摘要
目的:评价磁共振电影成像(cine-MRI)对成人房间隔缺损(ASD)的诊断价值及临床意义。方法:39例ASD患者分为外科修补术与DSA封堵术两组,采用cine-MRI检查,Philips Achieva Extended MR Workspace 2.6.3.1工作站心脏软件分析ASD最大孔径、伴发的血管畸形和心功能情况,并与经胸超声心动图(TTE)和手术结果作比较。结果:外科修补术组cine-MRI、TTE和手术测量的ASD最大孔径分别为(2.55+1.19)cm、(2.36+1.14)cm和(2.43+1.09)cm,cine-MRI、TTE与手术测值相关性高(r值分别为0.991、0.994,P>0.05);DSA封堵术组cine-MRI、TTE和手术测量的的ASD最大孔径分别为(1.65+0.95)cm、(1.57+0.85)cm和(1.65+0.85)cm,r值分别为0.981、0.992,P>0.05;cine-MRI对右心房增大、右心室增大、右心室功能降低较TTE有优势,χ2分别为4.76、6.85、4.3,两者有统计学意义(P<0.05);并且cine-MRI能够发现TTE不易显示的伴发ASD的动静脉畸形。结论:cine-MRI对成人ASD有较高的诊断价值。
Purpose: To evaluate the diagnostic value and clinical significance of cine magnetic resonance imaging (cine-MRI) in adult atrial septal defect (ASD). Methods: Thirty-nine patients with ASD were divided into two groups, which was surgical repair group and DSA closure group. They were all examined with cine-MRI. Philips Achieva Extended MR Workspace heart 2.6.3.1 software was used to analyze the ASD maximum aperture, comorbid vascular malformation and heart function. And statistical analysis was further done between the thoracic echocardiography (TTE) and the surgical results. Results: The ASD maxium apertures measured by Cine-MRI, TTE and operation in the group of surgical repair were (2.55±1.19), (2.36±1.14), (2.43±1.09) cm, and high correlations between cine-MRI, TTE and the operation were presented (r = 0.991 and 0.994,P〉0.05). For the group of DSA closure, the corresponding values were (1.65±0.95), (1.57±0.85), (1.65±0.85) cm, and r = 0.981, 0.992 (P〉 0.05); Compared with TTE, cine-MRI had advantages in the display of right atrium expansion, right ventricle enlargement, and the function decline of right ventricular, with Z2 values of 4.76, 6.85 and 4.3, which were with statistical significant (P 〈 0.05). Additionally, cine- MRI could be used to distinguish the arteriovenous malformation in the ASD, which could not be easily found with TTE. Conclusion: Cine-MRI has higher clinical diagnosis value in the adult ASD, and can provide a new noninvasive approach for the clinical diagnosis of heart disease..
