Weekly albumin-bound paclitaxel/cisplatin versus gemcitabine/cisplatin as first-line therapy for patients with advanced non-small-cell lung cancer:A phase II open-label clinical study 被引量:9

Objective: The aim of this trial was to compare both the efficacy and the safety of a weekly nanoparticle albumin-bound paclitaxel(nab-paclitaxel) plus cisplatin vs. gemcitabine plus cisplatin in patients with advanced non-small-cell lung cancer(NSCLC).Methods: A total of 84 participants received either 100 mg/m^2 nab-paclitaxel each week on d 1, 8 and 15 of a 28 day cycle, as well as cisplatin 75 mg/m^2 on d 1 every three weeks(nab-TP arm); or gemcitabine 1,000 mg/m^2 on d 1 and 8, plus cisplatin 75 mg/m^2 on d 1 every three weeks(GP arm). The primary end point was progression-free survival(PFS). The secondary end points were overall response rate(ORR) and overall survival(OS).Results: According to our analysis, the median PFS was 4.8 months for the nab-TP arm vs. 5.2 months for the GP arm(P=0.55). Analysis showed the median OS was 14.6 months for participants who were in the nab-TP arm vs. 15.1 months for those in the GP arm(P=0.94). Besides, nab-TP showed OS advantages over GP in patients harboring epidermal growth factor receptor(EGFR) mutation(26.7 vs. 15.3 months, P=0.046) and patients with a performance status of 0(23.5 vs. 14.7 months, P=0.020). It was found that incidences of drug-related grade 3 or 4 toxicities were comparable between the two treatment arms.Conclusions: Therefore, it can be seen that weekly nab-TP treatment has a similar efficacy and tolerability to GP treatment for patients who are undergoing their first-line treatment for NSCLC. It could be that survival differences among platinum doublets in the context of both EGFR mutation and performance status have the potential to be the basis for our further clinical trials. 展开更多

Chemotherapy-induced neutropenia(CIN)is a potentially fatal and common complication in myelosuppressive chemotherapy.The timing and grade of CIN may play prognostic and predictive roles in cancer therapy.CIN is associated with older age,poor functional and nutritional status,the presence of significant comorbidities,the type of cancer,previous chemotherapy cycles,the stage of the disease,specific chemotherapy regimens,and combined therapies.There are many key points and new challenges in the management of CIN in adults including:(1)Genetic risk factors to evaluate the patient’s risk for CIN remain unclear.However,these risk factors urgently need to be identified.(2)Febrile neutropenia(FN)remains one of the most common reasons for oncological emergency.No consensus nomogram for FN risk assessment has been established.(3)Different assessment tools[e.g.,Multinational Association for Supportive Care in Cancer(MASCC),the Clinical Index of Stable Febrile Neutropenia(CISNE)score model,and other tools]have been suggested to help stratify the risk of complications in patients with FN.However,current tools have limitations.The CISNE score model is useful to support decision-making,especially for patients with stable FN.(4)There are still some challenges,including the benefits of granulocyte colony stimulating factor treatment and the optimal antibiotic regimen in emergency management of FN.In view of the current reports,our group discusses the key points,new challenges,and management of CIN. 展开更多

The effects of externally applied resonant magnetic perturbation (RMP) on the locked mode of the neoclassical tearing mode (NTM) are numerically investigated by means of a set of reduced magnetohydrodynamic equations.It is found that,for a small bootstrap current fraction,three regimes,namely the slight suppression regime,the small locked island (SLI) regime and the big locked island (BLI) regime,are discovered with the increase of RMP strength.For a much higher bootstrap current fraction,however,a new oscillation regime appears instead of the SLI regime,although the other regimes still remain.The physical process in each regime is analyzed in detail based on the phase difference between the NTM and the RMP.Moreover,the critical values of the RMP in both SLI and BLI regimes are obtained,and their dependence on key plasma parameters is discussed as well. 展开更多

The traditional tenon and mortise joint has low processing efficiency and a weak theoretical basis,making the structure easy to deform and damage,reducing the safety,and increasing waste of resources.This study aims to determine the optimum dowel center spacing parameter for chamfered-joint components and the maximum value of the strength of joints loaded into bending strength and tensile strength.In this study,an integrated opti-mization method combining the single-factor test and one-way ANOVA analysis was proposed to study the influ-ence of the dowel center spacing on the bending strength and the tensile strength of chamfered-joint components made by Cupressus funebris wood.The results revealed that the bending strength of chamfered-joint components decreases linearly with the increase of the dowel center spacing.In addition,the tensile strength of chamfered-joint components increases first and then decreases with the increase of the dowel center spacing,showing para-bola change.The relational expression between dowel center spacing,the bending strength,dowel center spacing and the tensile strength were obtained. 展开更多

The performance of a superconducting quantum interference device(SQUID) gradiometer is always determined by its pick-up coil geometry, such as baseline and radius. In this paper, based on the expressions for the coupled flux threading a magnetometer obtained by Wikswo, we studied how the gradiometer performance parameters, including the current dipole sensitivity, spatial resolution and signal-to-noise ratio(SNR), are affected by its pick-up coil via Mat Lab simulation.Depending on the simulation results, the optimal pick-up coil design region for a certain gradiometer can be obtained.To verify the simulation results, we designed and fabricated several first-order gradiometers based on the weakly damped SQUID with different pick-up coils by applying superconducting connection. The experimental measurements were conducted on a simple current dipole in a magnetically shielding room. The measurement results are well in coincidence with the simulation ones, indicating that the simulation model is useful in specific pick-up coil design. 展开更多

The degradation of omethoate was conducted using H_(2)O_(2) as oxidant,TiO_(2) supported on NaY zeolite as photocatalyst and a 300W lamp as light source.The effect of the calcination temperature of the photocatalyst,the amount of TiO_(2) loaded on NaY zeolite,the photocatalyst amount,the pH value and the radiation time on the degradation ratio of omethoate were investigated.The results show that TiO_(2)/NaY zeolite photocatalyst prepared by sol-gel method had good photocatalysis.The photo-catalytic optimum oxidationconditions of omethoate are as follows:the calcination temperature of the photocatalyst is 550℃,the amount of TiO_(2) loaded on NaY zeolite is 35.2 wt-%,the amount of photocatalyst is 5g/L,pH=8 and the radiation time is 180min.Under these conditions,the removal ratio of omethoate is up to 93%. 展开更多

Blueberries are rich in anthocyanins,which have numerous nutritional and pharmacological effects.In this study,the absorption and stability of rabbiteye blueberry anthocyanins extract(BAE)were assessed using in vitro digestion and Caco-2 cell monolayers.Overall,similar patterns were found through different phases of simulated in vitro digestion,however,the total anthocyanin content(TAC)of BAE significantly decreased.Among all thirteen anthocyanins from BAE,the highest bioaccessibility of>50%was found in Cyanidin-3-glucoside(Cy-3-glc),Petunidin-3-galactoside,(Pt-3-gal),and Cyanidin-3-arabinoside(Cy-3-ara).However,malvidin glycosides represented around 55%of TAC with 27-35%bioaccessibility.The absorption Caco-2 cell monolayers assay indicated that blueberry anthocyanins with higher hydrophobicity are more efficiently absorbed.In addition,the structure of glycosides impacts the absorption and transport efficiencies of anthocyanins.This study provides data on the stability and absorption of blueberry anthocyanins,that can be used to explore the underlying mechanism of BAE absorption. 展开更多
