 with toxic hepatitis, and 1 with non-alcoholic steatohepatitis. Unexpectedly, the highest prevalence of celiac disease was found in patients with Wilson's disease (9.7%), with which it is only rarely associated. On the other hand, no OLTx patients were diagnosed with celiac disease in our study. A pilot study of the expression of tTG in liver tissue using immunohistochemistry documented the overexpression of this molecule in endothelial cells and periportal hepatocytes of patients simultaneously suffering from celiac disease and toxic hepatitis, primary sclerosing cholangitis or autoimmune hepatitis type Ⅰ. CONCLUSION:We suggest that screening for celiac disease may be beneficial not only in patients with associated liver diseases, but also in patients with Wilson's disease.展开更多

Serologic tests ruled out infectious and autoimmune causes apart from drug toxicity. During clinical examinations, he was found to have generalized enlarged lymph nodes that were then taken biopsy. It was diagnosed as classical Hodgkin's lymphoma based on histological examination of the lymph node. A bone marrow biopsy showed marrow infiltration by Hodgkin's lymphoma with hemophagocytosis and secondary myelofibrosis. A percutaneous liver biopsy demonstrated hepatic involvement of the same disease. After an extensive work-up, the cause of liver failure was figure out to be only attributed to the involvement of the lymphoma. Hodgkin's lymphoma as a cause of hepatic failure is rare and young patients diagnosed as Hodgkin's lymphoma causing hepatic failure has been reported very rarely so far.展开更多

Four types of immune c...Mathematical model that describes breast cancer and immune interactions were presented using system of differential equations to provide analytic and nnmeric framework of cancer-immune dynamics. Four types of immune cells-CTLs (cytotoxic T lymphocytes), macrophages, NK (natural killer) and helper T cells-known to play the most significant roles in developing breast cancer immunity were modeled using differential equations. The model was then applied to different cancer growth rates and simulated using MATLAB software tool. The parameters of the model were based on experimental and clinical results from published articles. Results supported clinical studies that maximal breast cancer immunity depends mostly on each of the four immune cell types chosen. It was observed that for a given breast cancer growth rate, there was an optimal activation that maximized the response of the immune system. The effectiveness of the immune system resulted in the decrease in breast cancer killing rates. These results highlighted the importance of immune system activations in breast cancer development and treatment. Therefore, the model and its simulation provided a robust framework to better understand breast cancer progression and response to the immune system.展开更多
Huanglian Jiedu Decoction(HLJDD)is a quintessential prescription renowned for its heat-clearing and detoxifying properties.It is primarily prescribed to counteract the syndrome characterized by the excessive heat of t...Huanglian Jiedu Decoction(HLJDD)is a quintessential prescription renowned for its heat-clearing and detoxifying properties.It is primarily prescribed to counteract the syndrome characterized by the excessive heat of the Sanjiao fire.Notably,the hyperactivity of liver fire is frequently linked with hypertension,where wind fire and wind toxicity emerge as pivotal pathogenic factors.This study aimed to investigate the impact of HLJDD on the endoplasmic reticulum in spontaneously hypertensive rats(SHR),further delving into the interplay between endoplasmic reticulum stress(ERS)and myocardial remodeling and damage.Fifty SHR rats were stratified randomly into five cohorts:model,low-dose HLJDD,medium-dose HLJDD,high-dose HLJDD,and captopril groups.For comparison,a set of Wistar-Kyoto(WKY)rats served as the baseline control group,with each group comprising 10 rats.While the model and control groups received equivalent volumes of normal saline via gavage,the other groups were administered the respective drug dosages through the same route daily for a span of 6 weeks.Upon the experiment’s conclusion,metrics such as the heart mass index(HWI)and left ventricular mass index(LVWI)were assessed.Cardiac tissue anomalies were identified using H&E staining,while ERS-related protein and mRNA expression levels were ascertained via Western blotting analysis and qPCR.Moreover,TUNEL staining was employed to detect cardiomyocyte apoptosis.The findings indicated that increasing HLJDD concentrations corresponded with escalated HWI and LVWI in rat hearts(P<0.05).There was a marked enhancement in myocardial structural integrity,accompanied by a notable reduction in collagen fibers.The mRNA and protein expressions of myocardial inositol-dependent enzyme 1α(IRE1α),X-box binding protein 1(XBP1),glycoregulatory protein 78(GRP78),and CCAAT enhancer binding protein homologous protein(CHOP)in the medium and high-dose groups saw significant declines(P<0.05).These effects mirrored those observed in the captopril group.The study underscored HLJDD’s efficacy in mitigating myocardial tissue damage in SHR.This therapeutic effect was potentially attributed to the downregulation of IRE1α,XBP1,GRP78,and CHOP,curbing excessive ERS,diminishing cardiomyocyte apoptosis,and thereby conferring cardioprotection.展开更多
Hybrid antioxidants cinnamoyldopamine(2a), p-coumaroyldopamine(2b), caffeoyldopamine(2c), feruloyldopamine(2d) and sinapoyldopamine(2e) were synthesized by conjugation of dopamine(DA) and hydroxycinnamic a...Hybrid antioxidants cinnamoyldopamine(2a), p-coumaroyldopamine(2b), caffeoyldopamine(2c), feruloyldopamine(2d) and sinapoyldopamine(2e) were synthesized by conjugation of dopamine(DA) and hydroxycinnamic acids(HCAs). The stabilities were studied in buffers at p H 1.3, p H 5.0, and p H 7.4 including the human plasma. All the compounds were found highly stable at acidic p H, but underwent hydrolysis at neutral p H. Furthermore, the hydrolysis proceeded much faster in plasma in the following order as indicated by half-life values(t1/2), 2c(1.21 h)〈2e(1.52 h)〈2d(1.85 h)〈2b(3.38 h)〈2a(3.88 h), correlating with the number of electron-donating groups. It has been proven by UV spectrum that 2c, 2d, and 2e displayed red shift of more than 50 nm as compared to 2a and 2b, because of the presence of OH and OCH3 groups. In addition, the compounds(2b–e) showed no cytotoxicity on normal HUVEC cells as DA, although 2a displayed a 16% inhibition of proliferation at 40 μM following 48 h incubation. Their free radical-scavenging activities were evaluated using ABTS^*+ and superoxide anion assays and the mechanisms were proposed. It was found that they all exhibited higher activities than trolox, a recognized antioxidant. Amazingly, in the case of the hybrids(2a–e), their activity was higher than that of HCAs while lower or comparable to that of DA, suggesting that there may be a "saturation effect" with the hybrid molecules in the antioxidant activities.展开更多
To investigate the mechanism of effect of Huanglian Jiedu Decoction(HLJD) on vascular endothelium, we assessed the protective effect of HLJD on vascular endothelium in spontaneously hypertensive rats(SHR) and the expr...To investigate the mechanism of effect of Huanglian Jiedu Decoction(HLJD) on vascular endothelium, we assessed the protective effect of HLJD on vascular endothelium in spontaneously hypertensive rats(SHR) and the expression of RhoA in thoracic aorta. A total of 40 SHR rats were randomly and evenly divided into model group(SHR group), positive group(captopril group), HLJD high-dose group, and HLJD low-dose group. Simultaneously, 10 Wistar Kyoto rats were used in the blank group. All groups were treated by gavage for 6 weeks. The changes of nitric oxide synthase(NOS), von Willebrand factor(vWF), endothelin(ET-1) and calmodulin(CAM) in rat serum were tested by enzyme linked immunosorbent assay(ELISA) method. The expression of RhoA at the protein and mRNA levels in thoracic aorta was determined by Western blotting(WB) and quantitative real-time PCR, respectively. Compare with the blank group after 6 weeks, the levels of ET-l and VWF in serum of the model group were significantly increased(P<0.01), and the levels of NOS and CAM were significantly decreased(P<0.01). Conversely, the levels of ET-1 and vWF in the positive and HLJD groups were significantly decreased(P<0.01 or P<0.05), and the levels of NOS and CAM were significantly increased(P<0.01 or P<0.05) compared with the model group. The expression of Rho A at the mRNA and protein levels was decreased obviously(P<0.05) in HLJD high-dose group. The results shown that HLJD increased diastolic factor(CAM and NOS) in the vascular endothelial of rats, leading to reduced contraction factor(ET-1 and vWF). HLJD revealed the preventive function in the vascular endothelial dysfunction of the early stage hypertension through adjusting secretion of blood vessel endothelium(BVE) relaxing factor and improving vascular endothelial function. The mechanism might be associated with the inhibition of the activity of RhoA protein factor.展开更多

This work reveals that the antibacterial properties of these nano-metal oxides are strongly...The antibacterial properties of nano-metal oxides (ZnO, CuO) are based on the formation of reactive oxygen species (ROS). This work reveals that the antibacterial properties of these nano-metal oxides are strongly dependent on their crystalline structure. The antibacterial activity of the nanooxides was tested against four types of bacteria that commonly cause nosocomial infections. The sonochemical method was applied not only for synthesis of nanooxides but also to their coating on textiles. The antibacterial properties of textiles coated with commercial and sonochemically prepared nano-metal oxides were evaluated and compared. The toxicity was evaluated on human lung cells and amphibian embryos, as representative models for inhalation and aquatic toxicology. The sonochemically prepared metal nanooxides are better antimicrobials than commercially available metal oxides with the same particle size range. It was found that the crystallites which have more defects and less organized structure are more toxic. The formation of ROS was studied by electron spin resonance (ESR) measurements for both the sonochemically prepared and commercial samples of ZnO/CuO nanoparticles. A significant increase in the production of radical species was found in the more defective, sonochemically prepared samples, as compared to the commercial ones. Since modulation of the nanoparticle defects influenced their toxicity, the possibility of engineering safer nano-antibacterials is indicated.展开更多

We show that the complex dynamical system produces a Markov process at the later stages, whose eigenvectors corresponding to the eigenvalue 1 have physical significance for the long-term prognosis of the virus. It was observed that for a given breast cancer growth rate, there was an optimal activation that maximized the response of the immune system. The effectiveness of the immune system resulted in the decrease in breast cancer killing rates. These results highlighted the importance of immune system activations in breast cancer development and treatment. Therefore, the model and its simulation provided a robust framework to better understand breast cancer progression and response to the immune system.

文摘Huanglian Jiedu Decoction(HLJDD)is a quintessential prescription renowned for its heat-clearing and detoxifying properties.It is primarily prescribed to counteract the syndrome characterized by the excessive heat of the Sanjiao fire.Notably,the hyperactivity of liver fire is frequently linked with hypertension,where wind fire and wind toxicity emerge as pivotal pathogenic factors.This study aimed to investigate the impact of HLJDD on the endoplasmic reticulum in spontaneously hypertensive rats(SHR),further delving into the interplay between endoplasmic reticulum stress(ERS)and myocardial remodeling and damage.Fifty SHR rats were stratified randomly into five cohorts:model,low-dose HLJDD,medium-dose HLJDD,high-dose HLJDD,and captopril groups.For comparison,a set of Wistar-Kyoto(WKY)rats served as the baseline control group,with each group comprising 10 rats.While the model and control groups received equivalent volumes of normal saline via gavage,the other groups were administered the respective drug dosages through the same route daily for a span of 6 weeks.Upon the experiment’s conclusion,metrics such as the heart mass index(HWI)and left ventricular mass index(LVWI)were assessed.Cardiac tissue anomalies were identified using H&E staining,while ERS-related protein and mRNA expression levels were ascertained via Western blotting analysis and qPCR.Moreover,TUNEL staining was employed to detect cardiomyocyte apoptosis.The findings indicated that increasing HLJDD concentrations corresponded with escalated HWI and LVWI in rat hearts(P<0.05).There was a marked enhancement in myocardial structural integrity,accompanied by a notable reduction in collagen fibers.The mRNA and protein expressions of myocardial inositol-dependent enzyme 1α(IRE1α),X-box binding protein 1(XBP1),glycoregulatory protein 78(GRP78),and CCAAT enhancer binding protein homologous protein(CHOP)in the medium and high-dose groups saw significant declines(P<0.05).These effects mirrored those observed in the captopril group.The study underscored HLJDD’s efficacy in mitigating myocardial tissue damage in SHR.This therapeutic effect was potentially attributed to the downregulation of IRE1α,XBP1,GRP78,and CHOP,curbing excessive ERS,diminishing cardiomyocyte apoptosis,and thereby conferring cardioprotection.

文摘Hybrid antioxidants cinnamoyldopamine(2a), p-coumaroyldopamine(2b), caffeoyldopamine(2c), feruloyldopamine(2d) and sinapoyldopamine(2e) were synthesized by conjugation of dopamine(DA) and hydroxycinnamic acids(HCAs). The stabilities were studied in buffers at p H 1.3, p H 5.0, and p H 7.4 including the human plasma. All the compounds were found highly stable at acidic p H, but underwent hydrolysis at neutral p H. Furthermore, the hydrolysis proceeded much faster in plasma in the following order as indicated by half-life values(t1/2), 2c(1.21 h)〈2e(1.52 h)〈2d(1.85 h)〈2b(3.38 h)〈2a(3.88 h), correlating with the number of electron-donating groups. It has been proven by UV spectrum that 2c, 2d, and 2e displayed red shift of more than 50 nm as compared to 2a and 2b, because of the presence of OH and OCH3 groups. In addition, the compounds(2b–e) showed no cytotoxicity on normal HUVEC cells as DA, although 2a displayed a 16% inhibition of proliferation at 40 μM following 48 h incubation. Their free radical-scavenging activities were evaluated using ABTS^*+ and superoxide anion assays and the mechanisms were proposed. It was found that they all exhibited higher activities than trolox, a recognized antioxidant. Amazingly, in the case of the hybrids(2a–e), their activity was higher than that of HCAs while lower or comparable to that of DA, suggesting that there may be a "saturation effect" with the hybrid molecules in the antioxidant activities.

文摘To investigate the mechanism of effect of Huanglian Jiedu Decoction(HLJD) on vascular endothelium, we assessed the protective effect of HLJD on vascular endothelium in spontaneously hypertensive rats(SHR) and the expression of RhoA in thoracic aorta. A total of 40 SHR rats were randomly and evenly divided into model group(SHR group), positive group(captopril group), HLJD high-dose group, and HLJD low-dose group. Simultaneously, 10 Wistar Kyoto rats were used in the blank group. All groups were treated by gavage for 6 weeks. The changes of nitric oxide synthase(NOS), von Willebrand factor(vWF), endothelin(ET-1) and calmodulin(CAM) in rat serum were tested by enzyme linked immunosorbent assay(ELISA) method. The expression of RhoA at the protein and mRNA levels in thoracic aorta was determined by Western blotting(WB) and quantitative real-time PCR, respectively. Compare with the blank group after 6 weeks, the levels of ET-l and VWF in serum of the model group were significantly increased(P<0.01), and the levels of NOS and CAM were significantly decreased(P<0.01). Conversely, the levels of ET-1 and vWF in the positive and HLJD groups were significantly decreased(P<0.01 or P<0.05), and the levels of NOS and CAM were significantly increased(P<0.01 or P<0.05) compared with the model group. The expression of Rho A at the mRNA and protein levels was decreased obviously(P<0.05) in HLJD high-dose group. The results shown that HLJD increased diastolic factor(CAM and NOS) in the vascular endothelial of rats, leading to reduced contraction factor(ET-1 and vWF). HLJD revealed the preventive function in the vascular endothelial dysfunction of the early stage hypertension through adjusting secretion of blood vessel endothelium(BVE) relaxing factor and improving vascular endothelial function. The mechanism might be associated with the inhibition of the activity of RhoA protein factor.
文摘The antibacterial properties of nano-metal oxides (ZnO, CuO) are based on the formation of reactive oxygen species (ROS). This work reveals that the antibacterial properties of these nano-metal oxides are strongly dependent on their crystalline structure. The antibacterial activity of the nanooxides was tested against four types of bacteria that commonly cause nosocomial infections. The sonochemical method was applied not only for synthesis of nanooxides but also to their coating on textiles. The antibacterial properties of textiles coated with commercial and sonochemically prepared nano-metal oxides were evaluated and compared. The toxicity was evaluated on human lung cells and amphibian embryos, as representative models for inhalation and aquatic toxicology. The sonochemically prepared metal nanooxides are better antimicrobials than commercially available metal oxides with the same particle size range. It was found that the crystallites which have more defects and less organized structure are more toxic. The formation of ROS was studied by electron spin resonance (ESR) measurements for both the sonochemically prepared and commercial samples of ZnO/CuO nanoparticles. A significant increase in the production of radical species was found in the more defective, sonochemically prepared samples, as compared to the commercial ones. Since modulation of the nanoparticle defects influenced their toxicity, the possibility of engineering safer nano-antibacterials is indicated.
文摘We analyze a Markov cellular automaton that models the spread of viruses that often progress to a chronic condition, such as human immunodeficiency virus (HIV) or hep- atitis C virus (HCV). We show that the complex dynamical system produces a Markov process at the later stages, whose eigenvectors corresponding to the eigenvalue 1 have physical significance for the long-term prognosis of the virus. Moreover we show that drug treatment leads to chronic conditions that can be modeled by Markov shifts with more optimal eigenveetors.

文摘A facile approach to assembled virus film with tunable structure is presented.Rod-like tobacco mosaic virus (TMV) was selected as the prototype in this study for its anisotropic structural feature.TMV can either "lie down" or "stand up" on gold substrate by tuning the solution pH.A quartz crystal microbalance with dissipation monitoring was used to monitor the pH-dependent self-assembly behavior of TMV nanoparticles,and atomic force microscopy and single molecule force spectroscopy further confirmed the different assembly structures.
