A new tumor-associated antigen prognostic scoring system for spontaneous ruptured hepatocellular carcinoma after partial hepatectomy 被引量:5

Objective:Spontaneous hepatocellular carcinoma(HCC)rupture can be fatal,and hepatic resection could achieve a favorable long-term survival among all strategies of tumor rupture.However,there is no available prognostic scoring system for patients with ruptured HCC who underwent partial hepatectomy.Methods:From January 2005 to May 2015,129 patients with spontaneous HCC rupture underwent partial hepatectomy.Preoperative clinical data were collected and analyzed.Independent risk factors affecting overall survival(OS)were used to develop the new scoring system.Harrell’s C statistics,Akaike information criterion(AIC),the relative likelihood,and the log likelihood ratio were calculated to measure the homogeneity and discriminatory ability of a prognostic system.Results:In the multivariable Cox regression analysis,three factors,including tumor size,preoperativeα-fetoprotein level,and alkaline phosphatase level,were chosen for the new tumor-associated antigen(TAA)prognostic scoring system.The 1-year OS rates were 88.1%,43.2%,and 30.2%for TAA scores of 0–5 points(low-risk group),6–9 points(moderate-risk group),and 10–13points(high-risk group),respectively.The TAA scoring system had superior homogeneity and discriminatory ability(Harrell’s C statistics,0.693 vs.0.627 and 0.634;AIC,794.79 vs.817.23 and 820.16;relative likelihood,both<0.001;and log likelihood ratio,45.21 vs.22.77 and 21.84)than the Barcelona Clinic Liver Cancer staging system and the Cancer of the Liver Italian Program in predicting OS.Similar results were found while predicting disease-free survival(DFS).Conclusions:The new prognostic scoring system is simple and effective in predicting both OS and DFS of patients with spontaneous ruptured HCC. 展开更多

印戒细胞癌是胃、结直肠肿瘤中恶性程度最高的病理类型之一。本研究分析了印戒细胞癌的转移特性及其对肿瘤特异性生存率的影响。这项研究纳入了4055例在2010—2012年被诊断为胃印戒细胞癌(signet ring cell carcin om a,SRCC)、结直肠... 印戒细胞癌是胃、结直肠肿瘤中恶性程度最高的病理类型之一。本研究分析了印戒细胞癌的转移特性及其对肿瘤特异性生存率的影响。这项研究纳入了4055例在2010—2012年被诊断为胃印戒细胞癌(signet ring cell carcin om a,SRCC)、结直肠印戒细胞癌患者,其中,胃印戒细胞癌2905例,结肠印戒细胞癌和直肠印戒细胞癌1150例。所有临床数据来源于癌症监测、流行病学、最终结果登记资料数据库(SEER)。我们通过卡方检验分析在确诊时伴有远处转移的患者及无远处转移的患者的临床特征。同时用K aplan-M eier、Cox风险比例回归模型分析出现不同部位转移的患者的预后差异。研究发现,在胃印戒细胞癌患者中,远处淋巴结转移是其最常见的转移部位,而出现脑转移的患者其预后最差。而在患有结肠或直肠的印戒细胞癌患者中,肝脏是其最常见的转移部位,而在确诊时出现远处淋巴结转移的患者拥有最高的死亡率。总之,出现远处转移常常是预后较差的标志之一,我们的研究结果为印戒细胞癌患者的临床随访、预后分析提供一定的建议。 展开更多

Objective:This study aimed to evaluate the safety,efficacy,and feasibility of the rituximab,fotemustine,pemetrexed,and dexamethasone(R-FPD)regimen followed by whole-brain radiotherapy(WBRT)for patients with primary central nervous system lymphoma(PCNSL).Methods:A prospective,single-center phase II clinical trial was conducted.Patients with PCNSL newly diagnosed at the First Affiliated Hospital of Zhengzhou University between July 2018 and July 2020 were studied.The R-FPD regimen consisted of rituximab(375 mg/m2 i.v.on D0),fotemustine(100 mg/m2 i.v.on D1),pemetrexed(600 mg/m2 i.v.on D1),and dexamethasone(40 mg i.v.on D1-5).Patients 60 years or younger who showed a complete response(CR)were treated with 23.4 Gy of WBRT after the end of chemotherapy;those older than 60 years with CR were treated with a wait-and-see approach;and those who did not show CR after the 4th cycle of chemotherapy were given salvage WBRT 30 Gy+local tumor field irradiation up to 45 Gy,regardless of age.Results:A total of 30 patients were included.After 2 cycles,the objective response rate(ORR)was 96.5%(28/29,1 CR,27 PR,0 SD,and 1 PD).After 4 cycles,the ORR was 73.1%(19/26,11 CR,8 PR,4 SD,and 3 PD).After WBRT,the ORR was 90.9%(10/11,7 CR,3 PR,and 1 SD).The grade III and IV toxicity responses were mainly leukopenia(20.0%),thrombocytopenia(23.3%),and anemia(10.0%).Conclusions:Fotemustine-based therapy in combination with rituximab chemotherapy followed by WBRT can improve outcomes,providing ORR benefits and favorable tolerability in patients newly diagnosed with PCNSL. 展开更多

In this study,a series of metal-organic frameworks(MOFs)NH_(2)-UiO-66-x HAc catalysts were synthesized by solvothermal method using acetic acid(HAc)as a modulator,and were applied to the cycloaddition of CO_(2)and epichlorohydrin(EPIC)under ambient pressure.Influences of the modulation by HAc on morphologies and structures of the MOFs are demonstrated via PXRD,FESEM,FTIR,N_(2)adsorption-desorption,XPS and ^(1)H NMR characterizations.The results show that the MOFs containing mesoporous pores can be prepared by adjusting the concentration of HAc.By optimizing the amount of HAc added,the specific surface area of NH_(2)-UiO-66-8 HAc is as high as 879.17 m^(2)·g^(-1),which is 28.3%higher than that of the original MOFs.And the evaluation of catalytic performance showed that HAc modulation enhanced the activity of NH_(2)-UiO-66-x HAc under mild conditions.The exposure of Lewis acid sites,increased specific surface area and porosity via the modulation of HAc defective ligand can be supposed the key factors to determine the enhanced catalytic activities.In addition,considering the influence of gas concentration on the reaction,the concept of TOP(Turnover of Pressure,defined as the mass of conversions of a unit mass catalyst under unit pressure and unit time)was first proposed in this article. 展开更多

[Objective]The paper was to screen the effective agents for effectively controlling the occurrence and prevalence of taro soft rot.[Method]The fungicidal effects of 11 fungicides on taro soft rot were determined by inhibition zone method.[Result]0.3%Tetramycin,25%bromothalonil,3%Zhongshengmycin and 30%zinc thiazole had better antifungal effect on Pectobacterium aroidearum at recommended concentration.The indoor toxicity of four fungicides showed that the EC50 of 0.3%tetramycin AS was the smallest of 0.6μg/mL,indicating that tetramycin had the largest toxicity and the best inhibitory effect against P.aroidearum;followed by 25%bromothalonil EC and 3%Zhongshengmycin WP,with the EC_(50) of 2.57 and 97.72μg/mL,respectively;the inhibitory effect of 30%zinc thiazole SC against P.aroidearum was the poorest.[Conclusion]The study provides a reference for screening out new and efficient chemical agents against taro soft rot. 展开更多

The second proposed density estimator is a geometric extrapolation of the first bias reduced estimator. Theoretical properties such as bias, variance and mean squared error are investigated for both estimators. [Objectives]This study was conducted to clarify the improvement effects of different soil amelioration materials such as lime,green manure and bio-organic fertilizer on acidic tobacco-planting soil.[Methods]The dynamic changes of soil pH,soil nutrients and enzyme activity were studied by applying lime,lime+green fertilizer,and lime+green fertilizer+biological organic fertilizer.[Results]①After the application of amelioration materials,the soil pH and available phosphorus content of tobacco-growing soil showed a stepwise change of first increasing and then decreasing,and became stable at 60 d after tobacco transplanting;the soil organic matter,alkali-hydrolyzable nitrogen and available potassium content showed a gradual change trend of"high-low-high";and the soil invertase and urease activity showed a gradual change trend of"low-high-low".②To 90 d after tobacco transplanting,the application of amelioration materials increased soil pH by 1.29-1.62 units,and increased organic matter content by 15.21%-20.86%,alkali-hydrolyzable nitrogen content by 6.83%-18.17%,available phosphorus content by 54.15%-217.85%,rapidly available potassium content by 11.42%-30.86%,soil invertase activity by 70.09%-18.93%,and soil urease activity by 64.07%-130.47%.③The combination of lime+green manure+alkaline microbial organic fertilizer had the best effect on the improvement of acidic tobacco-growing soil,and the effect of lime+green manure+acid microbial organic fertilizer on acidic soil was the second.[Conclusions]When applying lime,green fertilizer and alkaline bio-organic fertilizer should be applied to achieve sustainable improvement of strongly acidic soil. 展开更多

Based on density functional theory,first-principles calculation is applied to study the electronic properties of undoped and Ag-doped Zn O-Σ7(123^-0)twin grain boundaries(GBs).The calculated result indicates that the twin GBs can facilitate the formation and aggregation of Ag substitution at Zn sites(AgZn)due to the strain release.Meanwhile,some twin GBs can also lower the ionization energy of AgZn.The density of state shows that the O–O bonds in GBs play a key role in the formation of a shallow acceptor energy level.When AgZnbonds with one O atom in the O–O bond,the antibonding state of the O–O bond becomes partially occupied.As a result,a weak spin splitting occurs in the antibonding state,which causes a shallow empty energy level above the valence band maximum.Further,the model can be applied to explain the origin of p-type conductivity in Ag-doped Zn O. 展开更多

The aim of this research was to develop a quantitative method for clinicians to predict the probability of improved prognosis in patients with coronavirus disease 2019(COVID-19).Data on 104 patients admitted to hospital with laboratory-confirmed COVID-19 infection from 10 January 2020 to 26 February 2020 were collected.Clinical information and laboratory findings were collected and compared between the outcomes of improved patients and non-improved patients.The least absolute shrinkage and selection operator(LASSO)logistics regression model and two-way stepwise strategy in the multivariate logistics regression model were used to select prognostic factors for predicting clinical outcomes in COVID-19 patients.The concordance index(C-index)was used to assess the discrimination of the model,and internal validation was performed through bootstrap resampling.A novel predictive nomogram was constructed by incorporating these features.Of the 104 patients included in the study(median age 55 years),75(72.1%)had improved short-term outcomes,while 29(27.9%)showed no signs of improvement.There were numerous differences in clinical characteristics and laboratory findings between patients with improved outcomes and patients without improved outcomes.After a multi-step screening process,prognostic factors were selected and incorporated into the nomogram construction,including immunoglobulin A(IgA),C-reactive protein(CRP),creatine kinase(CK),acute physiology and chronic health evaluation II(APACHE II),and interaction between CK and APACHE II.The C-index of our model was 0.962(95%confidence interval(CI),0.931-0.993)and still reached a high value of 0.948 through bootstrapping validation.A predictive nomogram we further established showed close performance compared with the ideal model on the calibration plot and was clinically practical according to the decision curve and clinical impact curve.The nomogram we constructed is useful for clinicians to predict improved clinical outcome probability for each COVID-19 patient,which may facilitate personalized counselling and treatment. 展开更多

Bone regeneration for large,critical-sized bone defects remains a clinical challenge nowadays.Guided bone regeneration(GBR)is a promising technique for the repair of multiple bone defects,which is widely used in oral and maxillofacial bone defects but is still unsatisfied in the treatment of long bone defects.Here,we successfully fabricated a bilayer mineralized collagen/collagen(MC/Col)-GBR membrane with excellent osteoinductive and barrier function by coating the MC particles prepared via in situ biomimetic mineralization process on one side of a sheet-like pure collagen layer.The aim of the present study was to investigate the physicochemical properties and biological functions of the MC/Col film,and to further evaluate its bone regeneration efficiency in large bone defect repair.Fouriertransform infrared spectra and X-ray diffraction patterns confirmed the presence of both hydroxyapatite and collagen phase in the MC/Col film,as well as the chemical interaction between them.stereo microscope,scanning electron microscopy and atomic force microscope showed the uniform distribution of MC particles in the MC/Col film,resulting in a rougher surface compared to the pure Col film.The quantitative analysis of surface contact angle,light transmittance and tensile strength demonstrated that the MC/Col film have better hydrophilicity,mechanical properties,light-barrier properties,respectively.In vitro macrophage co-culture experiments showed that the MC/Col film can effectively inhibit macrophage proliferation and fusion,reducing fibrous capsule formation.In vivo bone repair assessment of a rabbit critical segmental radial defect proved that the MC/Col film performed better than other groups in promoting bone repair and regeneration due to their unique dual osteoinductive/barrier function.These findings provided evidence that MC/Col film has a great clinical potential for effective bone defect repair. 展开更多

Based on rational design, benzoyl cyanide, a well-known ionic acylating reagent, has been developed as a general radical acylating reagent for benzylic site-selective acylation reactions for the first time. Using visible-light photoredox-catalyzed single electron transfer(SET) and hydrogen atom transfer(HAT) strategies, two different direct benzylic C–H acylation reactions with high atom economy have been developed. This newly developed radical acylating reagent could also be used to achieve three component acyltrifluoromethylation of styrenes, which is usually challenging to achieve by other means. All of these reactions proceed under mild nickel-free and NHC-free conditions with high functional group tolerance. In addition, based on our detailed mechanistic experiments, the process of radical-radical cross-coupling mechanism was suggested for these transformations. 展开更多

Objectives:This study aimed to investigate the impact of different dietary lipid levels on the physiological metabolism and odor quality of adult female Eriocheir sinensis during the fattening phase.Materials and Methods:Five dietary schemes,each containing different total lipid levels(4%,7%,10%,13%,and 16%),were administered over a 60-d fattening period for adult female crab(approximately 50 g,n=60).Chromatography and spectroscopy techniques were used for the detection of aroma and key aroma precursors.Results:Dietary lipids had no significant effect on growth performance and hepatopancreas index(P<0.05).The carotenoid content showed an increasing and then decreasing trend,with a significant increase to 1312.51 mg/kg(P<0.05)at 7%lipid content of the diet.In contrast,a diet with a 13%lipid level led to a significant increase in total unsaturated fatty acids(115.3 mg/g,P<0.05),which maximized the contents of major fatty acids(oleic,linoleic,and linolenic,among others),and presented a fluctuating trend.A diet with higher lipid levels(16%)inhibited fatty acid accumulation.Moreover,a 13%dietary lipid level enhanced characteristic aroma compounds(for example,nonanal,octanal,hexanal,3-methylbutanal,and 2-heptanone)in E.sinensis cooked hepatopancreas.Through an orthogonal partial least squares discriminant analysis model,it becomes evident that lipids and carotenoids together promote aroma formation in hepatopancreas.Conclusions:Based on the current findings,the optimal dietary lipid level for enhancing the odor quality of hepatopancreas in adult female E.sinensis during the fattening phase was 13%.This study provides more precise options for the high-quality cultivation of E.sinensis. 展开更多

Influencing assessment of different heating modes on thermal comfort in electric vehicle cabin

Electric vehicle(EV)is more environmental-friendly than gasoline vehicle because the main power is not petrol.For the heating system,the waste heat generated by engines is recycled to heat the cabin of gasoline ones,while the EV need extra electric energy consumption for heating the cabin which decreases the driving range.Personal comfort system(PCS)has been reported that it is an alternative technique to improve occupant thermal comfort and indirectly save building heating energy.This study aims to investigate the occupants’thermal sensation under four heating modes in the EV cabin in winter,including no heating,only PCS,only air conditioning,and both air conditioning and PCS.The field survey is conducted in a parked EV with 12 subjects.Three thermal comfort indexes(PMV,SET^(*),and operative temperature Top)are used and assessed for their applicability in the thermal environment of the EV.Results show that the thermal environment in the cabin is non-uniform and dynamic with the air conditioning mode,while the PCS mode just slightly affects the temperature differences inside.Meanwhile,the subjects’thermal sensations cannot maintain neutral or warmer under the PCS mode only.The results of PMV are lower than the subjects’actual thermal sensation votes(TSV),while the results of SET^(*)and Top show significant linear relationships with the TSV for the correlation coefficients above 0.80.Compare to the air conditioning mode,the neutral SET^(*)and neutral Top are decreased by 2.7℃ and 1.6℃ under the mode with PCS and air conditioning,respectively.The findings indicate that PCS not only has a positive effect on improving the cabin environment and occupant’s thermal sensation but also has the potential to lower the setting temperature of air conditioner of EVs in winter. 展开更多

目的:探讨配戴角膜塑形镜6个月后去片裸眼视力低下的影响因素及近视控制效果。方法:回顾性系列病例研究。收集2015年4月至2017年1月于北京同仁医院验光配镜中心验配角膜塑形镜并随访至少6个月的青少年儿童,以去片后即刻裸眼视力能否达到... 目的:探讨配戴角膜塑形镜6个月后去片裸眼视力低下的影响因素及近视控制效果。方法:回顾性系列病例研究。收集2015年4月至2017年1月于北京同仁医院验光配镜中心验配角膜塑形镜并随访至少6个月的青少年儿童,以去片后即刻裸眼视力能否达到0.8为标准将入选儿童分为2组。比较2组间年龄、视力、屈光度、眼压、角膜曲率、e值、角膜直径、瞳孔直径、非对称性指数、规则性指数的异同,以及对近视的控制效果。采用单因素及多因素线性回归分析去片裸眼视力的影响因素。结果:共50例(96眼)入组,年龄(10.6±2.4)岁,球镜度(-3.62±1.18)D,柱镜度(-0.33±0.43)D,眼轴长度(25.18±0.75)mm。其中裸眼视力≤0.8组52眼,裸眼视力>0.8组44眼,通过单因素分析,较差的裸眼视力与基础较长的眼轴长度(b=-0.09,β=-0.29,P=0.003)、较高的球镜度(b=0.07,β=0.36,P<0.001)、较高的柱镜度(b=0.12,β=0.22,P=0.030)有关。通过多因素线性回归分析,较差的裸眼视力与基础较高的球镜度(b=0.05,β=0.27,P=0.018)有关。6个月后裸眼视力≤0.8组儿童眼轴增长(0.07±0.15)mm,裸眼视力>0.8组儿童眼轴增长(0.11±0.15)mm,差异无统计学意义。结论:影响角膜塑形镜去片裸眼视力的主要因素是屈光度和眼轴长度,屈光度越高、眼轴越长,去片后裸眼视力越差,但其对近视仍有较好的控制作用。 展开更多
