摘要
目的探讨股骨粗隆间骨折行手术治疗后配合自拟中药方剂对改善肢体肿胀的效果及对D-Ⅱ聚体、FIB指标的作用。方法方便选取2020年1月—2021年6月该院收治的股骨粗隆间骨折患者120例,均接受手术治疗,根据随机数表法分为两组,各60例。对照组患者术后6 h给予利伐沙班治疗,观察组患者术后6 h在其基础上联合自拟中药方剂治疗。检测对比两组患者术后1、3、7、10 d下肢周径、D-Ⅱ聚体、FIB指标变化,评估两组患者术后不同时间疼痛程度变化。结果两组患者术后1 d下肢周径测量结果、D-Ⅱ聚体、FIB检测结果差异无统计学意义(P>0.05);术后3、7、10 d,观察组患者下肢周径、D-Ⅱ聚体、FIB检测结果均低于对照组,差异有统计学意义(P<0.05)。观察组患者术后3、7、10 d疼痛评分(4.48±0.56)分、(2.39±0.33)分、(2.01±0.15)分均低于对照组,差异有统计学意义(t=17.862、32.696、47.944,P<0.05)。结论股骨粗隆间骨折患者接受手术治疗后配合自拟中药方剂治疗可有效消除术后肿胀,改善临床指标,减轻患者痛感。
Objective To explore the effect of the treatment of intertrochanteric fracture of femur with self-made traditional Chinese medicine on improving limb swelling and the effect on D-Ⅱ polymer and FIB index.Methods 120 patients with femoral intertrochanteric fractures admitted to the hospital from January 2020 to June 2021 were conveniently selected,and all received surgical treatment.According to the random number table method,they were divided into two groups,with 60 cases in each group.The patients in the control group were treated with rivaroxaban 6 h after the operation,and the patients in the observation group were treated with self-made traditional Chinese medicine on the basis of 6 h after the operation.The changes of lower extremity circumference,D-Ⅱ polymer and FIB index were detected and compared the two groups at 1 d,3 d,7 d,10 d after operation,and the changes in pain degree of the two groups of patients at different times after operation were evaluated.Results There was no statistically significant difference in the measurement results of lower limb circumference,D-Ⅱ polymer and FIB between the two groups at 1 d after operation(P>0.05).At 3 d,7 d,and 10 d after operation,the detection results of lower limb circumference,D-Ⅱ polymer and FIB of the observation group were lower than those of the control group,and the difference was statistically significant(P<0.05).The pain scores(4.48±0.56)points,(2.39±0.33)points,(2.01±0.15)points of patients in the observation group at 3 d,7 d,10 d,after operation were lower than those in the control group,and the difference was statistically significant(t=17.862,32.696,47.944,P<0.05).Conclusion Patients with intertrochanteric fractures of the femur undergoing surgical treatment,combined with self-made traditional Chinese medicine prescriptions,can effectively eliminate postoperative swelling,improve clinical indicators,and reduce pain in patients.
