Macular edema after cataract surgery in diabetic eyes evaluated by optical coherence tomography 被引量:11

AIM:To assess quantitative changes of the macula in diabetic eyes after cataract surgery using optical coherence tomography(OCT)and to estimate the incidence of development or worsening of macular edema(ME)in diabetic eyes with or without pre-existing ME.·METHODS:In this prospective,observational study,92eyes of 60 diabetic patients who underwent cataract surgery were evaluated before surgery and 1,3mo after surgery using OCT.Macular thickness was measured with OCT at nine macular subfields defined by the 9zones early treatment of diabetic retinopathy study(ETDRS),as well as total macular volume obtained by OCT at 1,3mo after surgery were compared with baseline features obtained before surgery.In addition,the incidence of development or worsening of ME was analyzed in diabetic eyes with or without pre-existing ME.·R ESULTS:The central subfield mean thickness increased 21.0μm and 25.5μm at 1,3mo follow-up,respectively(〈0.01).The average thickness of inner ring and outer ring increased 14.2μm and 9.5μm at 1mo,18.2μm and 12.9μm at 3mo.Central-involved ME developed in 12 eyes at 3mo,including 4 eyes with preexisting central-involved and 8 eyes with pre-existing non-central involved ME.Pre-existing diabetic macular edema(DME)was significantly associated with centralinvolved ME development(〈0.001).·C ONCLUSION:A statistically significant increase could be detected in the central subfield as well as perifoveal and parafoveal sectors though the increase was mild.And eyes with pre-operative DME prior to cataract surgery are at higher risk for developing central-involved ME. 展开更多

Effect of early warning and evidence-based intervention combined with drug therapy on the infection process in children with mycoplasma pneumonia

Objective:To study the effect of early warning and evidence-based intervention combined with drug therapy on the infection process in children with mycoplasma pneumonia.Methods:Children who were treated and clearly diagnosed with mycoplasma pneumonia in Zigong Third People's Hospital between May 2014 and October 2017 were chosen and randomly divided into two groups, experimental group accepted early warning and evidence-based intervention combined with drug therapy, and control group accepted routine drug therapy. The levels of inflammatory cytokines and oxidative stress response indicators in serum as well as the expression of inflammation and oxidative stress signal molecules in peripheral blood were measured before treatment and 3 d after treatment.Results:Compared with those of same group before treatment, serum TNF-α, CysLTs, sTREM1, sP-selectin, sICAM1, MDA, SF and COR levels as well as peripheral blood TLR4, NF-κB, COX2, NOX4, MPO and iNOS expression intensity of both groups of patients significantly decreased whereas SOD and IgA levels significantly increased after treatment. Moreover, serum TNF-α, CysLTs, sTREM1, sP-selectin, sICAM1, MDA, SF and COR levels as well as peripheral blood TLR4, NF-κB, COX2, NOX4, MPO and iNOS expression intensity of experimental group were lower than those of control group whereas SOD and IgA levels were higher than those of control group. Conclusion: Early warning and evidence-based intervention combined with drug therapy can reduce the inflammatory response and oxidative stress response in the infection process of children with mycoplasma pneumonia. 展开更多

Value of serum PCT content for evaluating inflammatory factor release and organ function injury in neonatal septicemia

Objective: To explore the value of serum PCT content for evaluating inflammatory factor release and organ function injury in neonatal septicemia. Methods: 48 children who were diagnosed with neonatal septicemia in our hospital between March 2015 and May 2018 were selected as the septicemia group, and 50 healthy neonates delivered in our hospital during the same period were selected as the normal control group. The differences in PCT and inflammatory factor contents in serum as well as liver and kidney function index levels in peripheral blood were compared between the two groups. Pearson test was used to evaluate the correlation of serum PCT content with inflammatory factor release as well as liver and kidney function injury in children with neonatal septicemia. Results: The serum PCT content of septicemia group was significantly higher than that of normal control group;serum hypersensitive C-reactive protein (hs-CRP), interleukin-6 (IL-6), interleukin-8 (IL-8) and soluble intercellular adhesion molecule-1 (sICAM-1) contents were higher than those of normal control group;peripheral blood aspartate aminotransferase (AST), alanine aminotransferase (ALT), γ-glutamyl transpeptidase (γ-GGT) and alkaline phosphatase (ALP) levels were higher than those of normal control group;peripheral blood blood urea nitrogen (BUN), serum creatinine (Scr) and serum β2-microglobulin (β2-M) levels were higher than those of normal control group (P<0.05). Correlation analysis showed that serum PCT content in children with neonatal septicemia was directly correlated with the degree of inflammatory response as well as the degree of liver and kidney function injury (P<0.05). Conclusion: The serum PCT content abnormally increases in children with neonatal septicemia, and the specific content is consistent with the disease severity and has certain clinical value for the judgment of children's condition. 展开更多

Methods: A total of 156 children who were diagnosed with obesity in Zigong Third People's Hospital between May 2014 and December 2016 were selected as research subjects and randomly divided into the intervention group who received exercise and dietary comprehensive intervention and the control group who received no special intervention. Before enrollment as well as 5 weeks and 10 weeks after enrollment, the serum contents of adipocytokines and inflammatory response indicators were detected, and the insulin sensitivity was assessed. Results: Serum Leptin, Chemerin, TNF-α, IL-6, hs-CRP, MCP-1, VCAM-1 and ICAM-1 contents as well as FBG and F-Ins levels of intervention group 5 weeks and 10 weeks after enrollment were significantly lower than those before enrollment while Adiponectin and Vaspin contents were significantly higher than those before enrollment;serum Leptin, Chemerin, Adiponectin, Vaspin, TNF-α, IL-6, hs-CRP, MCP-1, VCAM-1 and ICAM-1 contents as well as FBG and F-Ins levels of control group 5 weeks and 10 weeks after enrollment were not different from those before enrollment. Conclusion: Comprehensive exercise and dietary intervention can improve the lipid metabolism, insulin sensitivity and microinflammatory state in obese children. 展开更多
