摘要
目的探讨DNA倍体分析联合液基薄层细胞学(thinprep cytology test,TCT)检查在宫颈癌筛查中的临床价值。方法收集在本院参与宫颈癌筛查的1185例患者的宫颈细胞学标本,每例标本涂片2张,分别行福根(Feulgen)染色后行全自动扫描DNA倍体分析及巴氏染色后TCT诊断,比较分析两种检查方法及联用的阳性检出率及与组织病理学的符合情况。结果 TCT阳性检出率为14.94%(177/1185),与组织病理学的符合率为83.43%(151/181),TCT的准确率为89.85%(177/197);DNA倍体分析>5C的非整倍体细胞检出率为15.70%(186/1185),与组织病理学的符合率为95.58%(173/181),准确率为94.42%(186/197);TCT联合DNA倍体分析与组织病理学的符合率为97.24%(176/181)。结论在宫颈癌筛查中,TCT联合DNA倍体分析可提高宫颈早期癌及癌前病变的检出率和诊断准确率。
Objective To investigate the clinical value of DNA ploidy analysis combined with thinprep cytology test (TCT) in the screening of cervical cancer. Method Participate in the screening of cervical cancer in our hospital from 1185 cases of cervical cytology specimens (in some cases, TCT biopsy pathology) smear 2, respectively Feulgen staining and Papanicolaou staining, Feulgen staining of smear automatic scanning of DNA ploidy analysis diagnosis, Papanicolaou staining tablets TCT comparative analysis of two methods of positive rate, and histopathology to fit the situation. Result The positive rate of TCT is 14.9% (177/1185), and the histological coincidence rate is 83.43% (151/181), the accuracy rate of TCT is 89.85% (177/197). The detection rate of DNA ploidy analysis 〉5C aneuploid cells was 15.70% (186/1185). The coincidence rate with histopathology was 95.58% (173/181). The DNA ploidy analysis was 〉5C. The accuracy rate of somatic cells was 94.42% (186/197). The coincidence rate of positive and histopathological findings of TCT combined DNA ploidy analysis was 97.24% (176/181). Conclusion In the screening of cervical cancer, liquid based cytology and DNA ploidy analysis can improve the detection rate of cervical cancer and precancerous lesions and its diagnostic accuracy .
