Background:Ischemia-reperfusion can worsen myocardial damage and increase the risk of death.Studies have revealed that ischemic preconditioning provides the best endogenous protection against myocardial ischemia-reperfusion injury(MIRI),and the principle of electroacupuncture(EA)preconditioning is comparable to that of myocardial ischemic preconditioning adaption.Our earlier research demonstrated that EA pretreatment inhibits the expression of calmodulin-dependent protein kinase IIδ(CaMKIIδ),sodium/calcium exchanger 1(NCX1),and cyclophilin D,hence providing protection against MIRI.However,the exact mechanism is still unknown.The expression of NCX1 mRNA is directly regulated by microRNA-214(miR-214).Moreover,it suppresses the levels of CaMKIIδand cyclophilin D.Whether these variables contribute to EA preconditioning to improve MIRI needs to be investigated,though.This study aimed to preliminarily determine whether EA pretreatment ameliorates MIRI by modulating the miR-214-3p/NCX1 axis.Methods:We used a rat MIRI model to investigate the effect of EA pretreatment on MIRI and the expression of miR-214-3p.In addition,adenovirus injection inhibited miR-214-3p expression in the rat MIRI model,and the influence of EA pretreatment towards MIRI was observed in the context of blocked miR-214-3p expression.Both the myocardial histological abnormalities and the alterations in the ST segment of the rat electrocardiogram were analyzed.NCX1 mRNA,cyclophilin D,and CaMKIIδexpression levels were also analyzed.Results:EA pretreatment improved MIRI.In rats with MIRI,EA administration increased miR-214-3p expression while decreasing NCX1 mRNA,cyclophilin D,and CaMKIIδproteins in cardiac tissues.The beneficial effect of EA pretreatment against MIRI was reversed,coupled with elevated levels of NCX1 mRNA,cyclophilin D,and CaMKIIδprotein expression,when an adenovirus injection disrupted the expression of miR-214-3p.Conclusions:Our findings preliminarily show that EA pretreatment inhibits the expression of NCX1 mRNA,cyclophilin D,and CaMKIIδproteins via miR-214-3p,hence exerting MIRI protection. 展开更多

目的探讨血清超敏C反应蛋白(hs-CRP)、白细胞介素27(IL-27)对糖尿病肾病(DN)的早期诊断价值。方法选取2020年1月—2021年7月于青岛市海慈医院住院治疗的DN患者146例作为观察组,同时选取同期该院就诊的糖尿病患者60例作为对照组。比较两... 目的探讨血清超敏C反应蛋白(hs-CRP)、白细胞介素27(IL-27)对糖尿病肾病(DN)的早期诊断价值。方法选取2020年1月—2021年7月于青岛市海慈医院住院治疗的DN患者146例作为观察组,同时选取同期该院就诊的糖尿病患者60例作为对照组。比较两组患者的收缩压、舒张压、体质量指数(BMI)、空腹血糖(FPG)、餐后2小时血糖(2 h PG)、总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、hs-CRP、IL-27水平以及尿白蛋白/肌酐比值(UACR)、肾小球滤过率估计值(eGFR)。采用逐步多因素非条件Logistic回归模型分析DN发生、发展的影响因素,采用受试者工作特征(ROC)曲线预测血清hs-CRP、IL-27水平对DN的早期诊断价值。结果观察组TC、TG、LDL-C、hsCRP、IL-27、UACR水平高于对照组组,eGFR水平低于对照组(P<0.05)。两组FPG、2 hPG、HDL-C水平比较,差异无统计学意义(P>0.05)。逐步多因素非条件Logistic回归模型分析结果显示,TC水平[OR=1.721(95%CI:1.187,2.495),P<0.05]、hs-CRP水平[OR=2.409(95%CI:1.477,3.929),P<0.05]、IL-27水平[OR=1.987(95%CI:1.256,3.145)]及UACR值[OR=1.693(95%CI:1.127,2.543),P<0.05]是DN的影响因素。ROC曲线分析结果显示:hs-CRP、IL-27单独诊断时AUC为0.769(95%CI:0.596,0.941)、0.827(95%CI:0.717,0.939)。而联合诊断时AUC为0.895(95%CI:0.848,0.929),诊断效能较高。结论血清hs-CRP、IL-27水平升高是DN的影响因素,血清hs-CRP、IL-27可以作为DN的早期诊断指标。 展开更多

目的分析O-RADS、GI-RADS、ADNEX模型超声诊断分类系统鉴别附件肿瘤良恶性的价值及一致性。方法回顾性分析2017年1月—2020年12月在常州市第二人民医院行手术治疗的299例患者的324个附件肿瘤,由3位经验丰富的超声医师分别根据O-RADS、GI... 目的分析O-RADS、GI-RADS、ADNEX模型超声诊断分类系统鉴别附件肿瘤良恶性的价值及一致性。方法回顾性分析2017年1月—2020年12月在常州市第二人民医院行手术治疗的299例患者的324个附件肿瘤,由3位经验丰富的超声医师分别根据O-RADS、GI-RADS、ADNEX模型进行分类,绘制受试者工作特征曲线,获得最佳分界值,以病理检查结果为金标准,分析3种超声分类系统诊断附件肿瘤的有效性。使用κ值评估各超声医师分类的一致性。结果当取O-RADS>3类的cut-off值时,其诊断附件肿瘤良恶性的曲线下面积(AUC)为0.981,大于GI-RADS(0.934)和ADNEX模型(0.907)(P<0.05);而GI-RADS与ADNEX模型的AUC比较,差异无统计学意义(P>0.05)。O-RADS诊断附件肿瘤良恶性的敏感性为96.0%,高于GI-RADS(90.0%)和ADNEX模型(88.0%)(P<0.05);GI-RADS与ADNEX模型的敏感性比较,差异无统计学意义(P>0.05)。O-RADS、GI-RADS、ADNEX模型诊断附件肿瘤良恶性的特异性分别为92.9%、88.4%和93.3%,差异无统计学意义(P>0.05)。3位超声医师采用O-RADS、GI-RADS、ADNEX模型诊断附件肿瘤良恶性的一致性均较好,κ值为0.857~0.937。结论O-RADS、GI-RADS、ADNEX模型均有较高的诊断价值,且一致性较好,但O-RADS的诊断效能优于GI-RADS和ADNEX模型。 展开更多

Objective To investigate the relation between air pollution exposure and preterm birth in Shanghai, China. Methods We examined the effect of ambient air pollution on preterm birth using time-series approach in Shanghai in 2004. This method can eliminate potential confounding by individual risk factors that do not change over a short period of time. Daily numbers of preterm births were obtained from the live birth database maintained by Shanghai Municipal Center of Disease Control and Prevention. We used the generalized additive model (GAM) with penalized splines to analyze the relation between preterm birth, air pollution, and covariates. Results We observed a significant effect of outdoor air pollution only with 8-week exposure before preterm births. An increase of 10 μg/m^3 of 8-week average PM10, SO2, NO2, and O3 corresponded to 4.42% (95%CI 1.60%, 7.25%), 11.89% (95%CI 6.69%, 17.09%), 5.43% (95%CI 1.78%, 9.08%), and 4.63% (95%CI 0.35%, 8.91%) increase of preterm birth. We did not find any significant acute effect of outdoor air pollution on preterm birth in the week before birth. Conclusion Ambient air pollution may contribute to the risk of preterm birth in Shanghai. Our analyses also strengthen the rationale for further limiting air pollution level in the city. 展开更多

BACKGROUND Mesonephric adenocarcinoma(MNA)of the female reproductive system is a rare tumor arising from remnants of the mesonephric duct,which is mainly located in the cervix.MNA often occurs in adult women.Due to the rarity of the disease and few reports,the specific clinical features have not been established.CASE SUMMARY We present a case of a cervical MNA in a 48-year-old woman with an incidental intra-operative diagnosis who received postoperative chemotherapy.Rare lung metastases were detected during follow-up.The existing literature is reviewed.CONCLUSION The clinical manifestations,pathological characteristics,diagnosis,treatment,and prognosis of MNA have been summarized through the review of the existing literature and the case in this paper.Due to the rarity of this disease,it is very important for the research of MNA in the future. 展开更多

Methods A generalized additive model with penalized spline function was used to observe the acute effect of PM10 and O3 on daily mortality. Results Higher PM10 significantly increased the effect of 03 on total mortality, and O3 also increased the effect of PM10 although the estimated increment was statistically insignificant. Conclusion Our findings provide further evidence for the effect of PM10 and O3 on daily mortality. 展开更多

目的:探讨分析玻璃体腔注射康柏西普后短期眼压(IOP)升高的影响因素。方法:临床前瞻性观察研究。共纳入玻璃体腔注射康柏西普的视网膜病变患者269例269眼,所有患眼均行康柏西普玻璃体腔注射治疗。其中,男143例,女126例;年龄相关性黄斑变... 目的:探讨分析玻璃体腔注射康柏西普后短期眼压(IOP)升高的影响因素。方法:临床前瞻性观察研究。共纳入玻璃体腔注射康柏西普的视网膜病变患者269例269眼,所有患眼均行康柏西普玻璃体腔注射治疗。其中,男143例,女126例;年龄相关性黄斑变性(ARMD)患者201例,其他类型视网膜变性患者68例。平均年龄62.86±11.74岁,采用非接触气动式眼压计,分别在注射前,注射后10、30min,2和4h对患者进行IOP测量。根据注射后10min IOP升高情况来进行分组,IOP升高10 mmHg及以上定义为IOP升高组,IOP升高小于10 mmHg定义为IOP稳定组。采用多因素Logistic回归分析分析两组之间的差异。结果:患者在注射后10、30min,2和4h的平均IOP分别为24.1、20.2、19.5、16.9 mmHg,注射后各个时间点的IOP较注射前平均升高6.7、3.1、1.7、0.5 mmHg。其中,IOP高组56例,IOP稳定组213例。两组患者年龄、最佳矫正视力(BCVA)、性别、眼别、疾病类型比较均无统计学差异(均P>0.05);注药次数(Z=-4.389,P=0.012)、注射前IOP及注射后各时间点的IOP(t=-5.343,-10.467,-8.933,-6.124,-4.635,均P<0.01)比较差异均有统计学差异。多变量Logistic回归分析显示,基础IOP与注射后10min IOP升高呈正相关(B=-0.913,OR=0.521,95%CI:0.211~0.694,P=0.011)。结论:基线IOP水平越高,注射后IOP升高的风险越高。基线IOP是影响玻璃体腔注射康柏西普后患眼短期IOP升高的主要相关因素,而注射次数可能是另一个风险因素。 展开更多

AIM:To investigate the expression profiles of the transcription factor specificity protein 1(Sp1)and collagenⅠin recurrent pterygial tissues.What is more,to compare the changes of Sp1 and collagen I among primary pterygial tissue,recurrent pterygial tissue and conjunctival tissue.METHODS:In the prospective study,we collected the pterygial tissues of 40 patients who underwent resection of primary pterygial tissue and recurrent pterygial tissue,and the conjunctival tissues of 10 patients with enucleation due to trauma.The relative expression levels of Sp1 and collagen I were analyzed by reverse transcription quantitative-polymerase chain reaction and Western blot.Paired t-test was performed to compare the Sp1 and collagen I of recurrent pterygial tissues,as well as the primary pterygial tissues and conjunctival tissues.In further,Pearson’s hypothesis testing of correlation coefficients was used to compare the correlations of Sp1 and Collagen I.RESULTS:The content of Sp1 and collagen I m RNA and protein was significantly greater in recurrent pterygial tissue than that was in primary and conjunctival tissue(P<0.05).There was a positive correlation between the m RNA and protein levels of Sp1 and collagen I in recurrent pterygial tissues(protein:r=0.913,P<0.05;m RNA:r=0.945,P<0.05).CONCLUSION:Sp1 and collagen I are expressed in normal conjunctival,primary,and recurrent pterygial tissues,but expression is significantly greater in the latter.Sp1 and collagen I may be involved in the regulation of the development of recurrent pterygium. 展开更多

BACKGROUND Salmonella derby(S.derby)is a Gram-negative diplococcus that is common in the digestive tract.Infected patients generally experience symptoms such as fever and diarrhea.Mild cases are mostly self-healing gastroenteritis,and severe cases can cause fatal typhoid fever.Clinical cases are more common in children.The most common form of S.derby infection is self-healing gastroenteritis,in which,fever lasts for about 2 d and diarrhea for<7 d.S.derby can often cause bacterial conjunctivitis,pneumonia,endocarditis,peritonitis and urethritis.However,intracranial infections in infants caused by S.derby are rare in clinical practice and have not been reported before in China.CASE SUMMARY A 4-mo-old female infant had recurrent fever for 2 wk,with a maximum body temperature of around 39.4°C.Treatment for infectious fever in a local hospital was ineffective,and she was admitted to our hospital.Before admission,there was one sudden convulsion,characterized by unclear consciousness,limb twitching,gaze in both eyes,and slight cyanosis on the face.Cerebrospinal fluid(CSF)culture was positive for Gram-negative bacilli,which conformed to S.derby.After treatment with meropenem and ceftriaxone antibiotics,the patient was discharged home in a clinically stable state after 4 wk of treatment.CONCLUSION We reported a rare case of S.derby cultured in CSF.S.derby enters the CSF through the blood–brain barrier,causing purulent meningitis.If not treated timeously,it can lead to serious,life-threatening infection. 展开更多

Down-regulation of Helios Expression in Tregs from Patients with Hypertension

Regulatory T cells(Tregs) play a pivotal role in the pathological development of hypertension.Helios,a transcription factor from the Ikaros family,was recently reported to be a bona fide marker for natural Tregs or activated Tregs with suppression function,however,little has been known about its role in hypertension.This study was aimed to find whether Helios+ Tregs really play a vital role in hypertension.A total of 60 hypertension patients,and 46 normotension subjects were enrolled in this study.Frequencies of different Tregs subsets in peripheral blood were measured by flow cytometry.Plasma cytokine level was determined by ELISA.The m RNA expression of Foxp3 and Helios in purified CD4+ T cells was detected by RT-PCR.Proportion of CD4+Foxp3+Helios+ Tregs was decreased significantly in patients with hypertension(62.52%±1.18% vs.71.89%±1.03%,P〈0.01),and it was correlated with plasma level of IL-10 positively(r=0.505,P〈0.05) and plasma level of IFN-gamma negatively(r=–0.551,P〈0.05).The m RNA expression of Foxp3(7.23±1.00 vs.10.58±0.54,P〈0.05) and Helios(8.47±0.95 vs.15.52±2.0,P〈0.05) was decreased in CD4+ T cells from patients with hypertension.Helios+ Tregs were decreased in patients with hypertension and may play a protective role in hypertension progression. 展开更多

Fine-root decomposition is a critical process regulating ecosystem carbon cycles and affecting nutrient cycling and soil fertility.However,whether interaction between warming and grazing affects fine-root decomposition is still under-researched in natural grasslands.A two-factorial experiment with asymmetric warming(i.e.daytime vs.nighttime and growing season vs.nongrowing season)and moderate grazing(i.e.about average 50%forage utilization rate)was conducted to explore whether warming and grazing affect fine-root decomposition and loss of nutrients during a 2-year decomposition period in an alpine meadow on the Tibetan Plateau.Both warming and grazing facilitated carbon cycling through increase in fine-root decomposition,and influenced element cycling which varies among elements.The effects of warming and grazing on fine-root decomposition and loss of nutrients were additive.Both warming and grazing significantly increased cumulative percentage mass loss and total organic carbon loss of fine roots during the 2-year experiment.Only warming with grazing treatment reduced percentage nitrogen loss,whereas warming,regardless of grazing,decreased percentage phosphorus loss.Warming and grazing alone increased percentage loss of potassium,sodium,calcium and magnesium compared with control.There were no interactions between warming and grazing on fine-root decomposition and loss of nutrients.There was greater temperature sensitivity of decreased phosphorus loss than that of decreased nitrogen loss.Different temperature sensitivities of percentage loss of nutrients from fine-root decomposition would alter ratios of the available nutrients in soils,and may further affect ecosystem structure and functions in future warming. 展开更多

Background A large number of studies pointed that being delivered by cesarean section(CS)would affect the health outcomes of offspring,however,whether CS would affect the risk of childhood leukemia remained uncertain.This study conducted a meta-analysis to quantitatively evaluate whether being delivered by CS would influence the onset of childhood leukemia.Methods PubMed,Embase and Web of Science databases were searched from 3rd June,1950 to 13th October,2019 to identify the literature,which examined the relationship between CS and childhood leukemia.This study used Newcastle-Ottawa Scale to assess the quality of literature.Subgroup analyses were conducted on region,mode of delivery,design of the study and number of confounders adjusted.Egger's test and Begg's test were performed to evaluate possible publication bias.Results The pooled odds ratio(OR)estimates illustrated that children delivered by CS had a higher risk of developing leukemia[OR 1.10,95%confidence interval(CI)1.04-1.17,P=0.002]and lymphoblastic leukemia(OR 1.12,95%CI 1.03-1.23,P=0.009),while a significant association for myeloid leukemia was not observed(OR 1.05,95%CI 0.92-1.20,P=0.451).Results of subgroup analyses indicated that elective CS would increase the risk of childhood lymphoblastic leukemia(OR 1.16,95%CI 1.06-1.27,P=0.002).However,a statistical relationship between emergency CS and lymphoblastic leukemia was not observed(OR 1.07,95%CI 0.93-1.23,P=0.364).Conclusions CS would increase the risk of childhood lymphoblastic leukemia.It is worth noting that subgroup analyses shows that elective CS rather than emergency CS increases the risk of lymphoblastic leukemia in offspring. 展开更多
