Inflammatory bowel disease(IBD)is believed to be caused by various factors,including abnormalities in disease susceptibility genes,environmental factors,immune factors,and intestinal bacteria.Proton pump inhibitors(PPIs)are the primary drugs used to treat acid-related diseases.They are also commonly prescribed to patients with IBD.Recent studies have suggested a potential association between the use of certain medications,such as PPIs,and the occurrence and progression of IBD.In this review,we summarize the potential impact of PPIs on IBD and analyze the underlying mechanisms.Our findings may provide insights for conducting further investigations into the effects of PPIs on IBD and serve as an important reminder for physicians to exercise caution when prescribing PPIs to patients with IBD. 展开更多

背景与目的全球肺癌的发病率正呈逐年上升趋势,其中腺癌所占的百分比日益升高。据统计,全球的肺癌平均初诊年龄在70岁左右,虽然肺癌仍以老年患者居多,但发病年龄的年轻化趋势愈加明显。结合现有研究数据,我们已知在非小细胞肺癌中,年轻... 背景与目的全球肺癌的发病率正呈逐年上升趋势,其中腺癌所占的百分比日益升高。据统计,全球的肺癌平均初诊年龄在70岁左右,虽然肺癌仍以老年患者居多,但发病年龄的年轻化趋势愈加明显。结合现有研究数据,我们已知在非小细胞肺癌中,年轻患者疾病的发生有其独特的生物学特点。但年轻肺腺癌患者的基因组学特性和临床特征仍有待确定。本研究采用高通量测序(next-generation sequencing technology,NGS)技术对中国年轻肺腺癌患者的基因突变状态进行了研究。方法共收集了89例年龄≤45岁的肺腺癌患者组织标本,所有患者均知情同意。使用NGS检测用于确定癌组织中驱动基因突变。此外,对同期行NGS检测的95例>45岁肺腺癌患者的基因组和临床病理特征进行回顾性分析。结果根据年龄分类对184例肺腺癌患者的驱动基因突变频率进行了分析,揭示了年龄≤45岁的年轻组患者的独特基因特征。其中间变淋巴瘤激酶(anaplastic lymphoma kinase,ALK)融合基因和人表皮生长因子受体-2(human epidermal growth factor receptor 2,HER2)基因的突变频率较高。而鼠类肉瘤病毒癌基因(kirsten rat sarcoma viral oncogene,KRAS)、丝氨酸/苏氨酸蛋白激酶11(serine/threonine kinase 11,STK11)和表皮生长因子受体(epidermal growth factor receptor,EGFR)20外显子突变的趋势则相反,这些突变在年龄>45岁的老年组中更为常见。此外,年轻组EGFR基因突变同时伴有肿瘤蛋白p53(tumor protein p53,TP53)基因突变较老年组更为普遍(81.6%vs 44.9%),这可能使其应用EGFR酪氨酸激酶抑制剂(EGFR-tyrosine kinase inhibitor,EGFR-TKI)后疗效较差。结论NGS分析显示年轻腺癌患者具有独特的基因突变特点。在年轻患者中发现EGFR/TP53共突变的频率较高,这些独特的基因组学特征对临床治疗有重要的指导意义。 展开更多

背景与目的本研究旨在探究非小细胞肺癌患者应用帕博利珠单抗治疗前后外周血淋巴细胞亚群的变化及临床意义。方法选取2015年1月-2020年12月至青岛大学附属医院及潍坊市人民医院行帕博利珠单抗治疗的32例非小细胞肺癌的患者为观察组,另... 背景与目的本研究旨在探究非小细胞肺癌患者应用帕博利珠单抗治疗前后外周血淋巴细胞亚群的变化及临床意义。方法选取2015年1月-2020年12月至青岛大学附属医院及潍坊市人民医院行帕博利珠单抗治疗的32例非小细胞肺癌的患者为观察组,另外选取同期30例健康查体者为对照组。治疗前、治疗后第1、2、4周期,采用流体细胞术检测患者外周血淋巴细胞亚群水平的变化。结果非小细胞肺癌患者治疗前的CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)指标较对照组明显下降(P<0.05),CD8^(+)水平明显升高(P<0.05);应用帕博利珠单抗治疗1个周期后,淋巴细胞亚群较免疫治疗前的变化未见明显统计学差异;治疗2周期后CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)数值较治疗前升高(P>0.05),CD8^(+)指标较治疗前下降(P<0.05);治疗4周期后CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)数值较治疗前得到显著改善(P<0.05),CD8^(+)指标较治疗前明显下降(P<0.05);疗效达到疾病稳定(stable disease,SD)/部分缓解(partial response,PR)患者在治疗过程中,第4周期的CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)数值较治疗前升高(P<0.05),CD8^(+)指标较治疗前降低(P<0.05);疗效达到疾病进展(progressive disease,PD)患者在治疗过程中,淋巴细胞亚群变化较治疗前均没有明显统计学差异(P>0.05)。同时本文通过分析显示,程序性死亡受体配体1(programmed cell death ligand 1,PD-L1)表达与否及病理学类型对免疫治疗效果的影响不明显。通过多因素分析显示,同时观察CD3^(+)、CD4^(+)、CD8^(+)的变化对预测免疫治疗效果更有意义。结论帕博利珠单抗可以调节非小细胞肺癌患者T淋巴细胞亚群的变化,改善患者机体的免疫状态,且未见明显不良反应,同时治疗过程中监测淋巴细胞亚群变化可预测免疫治疗效果。 展开更多

目的探讨多奈哌齐(胆碱酯酶抑制剂)对血管性痴呆(vascular dementia,VD)大鼠室管膜下区(subventricular zone,SVZ)神经发生相关因子血管内皮生长因子(vascular endothelial growth factor,VEGF)及胰岛素样生长因子-1(insulin-like growt... 目的探讨多奈哌齐(胆碱酯酶抑制剂)对血管性痴呆(vascular dementia,VD)大鼠室管膜下区(subventricular zone,SVZ)神经发生相关因子血管内皮生长因子(vascular endothelial growth factor,VEGF)及胰岛素样生长因子-1(insulin-like growth factor-1,IGF-1)的影响。方法将雄性大鼠按随机数字表法分为假手术+溶剂组、2VO+溶剂组、2VO+多奈哌齐组3组。用永久性结扎大鼠双侧颈总动脉法(two-vessel occlusion,2VO)制造VD模型。用Morris水迷宫系统对各组大鼠进行学习记忆能力行为学测试。采用5-溴-2-脱氧尿嘧啶核苷(5-bromo-2-deoxyuridine,Brd U)标记增殖细胞,Nestin标记神经干细胞,免疫荧光观察各组大鼠SVZ内nestin+/Brd U+细胞的数量。Western blot分析各组大鼠SVZ中VEGF和IGF-1蛋白表达量的变化。结果 2VO+溶剂组大鼠水迷宫表现明显比另两组差,差异有统计学意义(P <0. 05)。术后15 d,2VO+多奈哌齐组大鼠SVZ中nestin+/Brd U+细胞数,及VEGF和IGF-1蛋白表达量较2VO+溶剂组显著增多(P <0. 05)。结论我们认为多奈哌齐对VD大鼠SVZ神经发生相关因子VEGF和IGF-1表达量有显著上调作用,并部分通过提高VEGF和IGF-1表达量促进VD后SVZ神经发生。 展开更多

Patatin-like phospholipase domain-containing 3(PNPLA3 or adiponutrin) displays anabolic and catabolic activities in lipid metabolism,and has been reported to be significantly associated with liver fat content.Variousstudies have established a strong link between the 148 isoleucine to methionine protein variant(I148M) of PNPLA3 and liver diseases,including nonalcoholic fatty liver disease(NAFLD).However,detailed demographic and ethnic characteristics of the I148 M variant and its role in the development of nonalcoholic fatty liver fibrosis have not been fully elucidated.The present review summarizes the current knowledge on the association between the PNPLA3 I148 M variant and NAFLD,and especially its role in the development of nonalcoholic fatty liver fibrosis.First,we analyze the impact of demographic and ethnic characteristics of the PNPLA3 I148 M variant and the presence of metabolic syndrome on the association between PNPLA3 I148 M and NAFLD.Then,we explore the role of the PNPLA3 I148 M in the development of nonalcoholic fatty liver fibrosis,and hypothesize the underlying mechanisms by speculating a pro-fibrogenic network.Finally,we briefly highlight future research that may elucidate the specific mechanisms of the PNPLA3 I148 M variant in fibrogenesis,which,in turn,provides a theoretical foundation and valuable experimental data for the clinical management of nonalcoholic fatty liver fibrosis. 展开更多

Helicobacter pylori (H. pylorl) is a Gram-negative bac- terium that infects half of the human population. The infection is associated with chronic inflammation of the gastric mucosa and peptic ulcers. It is also a m... Helicobacter pylori (H. pylorl) is a Gram-negative bac- terium that infects half of the human population. The infection is associated with chronic inflammation of the gastric mucosa and peptic ulcers. It is also a major risk factor for gastric cancer. Phylogenetic analysis of global strains reveals there are seven populations of H. pylori, including hpAfrical, hpAfrica2, hpEastAsia, hpEurope, hpNEAfrica, hpAsia2 and hpSahul. These populations are consistent with their geographical origins, and pos- sibly result from geographical separation of the bac- terium leading to reduced bacterial recombination in some populations. For each population, H. pylori has evolved to possess genomic contents distinguishable from others. The hpEurope population is distinct in that it has the largest genome of 1.65 mbp on average, and the highest number of coding sequences. This confers its competitive advantage over other populations but at the cost of a lower infection rate. The large genomic size could be a cause of the frequent occurrence of the deletion of the cag pathogenicity island in H. pylori strains from hpEurope. The incidence of gastric cancer varies among different geographical regions. This can be attributed in part to different rates of infection of H. pylori. Recent studies found that different popula- tions of H, pylori vary in their carcinogenic potential and contribute to the variation in incidence of gastric cancer among geographical regions. This could be related to the ancestral origin of H, pylori. Further studies are indi- cated to investigate the bacterial factors contributing to differential virulence and their influence on the clinical features in infected individuals. 展开更多

We analyzed a novel cationic collector using chemical plant byproducts,such as cetyltrimethylammonium bromide(CTAB)and dibutyl phthalate(DBP).Our aim is to establish a highly effective and economical process for the removal of quartz from collophane.A microflotation test with a 25 mg·L^(−1)collector at pH value of 6-10 demonstrates a considerable difference in the floatability of pure quartz and fluorapatite.Flotation tests for a collophane sample subjected to the first reverse flotation for magnesium removal demonstrates that a rough flotation process(using a 0.4 kg·t−1 new collector at pH=6)results in a collophane concentrate with 29.33wt%P_(2)O_(5)grade and 12.66wt%SiO2 at a 79.69wt%P_(2)O_(5)recovery,providing desirable results.Mechanism studies using Fourier transform infrared spectroscopy,zeta potential,and contact angle measurements show that the adsorption capacity of the new collector for quartz is higher than that for fluorapatite.The synergistic effect of DBP increases the difference in hydrophobicity between quartz and fluorapatite.The maximum defoaming rate of the novel cationic collector reaches 142.8 mL·min−1.This is considerably higher than that of a conventional cationic collector. 展开更多

Next, screening indicators were collected through consultation with experts and statistical calculations. The indicators weight coefficient was calculated using the analytic hierarchy process(AHP). Finally, indicators were validated in two groups of nurses in two different training courses.Results:(1) We created a three-level indicator system: level-Ⅰ dimensions have 4 indicators, while level-Ⅱ dimensions and level-Ⅲ dimensions have 13 and 34 indicators, respectively;(2) The coefficient of expert's judgment is 0.840, familiarity is 0.914 and authority is 0.877, and the three rounds of coordination coefficient are 0.456, 0.553 and 0.715, respectively;(3) There are at least 56 indicators in alternative quantitative standards; and(4) The alpha reliability value of the indicator system in the two training course had no significant difference(P 〉 0.05). The same result was observed when examining two groups of nurses in one training course(P 〉 0.05).Conclusions: This study established a training system of quality evaluation standards for emergency nurse specialists that is objective, reliable, easy to operate and representative according to scientific selection and verification. This system can therefore provide a basis for quality evaluation and targeted improvement for ENS training in addition to promoting health. 展开更多

We report a single-frequency linearly polarized Q-switched fiber laser based on an Nb_(2)GeTe_(4)saturable absorber(SA).The Nb_(2)GeTe_(4)SA triggers passive Q-switching of the laser,and an un-pumped Yb-doped fiber together with a 0.08-nmbandwidth polarization-maintaining fiber Bragg grating(FBG)acts as an ultra-narrow bandwidth filter to realize singlelongitudinal-mode(SLM)oscillation.The devices used in the laser are all kept polarized,so as to ensure linearly polarized laser output.Stable SLM linearly polarized Q-switching operation at 1064.6 nm is successfully achieved,producing a laser with a shortest pulse width of 1.36μs,a linewidth of 28.4 MHz,a repetition rate of 28.3 kHz-95.9 kHz,and a polarization extinction ratio of about 30 dB.It is believed that the single-frequency linearly polarized pulsed fiber laser studied in this paper has great application value in gravitational wave detection,beam combining,nonlinear frequency conversion,and other fields. 展开更多

Non-glomerular Tip Lesion Focal Segmental Glomerulosclerosis as a Negative Predictor in Idiopathic Membranous Nephropathy

Objective To assess the significance of focal segmental glomerulosclerosis(FSGS)variants on clinicopathological characteristics and short-term outcomes in idiopathic membranous nephropathy(IMN)patients.Methods The clinicopathological data of 146 IMN patients diagnosed between December 2016 and March 2019 in our center were collected and analyzed.These patients were divided into the pure IMN group,IMN with glomerular tip lesion(GTL)group,and IMN with non-GTL FSGS group.Results The IMN with non-GTL FSGS and IMN with GTL groups both had higher proportions of patients with hypertension,lower serum albumin,and severe proteinuria,while the IMN with non-GTL FSGS group additionally showed higher blood pressure and serum cholesterol,and lower serum IgG than the IMN group(all P<0.05).As for pathology,the IMN with non-GTL FSGS group had higher proportions of patients with acute tubular injury and moderate to severe chronic injuries than the IMN group(all P<0.05).In the IMN,IMN with GTL,and IMN with non-GTL FSGS groups,the overall one-year remission rates were 81.6%,76%,and 58.8%,respectively.Furthermore,the IMN with non-GTL FSGS group showed the lowest cumulative incidence to reach remission within one year.Multivariate Cox logistic analysis demonstrated that higher level of serum anti-M-type phospholipase A2 receptor antibody and the existence of non-GTL FSGS lesion were independent predictors for no remission in IMN patients.Conclusion The non-GTL FSGS lesion was a novel negative predictor in IMN and should be taken into account in the management of IMN. 展开更多

Recently,biomass-derived three-dimensional (3D) porous carbon materials have been gaining more interest as promising microwave absorbers due to their low cost,vast availability,and sustainability.Here,a novel 3D interconnected porous magnetic carbon foams are in-situ synthesized via a combination of sol-gel and carbonization process with wheat straw as the carbon source and FeCl3·6H2O as the magnetic regulating agent.During the process of foams formation,the lignocelluloses from the steam-exploded wheat straw are converted into interconnected carbon sheet networks with hierarchical porous structures,and the precursor FeCl3·6H2O is converted into magnetic nanoparticles uniformly embedded in the porous carbon foams.The generated magnetic nanoparticles are benefit to enhance the interface polarization and magnetic loss ability to improve the efficient complementarities between the dielectric and magnetic loss,thus increasing the impedance matching.The obtained sample treated at 600 ℃ displays the best microwave absorption (MA) performance.It presents a minimal reflection loss (RL) of-43.6 dB at 7.1 GHz and the effective bandwidth (RL <-10 dB) is 3.3 GHz with the thickness of 4.7 mm.The 3D porous structure,multi-interfaces and the synergy of dielectric loss and magnetic loss make great contribution to the outstanding MA performance. 展开更多

KOH-Activated Porous Carbons Derived from Chestnut Shell with Superior Capacitive Performance 被引量:4

Porous carbons (PC) were prepared from a waste biomass named chestnut shell via a two-step method involving carbonization and KOH activation. The morphology, pore structure and surface chemical properties were investigated by scanning electron microscopy (SEM), N2 sorption, Raman spectroscopy, X-ray diffraction (XRD) and X-ray photoelectron spectroscopy (XPS). The carbons have been evaluated as the electrode materials for supercapacitors by a two-electrode system in 6 mol/L KOH electrolyte. Benefiting from the porous texture, high surface area and high oxygen content, the PCs derived from chestnut shell have exhibited high specific capacitance of 198.2 (PC-l), 217.2 (PC-2) and 238.2 F·g^-1 (PC-3) at a current density of 0.1 Aog l, good rate capability of 55.7%, 56.6% and 54.9% in a range of 0.1 -20 A·g^-1 and high energy density of 5.6, 6.1 and 6.7 Wh·kg^-1, respectively. This is believed to be due to electric double layer capacitance induced by the abundant micropores and extra pseudo-capacitance generated by oxygen-containing groups. At a power density of 9000 Wh·kg^-1, the energy density is 3.1, 3.5 and 3.7 Whokg 1 for PC-l, PC-2 and PC-3, respectively, demonstrating the potential of the carbons derived from chestnut shells in energy storage devices. 展开更多

Mg_(3)Sb_(2) as a Zintl compound is a promising thermoelectric material with the intrinsically low lattice thermal conductivity and excellent n-type electrical properties,but its p-type electrical transport properties are poor.Here,the thermoelectric performance of Mg_(3)Sb_(2) under the effect of biaxial strain is investigated by using first-principles method and Boltzmann transport theory.The application of biaxial strain enables tuning the band structure of Mg_(3)Sb_(2) in such a way that the band degeneracy of both the conduction band and valence band increases.As the biaxial strain increases,the Seebeck coefficient of ptype Mg_(3)Sb_(2) has a remarkable increase,leading to a significant improvement in power factor.This is mainly ascribed to the achievement of valence band orbital degeneracy.Meanwhile,the lattice thermal conductivity exhibits very slight biaxial strain dependence within the strain range considered in this work,which increases from 1.28 to 1.62 W m^(-1) K^(-1) at 300 K.Finally,the highest ZT of p-type Mg_(3)Sb_(2) at 700 K can be up to 2.6 along the in-plane direction under-2.5%biaxial strain,which is almost three times that of the unstrained counterpart.The realization of high thermoelectric performance of p-type Mg_(3)Sb_(2) will promote its practical applications as thermoelectric generators. 展开更多

By using tetrathiafulvalene as reducing and doping agents, three-dimensional (3D) sulfur-doped graphene hydrogels (SGHs) were facilely prepared in mixed solvents of dimethyl formamide and water. Several investigations reveal that TTF plays a critical role in the formation of such unique 3D architecture, as it not only reduces GO to self-assembly into 3D structures, but also can be transformed to TTF^·+ and TTF^2+ as doping agents in the reduction process. The morphology, crystal structure, chemical bonding, elemental composition and porosity of the as-prepared SGHs have been studied. Benefiting from well-defined and cross-linked 3D porous network architectures, the supercapacitors based on the SGHs in KOH 212.5 F·g^-1 at 0.3 A·g^-1. Furthermore, this capacitance also degree of reversibility in the repetitive charge/discharge cycling electrolyte exhibited a high specific capacitance of showed good electrochemical stability and a high test. 展开更多
