摘要
目的比较在小儿巨大疝中应用腹腔镜下双线与单线疝囊高位结扎术的治疗效果。方法随机选取2020年1月—2023年1月盱眙县人民医院收治的120例巨大疝患儿为研究对象,根据治疗方案的不同分为单线组(腹腔镜下行单线疝囊高位结扎术)与双线组(腹腔镜下行双线疝囊高位结扎术),每组60例。对比两组治疗效果,手术指标,术后6 h、12 h、24 h及48 h的视觉模拟评分法(Visual Analogue Scale,VAS)评分,记录并发症发生情况,并统计术后1年疝气复发率。结果双线组治疗总有效率为98.33%(59/60),高于单线组85.00%(51/60),差异有统计学意义(χ^(2)=6.982,P<0.05)。两组的术中出血量、术后下床活动、住院时间比较,差异均无统计学意义(P均>0.05)。与单线组手术时间相比,双线组更长,差异有统计学意义(P<0.05)。两组术后6 h、12 h、24 h、48 h的VAS评分及并发症发生情况差异均无统计学意义(P均>0.05)。术后1年复发率双线组低于单线组[0(0/60)比10.00%(6/60)],差异有统计学意义(χ^(2)=4.386,P<0.05)。结论在腹腔镜下行双线疝囊高位结扎术可进一步提高对巨大疝患儿的治疗效果,并降低疾病复发风险,可靠性较高。
Objective To compare the therapeutic effect of laparoscopic double-line and single-line high ligation of hernia sac in children with huge hernia.Methods A total of 120 children with giant hernia admitted to Xuyi County People's Hospital from January 2020 to January 2023 were randomly selected as the research objects.According to the different treatment schemes,they were divided into single-line group(laparoscopic single-line high ligation of hernia sac)and double-line group(laparoscopic double-line high ligation of hernia sac),with 60 cases in each group.The therapeutic effects,surgery index and the Visual Analogue Scale(VAS)scores at 6 h,12 h,24 h and 48 h after operation of the two groups were compared.The incidence of complications was recorded,and the recurrence rate of the disease at 1 year after operation was counted.Results The total effective rate of the double-line group was 98.33%(59/60),which was higher than that of the single-line group 85.00%(51/60),and the difference was statistically significant(χ^(2)=6.982,P<0.05).There were no significant differences in intraoperative blood loss,postoperative ambulation and hospitalization time between the two groups(all P>0.05).Compared with the single-line group,the operation time of the double-line group was longer,the difference was statistically significant(P<0.05).The 1-year recurrence rate of the double-line group was lower than that of the single-line group[0(0/60)vs 10.00%(6/60)],and the difference was statistically significant(χ^(2)=4.386,P<0.05).Conclusion Laparoscopic high ligation of double line hernia sac can further improve the treatment effect of giant hernia in children,and reduce the risk of disease recurrence,with high reliability.
