血清sIL-2R联合TNF-α、IgG、IgA对多发性骨髓瘤患者复发的预测价值

目的:探讨血清可溶性白细胞介素-2受体(soluble interleukin-2 receptor,sIL-2R)、肿瘤坏死因子-α(TNF-α)、IgG、IgA对多发性骨髓瘤(multiple myeloma,MM)复发的预测价值。方法:选取2017年1月-2019年3月在本院初诊且治疗的MM患者108例... 目的:探讨血清可溶性白细胞介素-2受体(soluble interleukin-2 receptor,sIL-2R)、肿瘤坏死因子-α(TNF-α)、IgG、IgA对多发性骨髓瘤(multiple myeloma,MM)复发的预测价值。方法:选取2017年1月-2019年3月在本院初诊且治疗的MM患者108例,最终确定符合诊断标准且有完整随访资料的72例患者作为研究对象,以MM复发为研究终点事件,随访至终点事件的发生或本研究随访截止日期。根据是否复发,将MM分为复发组(RG)和非复发组(NRG)。在初诊时和随访后(终点事件的发生或研究终止时)分别采集患者静脉血,酶联免疫吸附测定法(ELISA)检测患者血清中sIL-2R和TNF-α水平;自动免疫分析仪检测患者血清中IgG、IgA水平;比较两组之间各因子表达水平的差异,并分析初诊时和随访后sIL-2R与TNF-α、IgG、IgA之间的相关性。同时在治疗完全缓解时采集患者静脉血,测定血清中sIL-2R水平,比较初诊时、完全缓解时和复发时sIL-2R表达水平的差异。采用受试者工作特征(ROC)曲线判断血清sIL-2R、TNF-α、IgG、IgA的最佳截断值,并根据曲线下面积(AUC)分析sIL-2R、TNF-α、IgG、IgA在MM复发中的预测价值。结果:MM患者初诊时和复发时血清sIL-2R水平均明显高于完全缓解时(P<0.05)。复发组初诊时的血红蛋白(Hb)含量低于未复发组,β2-微球蛋白(β2-MG)含量高于未复发组(P<0.001),两组间的其他临床参数无显著差异(P>0.05)。复发组初诊时和随访后的sIL-2R、TNF-α、IgG、IgA水平均明显高于未复发组(P<0.05),sIL-2R与TNF-α、IgG和IgA在初诊时和随访后均存在显著正相关(P<0.001)。ROC曲线分析结果显示,初诊时sIL-2R、TNF-α、IgG和IgA预测MM的AUC分别为0.919、0.850、0.766和0.795,随访后预测MM的AUC分别为0.890、0.815、0.760和0.794(P<0.001)。结论:血清sIL-2R对MM复发的预测价值最高,可在特定的时间检测TNF-α、IgG和IgA水平来推断sIL-2R水平的变化,从而评估患者的预后。 展开更多

鼻咽癌放疗后患者内毒素和可溶性白细胞介素2受体水平与化脓性中耳炎发生的相关性

目的探究鼻咽癌放疗后患者内毒素和可溶性白细胞介素2受体(soluble interleukin-2 receptor,sIL-2R)表达水平与化脓性中耳炎发生的相关性。方法回顾性分析2022年1月~2023年7月在南阳市中心医院接受放疗的79例鼻咽癌患者作为研究对象,对... 目的探究鼻咽癌放疗后患者内毒素和可溶性白细胞介素2受体(soluble interleukin-2 receptor,sIL-2R)表达水平与化脓性中耳炎发生的相关性。方法回顾性分析2022年1月~2023年7月在南阳市中心医院接受放疗的79例鼻咽癌患者作为研究对象,对其进行为期1年的随访。根据随访期间化脓性中耳炎发生情况进行分组,其中发生组21例,未发生组58例,对比两组患者的基线资料,并在放疗结束时检测内毒素和sIL-2R表达水平,分析鼻咽癌放疗后内毒素、sIL-2R与化脓性中耳炎发生的关系。结果79例鼻咽癌患者放疗后共有21例(26.58%)发生化脓性中耳炎。与未发生组相比,发生组患者放疗时间、内毒素、sIL-2R、TNM分期Ⅲ期、咽鼓管受累、鼻腔结构异常、炎性反应、腭帆张肌萎缩程度≥30%均升高,差异有统计学意义(P均<0.05)。经Logistic多因素分析结果显示,炎性反应、内毒素、鼻腔结构异常、TNM分期Ⅲ期、sIL-2R、咽鼓管受累、放疗时间、腭帆张肌萎缩程度≥30%均属于化脓性中耳炎的风险因素。经Spearman秩相关分析结果显示,内毒素和sIL-2R与化脓性中耳炎发生呈正相关(r=0.493、0.516,P<0.001)。ROC曲线分析结果显示,内毒素:AUC值为0.657,敏感度为61.90%,特异度为72.41%,准确度为69.62%,95%CI=0.523~0.791;sIL-2R:AUC值为0.697,敏感度为71.43%,特异度为67.24%,准确度为68.35%,95%CI=0.566~0.829;联合检测:AUC值为0.804,敏感度为95.24%,特异度为65.52%,准确度为73.42%,95%CI=0.705~0.903,两项联合检测价值最佳。结论鼻咽癌放疗后患者内毒素和sIL-2R水平升高可能与化脓性中耳炎发生相关。 展开更多

目的:研究2型固有淋巴样细胞(ILC2)和髓源性抑制细胞(MDSC)及其相关细胞因子IL-13和诱导型一氧化氮合酶(iNOS)在宫颈癌(CC)中的表达,并基于其构建CC发病风险的列线图预测模型。方法:采集2022年5月至2024年1月在新疆医科大学第一附属医... 目的:研究2型固有淋巴样细胞(ILC2)和髓源性抑制细胞(MDSC)及其相关细胞因子IL-13和诱导型一氧化氮合酶(iNOS)在宫颈癌(CC)中的表达,并基于其构建CC发病风险的列线图预测模型。方法:采集2022年5月至2024年1月在新疆医科大学第一附属医院手术切除的40例CC组织及100例外周血作为CC组,选取同期收治的30例子宫肌瘤经CC筛查为阴性的宫颈组织和100例正常健康个体外周血作为对照组。用多重免疫荧光技术(mIF)及免疫组化染色(IHC)法检测两组组织中ILC2和MDSC细胞浸润及其相关细胞因IL-13和iNOS的表达;使用流式细胞术和ELISA技术分别检测两组外周血中ILC2和MDSC及IL-13和iNOS的表达差异;通过Person相关性分析评估其相关性;使用单因素和多因素Logistic分析来确定ILC2和MDSC及IL-13和iNOS是否为CC发病的独立危险因素,再利用R软件建立免疫预测模型,使用ROC曲线下面积(AUC值)、Hosmer-Lemeshow检验、校准曲线、临床决策曲线和临床影响曲线来分别评估模型。结果:CC组中ILC2及MDSC及其相关细胞因子IL-13和iNOS均高于对照组(均P<0.05),且其均呈正相关(均P<0.05);经过单因素及多因素Logistic回归分析显示,ILC2、MDSC及IL-13、iNOS均是CC发病的独立危险因素(均P<0.05);基于这些危险因素的CC发病列线图,经过验证提示,该列线图模型具有一定的临床实用价值。结论:ILC2和MDSC及其相关的细胞因子IL-13和iNOS在CC组织及外周血中均呈高表达,基于这些危险因素构建的预测模型具有良好的预测能力和一定的实用性,为CC的早期诊断和治疗提供了一种简便有效的辅助工具。 展开更多

Results: The slL-2R level in serum was significantly higher and the Na^+ -K^+ -exchanging ATPase activity in semen significantly lower in AsAb positive infertile men when compared with thecontrols. Conclusion: The AsAb titer varies with the slL-2R level in serum. A decrease in Na^+ -K^+ -exchangingATPase activity in semen may play a role in male infertility caused by AsAb. 展开更多

老年肺结核患者血清TNFSF14、sIL-2R水平及与短期预后的相关性研究 被引量:1

目的:分析老年肺结核患者血清肿瘤坏死因子超家族成员14(tumor necrosis factor superfamily 14,TNFSF14)、可溶性白细胞介素2受体(soluble interleukin 2 receptor,sIL-2R)水平变化及与短期预后的相关性。方法:选择2019年6月至2022年7... 目的:分析老年肺结核患者血清肿瘤坏死因子超家族成员14(tumor necrosis factor superfamily 14,TNFSF14)、可溶性白细胞介素2受体(soluble interleukin 2 receptor,sIL-2R)水平变化及与短期预后的相关性。方法:选择2019年6月至2022年7月于长沙市第一医院接收的80例老年肺结核患者作为观察组,另选同期80例老年健康体检者作为对照组。观察组治疗后随访3个月,按照患者短期预后情况分为预后良好组(65例)和预后不良组(15例)。采用logistic回归分析血清TNFSF14、sIL-2R与短期预后的关系。结果:观察组血清TNFSF14水平为(12.55±1.20)ng/L,sIL-2R水平为(255.80±25.15)pmol/L,均高于对照组[(3.80±0.53)ng/L和(100.96±10.10)pmol/L],差异均有统计学意义(t=9.434,P<0.001;t=8.166,P<0.001)。化疗后随访3个月发现,观察组痊愈10例(12.50%),有效55例(68.75%),无效15例(18.75%);预后良好合计65例(81.25%),预后不良合计15例(18.75%)。预后不良组血清TNFSF14水平为(14.35±1.51)ng/L,sIL-2R水平为(275.64±27.10)pmol/L,均高于预后良好组[(12.09±1.18)ng/L和(253.33±25.13)pmol/L],差异均有统计学意义(t=6.334,P<0.001;t=3.055,P=0.003)。预后不良组的痰菌阳性比例为93.33%(14/15),高于预后良好组的58.46%(38/65),差异有统计学意义(χ^(2)=6.515,P=0.011)。经logistic回归分析发现,血清TNFSF14水平升高(OR=8.913,95%CI:1.993~48.002)和sIL-2R水平升高(OR=7.132,95%CI:1.054~35.172)、痰菌阳性(OR=9.947,95%CI:1.233~80.253)均是导致老年肺结核患者预后不良的危险因素。ROC曲线显示,血清TNFSF14、sIL-2R及二者联合预测老年肺结核患者短期预后的AUC分别为0.703(95%CI:0.549~0.857)、0.726(95%CI:0.579~0.873)和0.901(95%CI:0.818~0.999)。结论:老年肺结核患者血清TNFSF14、sIL-2R均呈高表达,TNFSF14、sIL-2R表达升高会增加患者短期不良预后发生的风险,临床应密切监测二者表达,以预测患者不良预后的发生。 展开更多

Objective The current study was designed to find out the effect of Glucosidorum Tripterygii Totorum (GTT) on the serum level of soluble interleukin-2 receptor (sIL-2R) in patient with ankylosing spondylitis (AS). Method 29 patients with active AS were selected to take GTT (1mg per kg) three times a day for one year. After that, its curative effect was evaluated. The serum level of sIL-2R of these patients was measured by sandwich ELISA method and was compared with that of normal subjects. Result The serum level of sIL-2R in active AS patients was obviously higher than that of the non-active AS patients. (P<0.01). The total effective rate of GTT on AS was 89.6%, while clinical relief rate 27.6%, obvious effective rate 44.8%, effective rate 17.2% and non-effective rate 10.4%. The patients’ serum level of sIL-2R after therapy was significantly lower than that before therapy except patients with no effect. (P<0.05). Conclusion GTT has positive curative effect on active AS patients and could cause obvious decrease of the serum level of sIL-2R. The serum level of sIL-2R can be used as an important index of activity of AS and as a guide of therapy. 展开更多

Methods: Forty-four patients with SLE were randomly divided into conventional corticosteroid treated group (control group, n =20) and conventional treatment group with leeching intervention added (leeching group, n =24). Before and after treatment the concentration of plasma endothelin (ET) and soluble interleukin-2 receptor (sIL-2R) were determined. Results: Before treatment the level of plasma ET and sIL-2R in the SLE patients were all higher than those in the normal healthy group, ( P <0.01). But after treatment the level of these in both groups were significantly improved than those of before treatment ( P <0.05), and comparison between these two treated groups showed that the difference between them was significant ( P <0.05). Conclusion: Leeching added to conventional treatment of SLE could be more effective in improving the level of plasma ET and sIL-2R, and ameliorating the impairment of renal tissues. 展开更多

Interleukin-2 and its receptor are of importance in regulating immunity responses. The changes of interleukin-2 (IL-2) and soluble interleukin-2 receptor (IL-2R) during heart valve (s) replacement operation and effects of aprotinin on them were observed. Twenty patients undergoing heart valve (s) replacement were randomly divided into two groups: control group (n=10) and apro- tinin group (n=10). In aprotinin group, 1 000 000 KIU aprotinin was given by vein injection and then 2 000 000 KIU was given as a bolus in prime. Blood samples were collected before CPB, right after CPB and on the 1st, 3rd and 7th postoperative day (POD) for serum IL-2 and sIL-2R determination. Results showed that after CPB, IL-2 was reduced and slL-2R increased. Meanwhile, serum IL-2R was lower in aprotinin group than that of control. It is concluded that the immunity depression after CPB is associated with low level of IL-2 and high level of sIL-2R and aprotinin can ameliorate the situation. 展开更多

Background The present study aimed to investigate the detailed mode and specific sites for their binding as well as the functional relevance of this binding in the phenotypic proliferation of vascular smooth muscle cells(SMCs). Methods CREG knocked-down SMCs were employed to evaluate the biological activity of wtCREG and mCREG.Expressions of SMC differentiation markers SM myosin heavy chain(SM-MHC),SM-actin,heavy caldesmon and myocardin were determined by Western blotting using specific antibodies. Cellular growth of SMCs was assessed by bromide dewuridine (BrdU) incorporation and cell cycle analysis on fluorescence-activated cell sorting(FACS).A solid-phase binding assay was used to study the binding of CREG to extracellular domains of M6P/IGF2R.The cellular co-localization of the two recombinant CREGs with M6P/IGF2R was detected on SMC surface by immunoprecipitation and immunofluorescence analysis.Results The molecular weight of wtCREG was around 30 kD while that of the mCREG was~25 kD.Treatment of wtCREG with PNGase F reduced its molecular weight from~30 kD to~25 kD,whereas PNGase F treatment had no effect on the molecular weight of mCREG.Both wtCREG and mCREG proteins enhanced SMC differentiation,inhibited BrdU incorporation,and arrested cell cycle progression when added to the culture medium.In CREG knocked-down SMCs,the amount of CREG detected by immunoblotting in M6P/IGF2R immunoprecipitates was significantly reduced when compared to normal cells.Both recombinant CREGs co-immunoprecipitated with M6P/IGF2R, although slightly reduced amount of the mutant CREG was detected in M6P/IGF2R immunoprecipitates.Immunostaining revealed that His-tagged CREGs co-localized with IGF2R on the cell surface in a glycosylation-independent manner.In vitro binding assay showed that CREGs bound to M6P/ IGF2R extracellular domains 7-10 and 11-13 in a glycosylation -dependent and -independent manner,respectively.Further blocking experiments using soluble M6P/IGF2R fragments and M6P/IGF2R neutralizing antibody indicated that the biological activities of recombinant CREGs in SMC growth and the up-regulation of SMC differentiation markers were all abolished by treatment with the M6P/IGF2R neutralizing antibody. However,although the growth inhibitory effect of wtCREG was nearly abolished by D7-10 or D11-13,the effect of mCREG was only reversed by Dll-13,indicating that the binding to domains 11-13 is required for CREG to modulate the proliferation of SMCs.Conclusions These data suggest that solubleCREG proteins can exert their biological function via binding to the extracellular domains 7-10 and 11-13 of cell surface M6P/IGF2R in both a glycosylation-dependent and -independent manner. 展开更多

Fulminant type 1 diabetes is classified as type 1B diabetes, and characterized by the absence of anti-GADab, flu-like symptoms, and elevated serum exocrine pancreatic enzymes. We report a type 1 diabetic patient who showed flu-like symptoms, elevated serum exocrine pancreatic enzymes, and an extremely high-titer of anti-GADab, manifesting the characteristics of both type 1A and fulminant type 1 diabetes. 展开更多

血清MBL、IL-1R1、Nrf2与肺炎支原体感染诱发哮喘的患儿肺功能和预后的关系

目的探讨肺炎支原体(mycoplasma pneumoniae,MP)感染诱发哮喘患儿血清甘露糖结合凝集素(mannose-binding lectin,MBL)、白细胞介素-1受体1型(interleukin-1 receptor type 1,IL-1R1)、核因子E2相关因子2(nuclear factor E2-related fact... 目的探讨肺炎支原体(mycoplasma pneumoniae,MP)感染诱发哮喘患儿血清甘露糖结合凝集素(mannose-binding lectin,MBL)、白细胞介素-1受体1型(interleukin-1 receptor type 1,IL-1R1)、核因子E2相关因子2(nuclear factor E2-related factor 2,Nrf2)与肺功能的相关性,并分析血清MBL、IL-1R1、Nrf2与患儿预后的关系。方法选取2020年2月至2023年2月石家庄医学高等专科学校附属医院收治的MP感染诱发哮喘患儿126例(哮喘组),另选取同期体检健康儿童(健康组)及MP感染的肺炎支原体肺炎(mycoplasma pneumoniae pneumonia,MPP)患儿(MP感染组)各126例分别作为对照。检测血清MBL、IL-1R1、Nrf2水平及肺功能[用力肺活量(forced vital capacity,FVC)、第1秒用力呼气容积(forced expiratory volume in the first second,FEV_(1))、FEV_(1)/FVC及最大呼气峰流量(peak expiratory flow,PEF)占预计值的百分比(PEF%pred)]。采用Pearson相关性分析法分析血清MBL、IL-1R1、Nrf2水平与肺功能的相关性。采用多因素logistic回归分析MP感染诱发哮喘患儿预后的影响因素,采用受试者工作特征(receiver operating characteristic,ROC)曲线分析血清MBL、IL-1R1、Nrf2对预后不良的预测价值。结果哮喘组血清MBL、IL-1R1水平高于MP感染组和健康组,血清Nrf2水平、FEV_(1)、FVC、FEV_(1)/FVC及PEF%pred低于MP感染组和健康组。血清Nrf2水平与FEV_(1)、FVC、FEV_(1)/FVC及PEF%pred呈正相关(r=0.484、0.521、0.569、0.481,P<0.001),血清MBL、IL-1R1与FEV_(1)、FVC、FEV_(1)/FVC及PEF%pred呈负相关(r=-0.531、-0.533,-0.611、-0.545,-0.549、-0.564,-0.574、-0.597,P<0.05)。126例MP感染诱发哮喘患儿预后不良率为37.30%,logistic回归分析结果显示:过敏史、血清MBL、IL-1R1高水平及血清Nrf2低水平是MP感染诱发哮喘患儿预后不良的独立危险因素(P<0.05)。血清MBL、IL-1R1、Nrf2联合预测MP感染诱发哮喘患儿预后不良的ROC曲线下面积(AUC)为0.880(95%CI:0.810~0.934),高于各项单独预测的0.710、0.722、0.743。结论MP感染诱发哮喘患儿血清MBL、IL-1R1水平升高,Nrf2水平降低,与肺功能及预后存在明显的相关性,血清MBL、IL-1R1、Nrf2联合检测对于预测MP感染诱发哮喘患儿预后具有较高的参考价值。 展开更多

BACKGROUND: Studies have shown that estrogen receptor alpha (ERα), nerve growth factor (NGF), interleukin-2 (IL-2), and androgen receptor (AR) expression in the cerebellum decreases when estrogen levels decrease in vivo. Soybean isoflavone, a type of non-steroid estrogen with similar molecular structure and function to estradiol, exhibits estrogen-like characteristics. OBJECTIVE: To investigate the effects of various doses of soybean isoflavone on expression of ERa, NGF, IL-2, and AR in the cerebellum of ovariectomized rat, and to determine whether there is a dose-dependent effect.DESIGN, TIME AND SETTING: Controlled trial at the cellular and molecular level. The study was performed at the Experimental Animal Engineering Center, College of Veterinary Medicine, Sichuan Agricultural University from July 2006 to May 2008. MATERIALS: Soybean isoflavone, comprised of daidzin, genistein and isoflavone, was provided by Taiyuan Yuantai Biochemical Industry, China. The ERα, NGF, IL-2, and AR in situ hybridization kit, rabbit anti-rat ERa, NGF, IL-2, and AR monoclonal antibodies, and SABC kit were purchased from Wuhan Boster Biological Technology, China. METHODS: A total of 50 female, Sprague Dawley rats, aged 3 months, were randomly assigned to 5 groups, with 10 animals in each group. With the exception of the sham-operation group (abdominal cavity opening alone), all rats underwent bilateral ovariectomy. At 14 days after surgery, rats in the high-, middle-, and low-dose soybean isoflavone groups were subcutaneously injected with 1.5, 1.0, and 0.5 mg/kg soybean isoflavone, respectively, every 2 days for 6 consecutive weeks. Rats in the sham-operation and ovariectomized groups were subcutaneously injected with absolute alcohol (0.5 mL/kg). MAIN OUTCOME MEASURES: Expression levels and distribution of ERα, NGF, IL-2, and AR in the cerebellum were detected by immunohistochemistry and in situ hybridization. RESULTS: Compared with the sham-operation group, immunoreactive products and hybridization signals of ERa, NGF, IL-2, and AR were significantly decreased in the cerebellar cortex and nuclei of ovariectomized rats (P 〈 0.05 or P 〈 0.01), but increased following soybean isoflavone treatment. In particular, levels of the high-dose soybean isoflavone group were almost restored to levels of the sham-operation group (P 〉 0.05). The immunoreactive products were primarily located in the cytoplasm and neurites, and rarely in the cell membrane and nuclei. However, the hybridization signals were predominantly located in the nuclei, but rarely in the cytoplasm, cell membrane, or neurites. CONCLUSION: Soybean isoflavone upregulated ERα, NGF, IL-2, and AR protein and gene expression in a dose-dependent manner, and played an important role in sustaining and protecting structure and function of cerebellar neurons. Moreover, the similarity of expression patterns of these molecules indicated that they were mutually interactive during the regulation of soybean isoflavone to the cerebellum. 展开更多

The aim of this stu... Background: Cytokines are mediators of diseases. Expression levels in the blood could be of clinical relevance. Objective: sIL-2Rα is used as a marker for different malignancies in human medicine. The aim of this study was to show if sIL-2Rα is detectable and if there is any correlation to different diseases in dogs. Methods: For this purposes sIL-2Rα concentrations in the blood were measured in healthy dogs, in dogs with different non-neoplastic diseases and benign tumors and in dogs with malignant tumors. Serum levels of sIL-2Rα were measured by using a human specific enzyme linked immunosorbent assay (ELISA). Results: Measurement of sIL-2Rα was successful in most of the samples. Dogs with diseases have significantly increased serum levels of sIL-2Rα compared to healthy controls. sIL-2Rα serum levels are higher in patients with non-neoplastic diseases and benign tumors than in those with malignant neoplasia. There is a strong correlation between sIL-2Rα and leukocyte count. Conclusion: Measurements of sIL-2Rα in serum may be helpful in detecting stages and grades of inflammation in the progression of disease. sIL-2Rα could actually not be used as an indicator for malignant diseases in dogs like in humans. The strong correlation between sIL-2Rα and the leukocyte count indicates the inflammatory response to the disease. This could be helpful in giving a prognosis in some cases, because the inflammatory reaction is of prognostic relevance in different diseases including malignant and non-malignant neoplasia. Although the results of our research studies were very promising, further studies should be performed with a canine ELISA. 展开更多

Plasma levels of soluble interleukin-2 receptor (sIL-2R) in patients with chronic active hepatitis B (CAHB) or severe hepatitis B (SHB) were measured quantitatively by 'sandwich' ELISA with monoclonal antibodies in order to explore the change of sIL-2R levels, its clinical significance,and its relation to liver damage. The results showed that the plasma sIL-2R levels in patients with CAHB and SHB were much higher than those in normal controls (P < 0. 01 ), and the level ofplasma sIL-2R in patients with SHB was greatly higher than that in patients with CAHB. These results suggest that there is close relation between plasma level of sIL-2R, the clinical types of hepatitis B,and the severity of liver damage. In addition, there is no significant difference in plasma levels of sIL-2R between acute severe hepatitis B (ASHB), subacute severe hepatitis B (SASHB), and chronic severe hepatitis B (CSHB). 目的:探讨高频超声弹性成像参数联合血清抗甲状腺球蛋白抗体(TgAb)、可溶性白介素-2受体(sIL-2R)水平检测对甲状腺结节性质的鉴别诊断价值。方法:回顾性分析2021年6月至2023年6月我院收治的83例甲状腺结节患者的临床资料,均行高频超声... 目的:探讨高频超声弹性成像参数联合血清抗甲状腺球蛋白抗体(TgAb)、可溶性白介素-2受体(sIL-2R)水平检测对甲状腺结节性质的鉴别诊断价值。方法:回顾性分析2021年6月至2023年6月我院收治的83例甲状腺结节患者的临床资料,均行高频超声弹性成像检查,根据细针穿刺或术后病理结果,将患者分别纳入良性组(52例)与恶性组(31例)。比较2组影像学特征、高频超声弹性成像参数(弹性应变率、弹性评分)及血清TgAb、sIL-2R水平,分析联合应用对甲状腺恶性结节的诊断价值。结果:恶性组回声不均匀、形态不规则、伴穿支血管、边界不清晰及砂砾样钙化占比高于良性组(P<0.01);恶性组高频超声弹性成像参数弹性应变率、弹性评分及血清TgAb、sIL-2R水平均高于良性组(P<0.01);高频超声弹性成像参数弹性应变率、弹性评分及血清TgAb、sIL-2R联合诊断甲状腺恶性结节的AUC为0.931,敏感度为90.32%,特异度为82.69%,约登指数为0.730,优于各参数及血清指标单独诊断价值(P<0.05)。结论:高频超声弹性成像参数联合血清TgAb、sIL-2R水平对甲状腺结节性质具有一定鉴别诊断价值,可作为临床鉴别诊断结节性质的有效检查方案之一。 展开更多

目的 研究游离股前外侧皮瓣(ALTFF)方案治疗对口腔颌面软组织缺损患者的修复效果及对血清唾液酸、可溶性白细胞介素-2受体(sIL-2R)水平的影响。方法 回顾性选取2020年1月至2023年1月入西南医科大学附属简阳医院(简阳市人民医院)的90例... 目的 研究游离股前外侧皮瓣(ALTFF)方案治疗对口腔颌面软组织缺损患者的修复效果及对血清唾液酸、可溶性白细胞介素-2受体(sIL-2R)水平的影响。方法 回顾性选取2020年1月至2023年1月入西南医科大学附属简阳医院(简阳市人民医院)的90例口腔颌面部软组织缺损患者,根据治疗方式不同分为对照组(n=45)与研究组(n=45)。对照组予以游离上臂外侧皮瓣(LAFF)方案治疗,研究组予以ALTFF方案治疗。比较两组患者的皮瓣面积、皮瓣制备时间等围术期指标,血清唾液酸、sIL-2R水平,术后美观度,口腔功能恢复情况和术后并发症发生情况。结果 研究组患者的皮瓣面积、血管蒂长度、动脉管径、静脉管径、皮瓣成功率分别为(65.53±10.15) cm^(2)、(10.15±0.77) cm、(2.03±0.11) mm、(3.21±0.31) mm、100.00%,均高于对照组[(42.12±10.33) cm^(2)、(5.89±0.83) cm、(1.46±0.15) mm、(2.01±0.14) mm、80.00%],差异均有统计学意义(P<0.05);两组患者的皮瓣制备时间、皮瓣塑形率比较,差异均无统计学意义(P>0.05)。术后30 d,两组患者的血清唾液酸、sIL-2R水平均低于术前,且研究组患者术后30 d血清唾液酸、sIL-2R水平分别为(0.56±0.11) mg/L、(1.86±0.52)μg/L,均低于对照组[(0.89±0.21) mg/L、(2.38±0.83)μg/L],差异均有统计学意义(P<0.05)。研究组患者随访6个月术后美观度各项评分均低于对照组,口腔功能恢复情况优于对照组,差异均有统计学意义(P<0.05)。研究组患者的并发症发生率为11.11%,低于对照组(40.00%),差异有统计学意义(P<0.05)。结论 口腔颌面部软组织缺损修复中ALTFF与LAFF方案的皮瓣塑形率相当,ALTFF在皮瓣面积、成功率、美观度、口腔功能上优势更加显著,有利于改善血清指标,降低并发症发生风险。 展开更多
