This editorial discusses a case report recently published in the World Journal of Clinical Cases.The report describes the clinical presentation,imaging,diagnosis,and treatment of a patient with tuberous sclerosis complex(TSC)combined with primary lymphedema(PLE).Additionally,it retrospectively analyzes the data of 16 previously reported cases of children with TSC combined with PLE to summarize the epidemiology,genetic diagnosis,and current main treatments of these patients.The report also speculates on the pathological and physiological mechanisms underlying TSC combined with PLE.TSC combined with PLE is rare;therefore,the report provides a theoretical basis for understanding the pathophysiological mechanisms and treatment options for patients with TSC and PLE.Comprehensive clinical management of TSC is essential due to the diverse and multiorgan nature of its manifestations,often requiring a multidisciplinary approach for newly diagnosed cases. 展开更多

Enhancing outcomes in severe lymphedema through combined treatment strategies

Lymphedema,particularly in its advanced stages,presents significant challenges in treatment,often necessitating a combination of therapies to manage symptoms effectively and improve patient outcomes.This article reviews the findings of Wang et al,regarding the use of lymphovenous anastomosis and complex decongestive therapy in treating severe,deformed stage III lymphedema with recurrent infections.The case report details the promising results achieved through this combined therapy,highlighting substantial reductions in limb volume and the complete resolution of recurrent lymphangitis.The patient experienced notable improvements in weight loss,physical function,and quality of life.Despite its strengths,the study has several limitations.It lacks specific details on the types of lymphovenous anastomoses performed and complex decongestive therapy protocols,such as frequency and adherence,making reproducibility difficult.The short follow-up period of six months limits understanding of long-term efficacy,and more consistent reporting of key metrics such as weight loss and body mass index would enhance outcome assessments.This article emphasizes the importance of integrating minimally invasive surgical techniques with conservative therapies to address both the symptoms and underlying causes of lymphedema.Further research is essential to standardize protocols and refine combined treatment strategies. 展开更多

He underwent a surgical treatment with removal of the mass. Pathology of the specimen was suggestive of either a non specific fibrosis or a benign vascular tumor. Conclusion: Giant scrotum is rare. BACKGROUND Tuberous sclerosis complex(TSC)and primary lymphedema(PLE)are both rare diseases,and it is even rarer for both to occur in the same patient.In this work,we have provided a detailed description of a patient's clinical presentation,imaging findings,and treatment.And a retrospective analysis was conducted on 14 published relevant case reports.CASE SUMMARY A 16-year-old male came to our hospital for treatment due to right lower limb swelling.This swelling is already present from birth.The patient’s memory had been progressively declining.Seizures had occurred 1 year prior at an unknown frequency.The patient was diagnosed with TSC combined with PLE through multimodal imaging examination:Computed tomography,magnetic resonance imaging,and lymphoscintigraphy.The patient underwent liposuction.The swelling of the patient's right lower limb significantly improved after surgery.Epilepsy did not occur.after taking antiepileptic drugs and sirolimus.CONCLUSION TSC with PLE is a rare and systemic disease.Imaging can detect lesions of this disease,which are important for diagnosis and treatment. 展开更多

BACKGROUND Lower extremity lymphedema is a common complication following treatment for gynecological malignancies.Its incidence rate can reach up to 70%,affecting~20 million people worldwide.However,specialized treatment centers are scarce,and there is a lack of consensus on treatment approaches.Furthermore,there are even fewer reports on the systematic and effective treatment of severe lymphedema with malformations.Effective management of this condition remains a significant challenge for clinicians.CASE SUMMARY A 40-year-old woman developed bilateral leg swelling 6 years after receiving treatment for endometrial cancer.Since August 2018,she experienced>30 episodes of lymphangitis.Upon presentation,she exhibited bilateral leg swelling and deformation,with four large swellings in the posterior thigh that impeded movement,and pain in the limbs.Skin manifestations included lichenoid lesions and features of deep sclerosis.Radionuclide lymphoscintigraphy confirmed the diagnosis of lower limb lymphedema.After 6 mo of complex decongestive therapy(CDT)and three lymphaticovenous anastomosis(LVA)treatments,the patient lost 49 kg in weight.She also experienced a maximum circumference reduction of 35.2 cm in the left lower limb and 37.5 cm in the right lower limb.The leg pain disappeared,her swelling significantly decreased,and she regained the ability to walk,cycle,and run normally.CONCLUSION The combined application of CDT and LVA therapy demonstrates significant positive effects in the treatment of severe,deformed stage III lymphedema. 展开更多

Objective To evaluate the therapeutic efficacy of complex decongestive therapy in the treatment of elephantiasis of the lower extremities.Methods Seventeen patients with unilateral lower limb elephantiasis were included in the study(5 with primary lymphedema and 12 with secondary lymphedema).All patients were treated with one course(4 weeks)of complex decongestive therapy.Changes inextracellular fluid(ECF),BMI,circumferences,and skin properties including skin stiffness(SF),percent water content(PWC),and trans-epidermal water loss(TEWL)were measured before and after treatment.Results The“elephantiasis-like”appearance of the affected limb was greatly improved after treatment.The ECF,BMI,and circumferences were significantly decreased after treatment(P<0.001)with an average ECF of 2.97±1.82 L,BMI of 1.638±2.647 kg/m^2,and circumference of 6.58±2.79 cm.The values of PWC,TEWL,and SFwere 50.5%±10.6%,18.55±10.2 g/m^2 h,and 0.161±0.176 N,respectively,before treatment and 36.05%±7%,8.3±2.07 g/m^2h,and 0.086±0.038 N,respectively,after treatment,there by showing significant decreases(P<0.05).Conclusion Complex decongestive therapy is effective in the treatment of advanced stages of chronic lymphedema. 展开更多

It is a chronic and complex disease. Once lymphedema occurs, there will be discomfort such as limb swelling, pain, numbness and tension, which will eventually lead to changes in the appearance of the affected limb and will seriously affect the quality of life and require lifelong treatment and psychosocial support. This study investigated the current situation of stigma and alexithymia in patients with lymphedema, and discussed the impact of alexithymia on stigma in patients with lymphedema. AIMS: To understand the current situation of stigma and alexithymia in patients with lymphedema, and to analyze the influence of alexithymia on stigma. METHODS: 195 patients with lymphedema in a hospital were selected by convenient sampling. General information questionnaire, Toronto Alexithymia Scale and social impact scale were used to investigate respectively, to study the general situation, stigma and alexithymia of the respondents. RESULTS: The results showed that the total score of stigma in 195 patients with lymphedema was (60.36 ± 11.08), and the total score of alexithymia was (56.53 ± 8.43). Multiple linear regression analysis showed that alexithymia and family relationship were the influencing factors of stigma in patients with lymphedema. CONCLUSIONS: The patients with lymphedema have obvious stigma, and alexithymia and family relationship are the influencing factors. 展开更多

Vascularized lymph node transfer using axilla as recipient site restores lymphatic flow in upper limb lymphedema:Evidence from magnetic resonance lymphangiography

Background:Lymphedema is a debilitating condition that frequently occurs after breast cancer treatment.Vas-cularized lymph node transfer(VLNT)is a promising approach to reduce lymphedema.This study used magnetic resonance lymphangiography(MRL)to assess lymphatic reconnections post-VLNT in patients with breast cancer-related lymphedema(BCRL).Methods:The clinical records of six female patients with unilateral upper limb BCRL who underwent VLNT(4 cases)or VLNT combined with breast reconstruction(2 cases)were retrospectively reviewed.All patients were examined using MRL preoperatively and at the 1-year follow-up.The morphological characteristics of the lymphatic network,dermal backflow patterns,and architecture of the lymph nodes were evaluated.Clinical outcomes,patient satisfaction,and complications were assessed.Results:At the 1-year follow-up,reduction in tissue edema and limb circumference was achieved in all six patients.In MRL,the implanted lymph nodes in the axillary region of the affected upper arm were enhanced and visualized in all six patients.Reconnected lymphatic vessels in the subcutaneous tissue associated with the implanted lymph nodes were observed in four patients.Decreased dermal backflow and lymphatic vessel dilation of the affected limbs were observed in all six patients.No disruption of the lymph flow in the donor area was detected.Conclusion:This is the first study to provide direct imaging evidence for the reconnection of afferent lymphatic channels between implanted lymph nodes and the recipient lymphatic system in patients with BCRL.Overall,our study demonstrates the mechanism and efficacy of VLNT in reducing lymphedema. 展开更多

Objective: To explore the pre-treatment and efficacy analysis of comprehensive anti-inflammatory treatment for lymphedema in patients with irritating contact dermatitis. Method: Convenience sampling method was used to observe the skin of 160 patients with upper limb lymphedema admitted to the lymphedema outpatient department of our hospital. They were divided into an observation group (80 cases) and a control group (80 cases), and both groups received a course of comprehensive anti-inflammatory treatment (20 treatments). The control group received routine skin care;On the basis of the control group, the observation group received pre-treatment of the affected limb skin: Laofuzi herbal ointment was applied externally to the prone areas of irritating contact dermatitis (such as the upper arm, inner forearm, and cubital fossa). Result: The incidence of irritating contact dermatitis in the observation group was significantly lower than that in the control group (P 0.05). Patients in the observation group felt significantly better in terms of comfort, skin moisture, and itching relief after being wrapped with low elasticity bandages than those in the control group (P Conclusion: Preventive treatment can effectively reduce the incidence of irritating contact dermatitis, prolong the time of stress treatment, thereby increasing efficacy and improving patient compliance. 展开更多

A novel mutation of vascular endothelial growth factor receptor gene (VEGFR-3), was identified in a four-generation Chinese family with hereditary lymphedema type I (HL-I). Genetic linkage analysis was performed on the known genetic locus for HL-I with a panel of polymorphic markers, and then mutations were screened out by direct sequencing. By genotyping, the family showed the linkage to HL-I locus on 5q35.3. Mutation screening analysis of the exons encoding the intracellular kinase domains of VEGFR-3, revealed a novel missense mutation D1055V. This mutation cosegregated with the disease phenotype in the family and was not found in 100 normal controls. This finding has expanded the spectrum of the VEGFR-3 gene mutations causing HL-I, and will be useful for further genetic consultation and genetic diagnosis. 展开更多

目的分析宫颈癌术后放疗后下肢淋巴水肿的危险因素及行桂枝茯苓丸治疗的效果。方法前瞻性选取2022年1月至2024年1月延安大学附属医院收治的140例宫颈癌患者,所有患者均接受宫颈癌根治手术及术后放疗;根据术后放疗后6个月是否发生下肢淋... 目的分析宫颈癌术后放疗后下肢淋巴水肿的危险因素及行桂枝茯苓丸治疗的效果。方法前瞻性选取2022年1月至2024年1月延安大学附属医院收治的140例宫颈癌患者,所有患者均接受宫颈癌根治手术及术后放疗;根据术后放疗后6个月是否发生下肢淋巴水肿,分为水肿组(n=62)和非水肿组(n=78);使用单因素分析和多因素Logistic回归分析对宫颈癌术后放疗后下肢淋巴水肿的危险因素进行分析。依据随机数字表法将下肢淋巴水肿患者分为对照组和观察组,每组各31例。对照组进行常规治疗,观察组在此基础上加用桂枝茯苓丸治疗。比较两组总有效率,治疗前、治疗20 d后的小腿周径、大腿周径、妇科癌症淋巴水肿问卷(GCLQ)评分、功能独立性评定量表(FIM)评分及下肢功能量表(LEFS)评分及药物不良反应发生情况。结果单因素分析结果显示,水肿组的年龄、淋巴结清扫数目、放疗持续时间、每日站立时间、规律运动锻炼比例与非水肿组比较,差异均有统计学意义(P<0.05)。经多因素Logistic回归分析,高龄、淋巴结清扫数目较多、放疗持续时间较长和每日站立时间较长均为独立危险因素(P<0.05),规律运动锻炼为保护因素(P<0.05)。观察组总有效率为96.77%,高于对照组(70.97%),差异有统计学意义(P<0.05)。治疗20 d后,观察组小腿周径、大腿周径分别为(30.22±2.51)、(33.01±2.78)cm,均小于对照组[(35.16±4.15)、(38.15±3.97)cm],差异均有统计学意义(P<0.05)。治疗20 d后,观察组GCLQ评分为(2.56±0.52)分,低于对照组[(4.65±0.83)分],观察组FIM评分、LEFS评分分别为(101.23±14.46)、(70.42±10.49)分,均高于对照组[(91.74±12.07)、(60.55±8.36)分],差异均有统计学意义(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论宫颈癌术后放疗后下肢淋巴水肿的独立危险因素是高龄、淋巴结清扫数目较多、放疗持续时间较长和每日站立时间较长,保护因素是规律运动锻炼。通过桂枝茯苓丸治疗宫颈癌术后放疗后下肢淋巴水肿的效果显著,有助于促进下肢功能恢复。 展开更多

目的基于数据挖掘和网络药理学分析曹建春教授治疗淋巴水肿的用药规律与作用机制。方法收集2019年1月至2023年7月经北京中医药大学东方医院周围血管科曹建春教授治疗淋巴水肿的病案处方,对处方进行药物应用频次、关联规则及聚类分析,结... 目的基于数据挖掘和网络药理学分析曹建春教授治疗淋巴水肿的用药规律与作用机制。方法收集2019年1月至2023年7月经北京中医药大学东方医院周围血管科曹建春教授治疗淋巴水肿的病案处方,对处方进行药物应用频次、关联规则及聚类分析,结合面对面专家访谈,获得核心药组。采用网络药理学方法预测核心药组的有效成分、关键靶点和通路等作用机制,使用分子对接技术对关键有效成分与靶点进行对接验证。结果共纳入183首处方,涉及中药261味。使用频次≥50的高频药物有16味,关联规则分析得到12种关联规则性强的药物组合,聚类分析得到5种药物组合,进一步专家访谈,得到核心药组为:当归、川芎、皂角刺、黄芪、桂枝、泽兰、泽泻、鸡血藤。网络药理学分析核心药组调控淋巴水肿的有效成分为槲皮素、常春藤皂苷元、山柰酚、β-谷甾醇、异黄酮等,关键靶点为Akt1、MTOR、EGFR、SRC、BCL2、IL-6、PIK3CA、MAPK1、PIK3R1、MAPK3,关键生物学效应和通路包括EGFR-TKIs抵抗、内分泌抵抗、HIF-1信号通路、ErbB信号通路等。分子对接验证关键有效成分与靶点具有良好的亲和力。结论曹建春教授治疗淋巴水肿以活血通络、利水消肿为主,核心药物通过多成分、多靶点、多通路发挥作用,可为该病的临床治疗和基础研究提供参考。 展开更多

下肢淋巴水肿是妇科恶性肿瘤手术后常见并发症之一,患者轻则出现感觉异常,重则演变为功能障碍,本病具有迁延不愈及进行性加重的特点,对患者身心可造成严重不良影响,西医治疗总以改善症状为主,短期内可减轻水肿,然而长期疗效欠佳。玄府... 下肢淋巴水肿是妇科恶性肿瘤手术后常见并发症之一,患者轻则出现感觉异常,重则演变为功能障碍,本病具有迁延不愈及进行性加重的特点,对患者身心可造成严重不良影响,西医治疗总以改善症状为主,短期内可减轻水肿,然而长期疗效欠佳。玄府理论萌发于《黄帝内经》,经后世医家不断阐释发挥,内涵日益丰富。玄府结构与西医淋巴循环系统相似,玄府理论对人体气液运行的病理认识与炎症反应、脂肪增生、纤维化等本病现代医学发病机制有相通之处,因此从玄府角度认识妇科恶性肿瘤术后下肢淋巴水肿客观合理且具科学性。基于玄府理论,可将下肢淋巴水肿病变过程概括为玄府郁闭,气液失通,郁而化热,终致痰瘀胶结,玄府空虚,虚实夹杂,水肿顽固难消。治疗上当顺玄府之性,立开玄之法,条畅气液流行,除湿浊痰瘀,以消水肿之患。按不同病程阶段,具体治法可分为理气利水开玄、清透火热开玄、祛痰活血开玄、扶正补虚开玄。风药辛散宣通之特性与本病玄府郁闭之基本病机尤为契合,故本病治疗应善伍以风药,根据病证灵活选择药物,常有增效的作用。文章附临床验案一则,为临床论治本病提供新思路。 展开更多

目的:探讨术前淋巴显像在预测妇科恶性肿瘤术后下肢淋巴水肿患者进行联合手术治疗效果中的作用。方法:回顾性分析2021年9月—2023年3月妇科肿瘤术后下肢淋巴水肿患者行联合手术治疗175例。收集人口学信息和相关临床参数。测量术前、术后... 目的:探讨术前淋巴显像在预测妇科恶性肿瘤术后下肢淋巴水肿患者进行联合手术治疗效果中的作用。方法:回顾性分析2021年9月—2023年3月妇科肿瘤术后下肢淋巴水肿患者行联合手术治疗175例。收集人口学信息和相关临床参数。测量术前、术后3月和6月下肢淋巴水肿指数(LELi),使用淋巴显像台湾分期系统进行评级,分析术前淋巴显像分期与下肢淋巴水肿改善的相关性。采用残疾与健康分类(LY-ICF-LL)和生活质量问卷(SF-36)评分系统评价所有患者手术后功能障碍和生活质量。最后收集患者并发症情况,分析淋巴显像分期与术后并发症关联。结果:患者术后3月和6月LELi值较术前明显下降(P<0.001),且P1~P3分期患者的术后LELi改善率(LC)变化率较为显著(P<0.05)。术后ICF评分下降,生活质量明显提高(P<0.05)。分期T4~T6的患者术后并发症发生率显著高于P1~P3的患者(P<0.05)。结论:淋巴显像可作为预测妇科恶性肿瘤术后下肢淋巴水肿进行联合手术疗效和风险的有效工具,为手术决策提供重要参考。未来需深入研究不同显像剂淋巴显像在指导妇科恶性肿瘤引起的淋巴水肿患者治疗中的差异。 展开更多
