Early recurrence(ER)following surgery for rectal cancer is a significant factor impacting patient survival rates.Tsai et al identified age,preoperative neoadjuvant therapy,length of hospital stay,tumour location,and pathological stage as factors influencing the risk of ER.Postoperative monitoring for ER should encompass a thorough medical history review,physical examination,tumour marker testing,and imaging studies.Additionally,noninvasive circulating tumour cell DNA testing can be utilized to predict ER.Treatment strategies may involve radical surgery,radiation therapy,chemotherapy,and immunotherapy.Through a comprehensive analysis of risk factors,the optimization of monitoring methods,and the development of personalized treatment strategies,it is anticipated that both the efficacy of treatment and the quality of life for rectal cancer patients with postoperative recurrence can be significantly improved. 展开更多

BACKGROUND The study on predicting the differentiation grade of colorectal cancer(CRC)based on magnetic resonance imaging(MRI)has not been reported yet.Developing a non-invasive model to predict the differentiation grade of CRC is of great value.AIM To develop and validate machine learning-based models for predicting the differ-entiation grade of CRC based on T2-weighted images(T2WI).METHODS We retrospectively collected the preoperative imaging and clinical data of 315 patients with CRC who underwent surgery from March 2018 to July 2023.Patients were randomly assigned to a training cohort(n=220)or a validation cohort(n=95)at a 7:3 ratio.Lesions were delineated layer by layer on high-resolution T2WI.Least absolute shrinkage and selection operator regression was applied to screen for radiomic features.Radiomics and clinical models were constructed using the multilayer perceptron(MLP)algorithm.These radiomic features and clinically relevant variables(selected based on a significance level of P<0.05 in the training set)were used to construct radiomics-clinical models.The performance of the three models(clinical,radiomic,and radiomic-clinical model)were evaluated using the area under the curve(AUC),calibration curve and decision curve analysis(DCA).RESULTS After feature selection,eight radiomic features were retained from the initial 1781 features to construct the radiomic model.Eight different classifiers,including logistic regression,support vector machine,k-nearest neighbours,random forest,extreme trees,extreme gradient boosting,light gradient boosting machine,and MLP,were used to construct the model,with MLP demonstrating the best diagnostic performance.The AUC of the radiomic-clinical model was 0.862(95%CI:0.796-0.927)in the training cohort and 0.761(95%CI:0.635-0.887)in the validation cohort.The AUC for the radiomic model was 0.796(95%CI:0.723-0.869)in the training cohort and 0.735(95%CI:0.604-0.866)in the validation cohort.The clinical model achieved an AUC of 0.751(95%CI:0.661-0.842)in the training cohort and 0.676(95%CI:0.525-0.827)in the validation cohort.All three models demonstrated good accuracy.In the training cohort,the AUC of the radiomic-clinical model was significantly greater than that of the clinical model(P=0.005)and the radiomic model(P=0.016).DCA confirmed the clinical practicality of incorporating radiomic features into the diagnostic process.CONCLUSION In this study,we successfully developed and validated a T2WI-based machine learning model as an auxiliary tool for the preoperative differentiation between well/moderately and poorly differentiated CRC.This novel approach may assist clinicians in personalizing treatment strategies for patients and improving treatment efficacy. 展开更多

BACKGROUND In laparoscopic proximal gastrectomy(LPG),the prolapse of the hepatic left lateral lobe near the lesser curvature and esophageal hiatus can obstruct the field of vision and operation.Therefore,it is necessary to retract or obstruct the hepatic left lateral lobe to ensure a clear field of vision.AIM To investigate the safety and clinical efficacy of the modified hepatic left lateral lobe inversion technique for LPG.METHODS A retrospective analysis was conducted on the clinical data of 13 consecutive patients with early-stage upper gastric adenocarcinoma or adenocarcinoma of the esophagogastric junction treated with LPG from January to December 2023 at the Department of Gastrointestinal Surgery,Second Affiliated Hospital of Fujian Medical University.The modified hepatic left lateral lobe inversion technique was used to expose the surgical field in all patients,and short-term outcomes were observed.RESULTS In all 13 patients,the modified hepatic left lateral lobe inversion technique was successful during surgery without the need for re-retraction or alteration of the liver traction method.There were no instances of esophageal hiatus occlusion,eliminating the need for forceps to assist in exposure.There was no occurrence of intraoperative hepatic hemorrhage,hepatic vein injury,or hepatic congestion.No postoperative digestive complications of Clavien-Dindo grade≥II occurred wi-thin 30 days after surgery,except for a single case of pulmonary infection.Some patients experienced increases in alanine aminotransferase and aspartate aminotransferase levels on the first day after surgery,which significantly decreased by the third day and returned to normal by the seventh day after surgery.CONCLUSION The modified hepatic left lateral lobe inversion technique has demonstrated satisfactory results,offering ad-vantages in terms of facilitating surgical procedures,reducing surgical trauma,and protecting the liver. 展开更多

Clinical efficacy of modified Kamikawa anastomosis in patients with laparoscopic proximal gastrectomy

BACKGROUND With the increasing incidence of proximal gastric cancer,laparoscopic proximal gastrectomy has been applied.However,reflux esophagitis often occurs after traditional esophagogastric anastomosis.In order to solve this problem,several methods of digestive tract reconstruction have emerged,but the most satisfying method remains to be discussed.Therefore,we modified traditional Kamikawa anastomosis to investigate the appropriate digestive tract reconstruction in laparo-scopic proximal gastrectomy.All the patients were successfully operated on without conversion to laparotomy.The duration of operation and digestive tract reconstruction were 203.500(150-224)min and 87.500(73-111)min,respectively.The intraoperative amount of bleeding was 20.500 mL±0.696 mL.The time of postoperative first flatus,the first postoperative fluid intake,and the postoperative length of stay were 2(1-3)d,4(3-5)d,and 9(8-10)d,respectively.All the patients were followed up for 12-23 months.The body mass index at 6 and 12 months after surgery were 22.577 kg/m2±3.098 kg/m2 and 22.594 kg/m2±3.207 kg/m2,respectively.The nutrition risk screening 2002 score,the patient-generated subjective global assessment score,and the gastroesophageal reflux disease scale score were good at 6 and 12 months after surgery.Reflux esophagitis and anastomotic stenosis were not observed in any of the patients during their 12-month postoperative gastroscopy or upper gastrointestinal tract visits.All the patients exhibited no tumor recurrence or metastasis.CONCLUSION The modified Kamikawa anastomosis is safe and feasible for laparoscopic proximal gastrectomy and has good antireflux effects and nutritional status. 展开更多

The Lijiang 2.4-meter Telescope(LJT), the largest common-purpose optical telescope in China,has been available to the worldwide astronomical community since 2008. It is located at the Gaomeigu site,Lijiang Observatory... The Lijiang 2.4-meter Telescope(LJT), the largest common-purpose optical telescope in China,has been available to the worldwide astronomical community since 2008. It is located at the Gaomeigu site,Lijiang Observatory(LJO), in the southwest of China. The site has very good observational conditions.During its 10-year operation, several instruments have been equipped on the LJT. Astronomers can perform both photometric and spectral observations. The main scientific goals of LJT include recording photometric and spectral evolution of supernovae, reverberation mapping of active galactic nuclei, investigating the physical properties of binary stars and near-earth objects(comets and asteroids), and identification of exoplanets and all kinds of transients. Until now, the masses of 41 high accretion rate black holes have been measured, and more than 168 supernovae have been identified by the LJT. More than 190 papers related to the LJT have been published. In this paper, the general observation conditions of the Gaomeigu site is introduced at first. Then, the structure of the LJT is described in detail, including the optical, mechanical, motion and control system. The specification of all the instruments and some detailed parameters of the YFOSC is also presented. Finally, some important scientific results and future expectations are summarized. 展开更多

Objective: Infra-pyloric artery (IPA) is an important anatomical landmark in treatment of gastric cancer and is the key vessel for pylorus-preserving gastrectomy and subgroup of infra-pyloric lymph nodes. However, its anatomical variation is not thoroughly understood. Our study aimed to clarify the origination of the IPA. Methods: We did this prospective, multicenter, open-label, observational study at gastric surgery departments of 34 hospitals in China. Gastric cancer patients aged 18 years or older and scheduled to undergo elective total or distal gastrectomy were assigned. During the surgery, IPA dissecting and exposing the origination point with photographs or video clips were required. The primary outcome was the origination of the IPA. Analysis of variance, χ2 tests and Fisher's tests were used to analyze the differences between groups. The study is registered at Clinicaltrials.gov (No. NCT03071237). Results: Between May 8 and July 31, 2017, 429 patients were assigned for the study, and 419 (97.7%) patients had the IPA dissected and recorded through photograph or video and were included in the primary outcome analysis. The median age was 62 years old, and 73.7% were male. Among the patients, 78.5% received laparoscopic surgery. Single IPA origination was identified in 398 (95.0%) patients, including gastroduodenal artery (GDA) in 154 (36.8%) patients, anterior superior pancreaticoduodenal artery (ASPDA) in 130 (31.0%) patients, and right gastroepiploic artery (RGEA) in 114 (27.2%) patients. Fifteen (3.6%) patients were identified with multiple IPA and 6 (1.4%) patients were identified as IPA absence. The differences in the distribution of surgical approach (P=0.003) and geographic area (P=0.030) were statistically significant. No difference was shown in sex, age, gastrectomy type, tumor location, and clinical T, N and M stage. Conclusions: Our study found that the IPA originates from GDA, ASPDA and RGEA in similar proportions. Laparoscopic surgery may be more helpful in dissection of the IPA than open surgery. 展开更多

We installed two sets of Astronomical Site Monitoring Systems(ASMSs)at Lijiang Observatory(GMG),for the running of the 2.4-meter Lijiang optical telescope(LJT)and the 1.6-meter Multi-channel Photometric Survey Telescope(Mephisto).The Mephisto is under construction.The ASMS has been running on robotic mode since 2017.The core instruments:Cloud Sensor,All-Sky Camera and AutonomousDIMM that are developed by our group,together with the commercial Meteorological Station and Sky Quality Meter,are combined into the astronomical optical site monitoring system.The new Cloud Sensor's Cloud-Clear Relationship is presented for the first time,which is used to calculate the All-Sky cloud cover.We designed the Autonomous-DIMM located on a tower,with the same height as LJT.The seeing data have been observed for a full year.ASMS's data for the year 2019 are also analysed in detail,which are valuable to observers. 展开更多

Development and validation of a nomogram for preoperative prediction of tumor deposits in colorectal cancer 被引量:2

BACKGROUND Based on the clinical data of colorectal cancer(CRC)patients who underwent surgery at our institution,a model for predicting the formation of tumor deposits(TDs)in this patient population was established.AIM To establish an effective model for predicting TD formation,thus enabling clinicians to identify CRC patients at high risk for TDs and implement personalized treatment strategies.METHODS CRC patients(n=645)who met the inclusion criteria were randomly divided into training(n=452)and validation(n=193)cohorts using a 7:3 ratio in this retrospective analysis.Least absolute shrinkage and selection operator regression was employed to screen potential risk factors,and multivariable logistic regression analysis was used to identify independent risk factors.Subsequently,a predictive model for TD formation in CRC patients was constructed based on the independent risk factors.The discrimination ability of the model,its consistency with actual results,and its clinical applicability were evaluated using receiveroperating characteristic curves,area under the curve(AUC),calibration curves,and decision curve analysis(DCA).RESULTS Thirty-four(7.5%)patients with TDs were identified in the training cohort based on postoperative pathological specimens.Multivariate logistic regression analysis identified female sex,preoperative intestinal obstruction,left-sided CRC,and lymph node metastasis as independent risk factors for TD formation.The AUCs of the nomogram models constructed using these variables were 0.839 and 0.853 in the training and validation cohorts,respectively.The calibration curve demonstrated good consistency,and the training cohort DCA yielded a threshold probability of 7%-78%.CONCLUSION This study developed and validated a nomogram with good predictive performance for identifying TDs in CRC patients.Our predictive model can assist surgeons in making optimal treatment decisions. 展开更多

BACKGROUND Gastric ectopic pancreas(GEP)is a rare developmental abnormality that refers to the existence of pancreatic tissue in the stomach with no anatomical relationship with the main pancreas.It is usually difficult to diagnose through histological examination,and the choice of treatment method is crucial.AIM To describe the endoscopic ultrasound characteristics of GEP and evaluate the value of laparoscopic resection(LR)and endoscopic submucosal dissection(ESD).METHODS Forty-nine patients with GEP who underwent ESD and LR in the Second Affiliated Hospital of Fujian Medical University from May 2018 to July 2023 were retrospectively included.Data on clinical characteristics,endoscopic ultrasonography(EUS),ESD,and LR were collected and analyzed.The characteristics of EUS and the efficacy of the two treatments were analyzed.RESULTS The average age of the patients was 43.31±13.50 years,and the average maximum diameter of the lesions was 1.55±0.70 cm.The lesion originated from the mucosa in one patient(2.04%),from the submucosa in 42 patients(85.71%),and from the muscularis propria in 6 patients(12.25%).Twenty-nine patients(59.20%)with GEP showed umbilical depression on endoscopy.The most common initial symptom of GEP was abdominal pain(40.82%).Tumor markers,including carcinoembryonic antigen(CEA)and carbohydrate antigen 19-9(CA19-9),were generally within the normal range.One patient(2.04%)with GEP had increased CEA and CA-19-9 levels.However,no cancer tissue was found on postoperative pathological examination,and tumor markers returned to normal levels after resecting the lesion.There was no significant difference in surgery duration(72.42±23.84 vs 74.17±12.81 min)or hospital stay(3.70±0.91 vs 3.83±0.75 d)between the two methods.LR was more often used for patients with larger tumors and deeper origins.The amount of bleeding was significantly higher in LR than in ESD(11.28±16.87 vs 16.67±8.76 mL,P<0.05).Surgery was associated with complete resection of the lesion without any serious complications;there were no cases of recurrence during the follow-up period.CONCLUSION GEP has unique characteristics in EUS.LR and ESD seem to be good choices for treating GEP.LR is better for large GEP with a deep origin.However,due to the rarity of GEP,multicenter large-scale studies are needed to describe its characteristics and evaluate the safety of LR and ESD. 展开更多

In this study, graphene sheets are prepared under a hydrogen atmosphere without a catalyst, and the growth mechanism of graphene by direct current arc discharge is investigated experimentally and numerically. The size and layer numbers of graphene sheets increase with the arc current.Distributions of temperature, velocity, and mass fraction of carbon are obtained through numerical simulations. A high current corresponds to a high saturation temperature, evaporation rate, and mass density of carbon clusters. When the carbon vapor is saturated, the saturation temperatures are 3274.9, 3313.9, and 3363.6 K, and the mass densities are 6.4×1022,8.42×1022, and 1.23×1023 m-3 under currents of 150, 200, and 250 A, respectively. A hydrogen-induced marginal growth model is used to explain the growth mechanism. Under a high current, the condensation coefficient and van der Waals force increase owing to the higher saturation temperature and mass density of carbon clusters, which is consistent with experimental results. 展开更多

We present a polarimeter named YFPOL for the Cassegrain focus of the Lijiang 2.4 m Telescope(LJT) of Yunnan Observatories, Chinese Academy of Sciences. YFPOL is a traditional single-beam polarimeter with a rotating polarizer. As the focal-reducer instrument Yunnan Faint Object Spectrograph and Camera(YFOSC) is always positioned on the Cassegrain focal plane of LJT, we develop two sets of ultra-thin(thickness <12 mm) polarizer rotation control systems with wireless charging and control functions, which are suitable for mounting on the two front-wheels of YFOSC. One set is used as the polarimetric calibration unit, and the other is for the polarimetric modulation unit. Both of the polarizers have an ultra-high contrast ratio of 1,000,000:1 in the optical band. We investigate the instrumental polarization characteristics(IPCs) in the full field of view that is transferred from YFOSC. Furthermore, we identify that the IPCs change when the Cassegrain axis rotates. The spurious polarization from the IPCs can be effectively minimized by flat-fielding using the unpolarized domeflat, when the Cassegrain rotation angle is the same or nearest to that of the polarization observation. We develop a quasiautomatic pipeline for YFPOL and its effectiveness has been verified by tests of the polarimetric observation with blazar S5 0716+714. The calibration is performed by observing the zero-polarized and highly-polarized standard stars. We successfully reach high precision polarization in the 7’ field of view, and the systematic uncertainty is below 0.8% for a V = 11.68 target with a 10 s exposure. The instrument polarization angle offset is 2°. 6. YFPOL is not only a simple polarimeter, but also a spectropolarimeter with grisms that can be considered in the future. 展开更多

针对传统单级热泵的效率随着温升要求的提高急剧恶化及辅助冷凝器余热直接排出造成巨大能源浪费的问题,本工作结合干燥过程前期较低温度干燥对物料烘干品质的提升具有促进作用的特点,提出了带热回收闪蒸级联热泵干燥系统(HR-FCHP)。本... 针对传统单级热泵的效率随着温升要求的提高急剧恶化及辅助冷凝器余热直接排出造成巨大能源浪费的问题,本工作结合干燥过程前期较低温度干燥对物料烘干品质的提升具有促进作用的特点,提出了带热回收闪蒸级联热泵干燥系统(HR-FCHP)。本工作建立了热力学模型,采用文献中的数据进行验证,并对系统进行了全面的热力学分析,同时将所提系统与单级热泵干燥系统(SSHP)、基础闪蒸级联热泵干燥系统(FCHP)和并联冷凝器热泵干燥系统(PCHP)在相同的运行工况下依次进行对比。结果表明,较于SSHP,FCHP和PCHP系统,所提出的HR-FCHP系统具有更高的节能效果和干燥效果,其系统能效比(COP_h)和单位能耗除湿量(SMER)为5.40与4.31 kg/kWh,与SSHP,FCHP和PCHP系统的COP_h和SMER相比分别高出53.85%和65.77%、33.00%和20.73%以及7.14%和9.40%。此外,所提HR-FCHP系统的初始投资成本(C_(IC))最低,为6.64万元,该系统的生命周期成本(C_(LCC))分别比SSHP,FCHP和PCHP系统降低了69.86%,40.92%和8.57%。 展开更多

Tandem duplication(TD)is a major type of structural variations(SVs)that plays an important role in novel gene formation and human diseases.However,TDs are often missed or incorrectly classified as insertions by most modern SV detection methods due to the lack of specialized operation on TD-related mutational signals.Herein,we developed a TD detection module for the Pindel tool,referred to as Pindel-TD,based on a TD-specific pattern growth approach.Pindel-TD is capable of detecting TDs with a wide size range at single nucleotide resolution.Using simulated and real read data from HG002,we demonstrated that Pindel-TD outperforms other leading methods in terms of precision,recall,F1-score,and robustness.Furthermore,by applying Pindel-TD to data generated from the K562 cancer cell line,we identified a TD located at the seventh exon of SAGE1,providing an explanation for its high expression.Pindel-TD is available for non-commercial use at https://github.com/xjtu-omics/pindel. 展开更多

Microsatellite instability(MSI)is an indispensable biomarker in cancer immunotherapy.Currently,MSI scoring methods by high-throughput omics methods have gained popularity and demonstrated better performance than the gold standard method for MSI detection.However,the MSI detection method on expression data,especially single-cell expression data,is still lacking,limiting the scope of clinical application and prohibiting the investigation of MSI at a single-cell level.Herein,we developed MSIsensor-RNA,an accurate,robust,adaptable,and standalone software to detect MSI status based on expression values of MSI-associated genes.We demonstrated the favorable performance and promise of MSIsensor-RNA in both bulk and single-cell gene expression data in multiplatform technologies including RNA sequencing(RNA-seq),microarray,and single-cell RNA-seq.MSIsensor-RNA is a versatile,efficient,and robust method for MSI status detection from both bulk and single-cell gene expression data in clinical studies and applications.MSIsensor-RNA is available at https://github.com/xjtu-omics/msisensor-rna. 展开更多

Arabidopsis thaliana is an important and long-established model species for plant molecular biology,genetics,epigenetics,and genomics.However,the latest version of reference genome still contains a significant number of missing segments.Here,we reported a high-quality and almost complete Col-0 genome assembly with two gaps(named Col-XJTU)by combining the Oxford Nanopore Technologies ultra-long reads,Pacific Biosciences high-fidelity long reads,and Hi-C data.The total genome assembly size is 133,725,193 bp,introducing 14.6 Mb of novel sequences compared to the TAIR10.1 reference genome.All five chromosomes of the Col-XJTU assembly are highly accurate with consensus quality(QV)scores>60(ranging from 62 to 68),which are higher than those of the TAIR10.1 reference(ranging from 45 to 52).We completely resolved chromosome(Chr)3 and Chr5 in a telomere-to-telomere manner.Chr4 was completely resolved except the nucleolar organizing regions,which comprise long repetitive DNA fragments.The Chrl centromere(CEN1),reportedly around 9 Mb in length,is particularly challenging to assemble due to the presence of tens of thousands of CEN180 satellite repeats.Using the cutting-edge sequencing data and novel computational approaches,we assembled a 3.8-Mb-long CEN1 and a 3.5-Mb-long CEN2.We also investigated the structure and epigenetics of centromeres.Four clusters of CEN180 monomers were detected,and the centromere-specific histone H3-like protein(CENH3)exhibited a strong preference for CEN180 Cluster 3.Moreover,we observed hypomethylation patterns in CENH3-enriched regions.We believe that this high-quality genome assembly,Col-XJTU,would serve as a valuable reference to better understand the global pattern of centromeric polymorphisms,as well as the genetic and epigenetic features in plants. 展开更多

Based on fluid inclusion results in whiteschist, primary high-pressure fluids are CO2-bearing aqueous solutions. Those prove that there exists CO2-H2O fluid during the ultrahigh-pressure metamorphism. Pseu-dosecondary H2O inclusions were trapped during ultra-high-pressure exhumation, whereas secondary H2O inclusions were related to the latest evolutionary stage of the ultrahigh-pressure exhumation. 展开更多

Microsatellite instability(MSI)is a key biomarker for cancer therapy and prognosis.Traditional experimental assays are laborious and time-consuming,and next-generation sequencingbased computational methods do not work on leukemia samples,paraffin-embedded samples,or patient-derived xenografts/organoids,due to the requirement of matched normal samples.Herein,we developed MSIsensor-pro,an open-source single sample MSI scoring method for research and clinical applications.MSIsensor-pro introduces a multinomial distribution model to quantify polymerase slippages for each tumor sample and a discriminative site selection method to enable MSI detection without matched normal samples.We demonstrate that MSIsensor-pro is an ultrafast,accurate,and robust MSI calling method.Using samples with various sequencing depths and tumor purities,MSIsensor-pro significantly outperformed the current leading methods in both accuracy and computational cost.MSIsensor-pro is available at https://github.com/xjtu-omics/msisensor-pro and free for non-commercial use,while a commercial license is provided upon request. 展开更多
