Manual aspiration thrombectomy for acute and subacute inferior vena cava thrombosis and lower extremity deep venous thrombosis 被引量:17

Deep vein thrombosis(DVT), which can lead to pulmonary embolism(PE), is a major contributor to the global disease burden and is the third most common cardiovascular pathology after coronary artery disease and stroke. ... Deep vein thrombosis(DVT), which can lead to pulmonary embolism(PE), is a major contributor to the global disease burden and is the third most common cardiovascular pathology after coronary artery disease and stroke. Venous thromboembolic disease, which encompasses the disease entities of DVT and PE, affects up to 10 million cases every year and represents a serious and potentially life-threatening condition. Standard anticoagulation therapy alone is ineffective at promoting deep venous system thrombus removal. Many patients develop postthrombotic syndrome(PTS) despite being on adequate anticoagulation therapy. Aggressive therapy for rapid thrombus removal is important to prevent the development of PTS. Besides impeding the onset of PTS, rapid clearance of the thrombus is also required in the treatment of phlegmasia cerulea dolens, an uncommon but life-threatening complication of acute DVT that can lead to arterial insufficiency, compartment syndrome, venous gangrene, and limb amputation. Manual aspiration thrombectomy(MAT) can provide rapid and effective therapy that could be compared to the open surgical thrombectomy approach with minimal risk of morbidity, mortality, or recurrence after surgery. Though many devices have been developed to date for pharmacomechanical thrombolysis, the cost of the treatment using these devices is very expensive. MAT is simple to perform, easy to learn, inexpensive, and rapid. This review will outline and dissect several studies and case reports, sourced from the Pub Med database, on the subject of the use of MAT in treating inferior vena cava thrombosis and lower extremity DVT, including in patients with compression of the iliac vein and phlegmasia cerulea dolens. 展开更多

The operational procedures, considerations, preoperative management, and prevention of complications were also updated, supplemented, and revised. Objective:To compare the safety of conventional transarterial chemoembolization(cTACE)vs drug-eluting bead TACE(DEB-TACE)in very early-and early-stage hepatocellular carcinoma(HCC).Methods:Data of patients with early-and very early-stage HCC treated with cTACE or DEB-TACE were evaluated retrospectively in this study.A total of 40 patients were included,20 treated with cTACE and 20 with DEB-TACE.The cTACE and DEB-TACE groups were comprised of 80%and 75%males,while there were 20%females in cTACE group and 25%in Deb-TACE group respectively.The mean age of patients in cTACE group was 57.43+5.6 years,while it was 56.4+5.5 years in DEB-TACE group.All patients had liver status of Child–Pugh Class A and a score≤7 in Child-Pugh class type B in very early-(stage 0)or early-phase(stage A)stages according to the Barcelona Clinic Liver Cancer(BCLC)system.Results:The Child-Pugh class degradation in the cTACE group was slightly higher than that in the DEB-TACE group.Serious complications like peritumoral parenchymal ischemia were observed in 4 patients in the cTACE group and 5 in the DEB-TACE group.Localized bile duct dilation was seen in 2 patients in the cTACE group and 6 in the DEB-TACE group.No significant variation in serious complications between the two groups was established in localized bile duct dilatation.Other minor complications noted were liver failure,liver abscess,liver infarction,acute cholecystitis,biliary tree necrosis,and mortality.Further,no substantial variation in tumor response between the groups was reported immediately and 1-year post-procedural assessment.Conversion rate to other treatment modalities such as surgical resection,radiofrequency ablation(RFA),or swap between cTACE and DEB-TACE was substantially higher in the DEB-TACE group(40%)than in the cTACE group(10%)at the 1-year completion period of the study.Conclusion:In terms of tumor response,the DEB-TACE group showed a better response,to some extent,as an initial therapy for HCC in the early stages as compared to the cTACE group,and DEB-TACE also exhibited better clinical efficacy in patients with HCC. 展开更多

Coronavirus disease 2019 or most commonly known as COVID-19 is a trending global infectious disease which a few months ago was affirmed as a global health emergency or a pandemic by the WHO Emergency Committee.The common symptoms manifested in this pandemic disease are high grade fever,cough,fatigue,shortness of breath and flu like symptom which can evolve into severe respiratory disorders such as pneumonia,acute respiratory distress syndrome(ARDS)and/or end-organ failure.Factors that contribute to the severity or high mortality rate in COVID-19 include old age,comorbidities like hypertension,diabetes,hyperlipidaemia,neutrophilia,and organ and coagulation dysfunction.Disseminated intravascular coagulation and other various coagulopathies including Venous thromboembolism have known to become a major contributing factor to high mortality rate.Venous thromboembolism is a disease which is a combination of deep vein thrombosis and pulmonary embolism.Prophylactic anticoagulation in patients prone to or with a pre-existing history of venous thromboembolism is associated with decreased mortality in severe COVID-19 pneumonia.This review article focuses upon COVID-19 and increased incidence of venous thromboembolism in patients infected by COVID-19 along with the role it has in high mortality rate in COVID-19 patients. 展开更多

Currently,there are a limited number of dynamic models available for braided composite plates with large overall motions,despite the incorporation of three-dimensional(3D)braided composites into rotating blade components.In this paper,a dynamic model of 3D 4-directional braided composite thin plates considering braiding directions is established.Based on Kirchhoff's plate assumptions,the displacement variables of the plate are expressed.By incorporating the braiding directions into the constitutive equation of the braided composites,the dynamic model of the plate considering braiding directions is obtained.The effects of the speeds,braiding directions,and braided angles on the responses of the plate with fixed-axis rotation and translational motion,respectively,are investigated.This paper presents a dynamic theory for calculating the deformation of 3D braided composite structures undergoing both translational and rotational motions.It also provides a simulation method for investigating the dynamic behavior of non-isotropic material plates in various applications. 展开更多

The nano-carbon powders are often used as fillers to endow the shape memory polymers(SMPs)with electroconductivity.It has been found that the shape memory effects(SMEs)of SMPs filled with nano-carbon powder can be triggered both by temperature and by water.To reveal the driving mechanism of SMEs,a constitutive model for describing the thermally activated and moisture activated SMEs of these shape memory polymer composite(SMPCs)is developed here.Because both of the SMEs share the same driving mechanism,the variable moisture is incorporated into the framework of a thermo-mechanical modeling approach to disclose the effect of moisture on the thermoviscoelastic properties.The SMPCs are regarded as isotropic materials and the effect of carbon powder on the mechanical properties of the matrix is also considered in the paper.Because the complete recovery may not be reached even they are exposed to the stimulus environment long enough,the blocking mechanism is also considered here.This is the mainly new contribution compared to the early work.Using the method of parameter determination presented here,the effectiveness of the proposed hygro-thermo-mechanical constitutive model is confirmed by comparing the model results with the test data of uniaxial deformation from the literature. 展开更多

目的了解介入相关医务人员工作压力现状,探索工作压力的影响因素,为提高介入相关医务人员社会心理健康及工作状态的干预措施相关研究提供参考。方法编制《介入相关医务人员工作压力现状调查问卷》,采用一般人口学资料调查表、工作压力... 目的了解介入相关医务人员工作压力现状,探索工作压力的影响因素,为提高介入相关医务人员社会心理健康及工作状态的干预措施相关研究提供参考。方法编制《介入相关医务人员工作压力现状调查问卷》,采用一般人口学资料调查表、工作压力源量表对南通市所有三级医院介入相关医务人员进行问卷调查,收集相关数据,并通过采用SPSS 24.0统计软件进行数据分析,包括独立样本t检验、方差分析以及回归分析等。结果采用Likert 5分计分法分非常同意、同意、无意见、不同意、非常不同意5个选项,分别赋予5至1分值,得分如下:南通市三级医院介入相关医务人员工作压力得分(3.78±0.806),处于偏高水平,排在前三位的是来自工作负荷得分(3.54±0.85),外部环境得分(3.45±0.87),组织管理得分(3.20±0.74),其他维度得分分别是职业前景得分(2.73±0.87),医患关系得分(2.71±0.83),工作兴趣得分(2.68±0.68),人际关系得分(2.04±0.85)。结论南通市介入相关医务人员工作压力较大,主要压力源来自工作负荷、外部环境和组织管理方面的压力。36~40岁的介入相关医务人员工作压力要高于其他年龄组,男性高于女性,减轻工作压力,提高心理健康水平,有助于减少介入相关医务人员的职业倦怠。 展开更多

This paper proposes a new thermoviscoelastic finite deformation model incorporating dual relaxation mechanisms to predict the complete thermo-mechanical response of amorphous shape memory polymers.The model is underpinned by the detailed microscopic molecular mechanism and effectively reflects the current understanding of the glass transition phenomenon.Novel evolution rules are obtained from the model to characterize the viscous flow,and a new theory named an internal stress model is introduced and combined with the dual relaxation mechanisms to capture the stress recovery.The rationality of the constitutive model is verified as the theoretical results agree well with the experimental data.Moreover,the constitutive model is further simplified to facilitate engineering applications,and it can roughly capture the characteristics of shape memory polymers. 展开更多
