胰胆管肿瘤手术中门静脉损伤的防治

摘要 目的探讨胰胆管肿瘤手术中门静脉损伤原因与防治措施。方法分析9年来收治17例胰胆管肿瘤手术中合并门静脉损伤的病例。结果17例门静脉损伤的患者中13例与局部组织粘连紧密和肿瘤侵犯有关,手术操作不当引起4例。17例中局部缝合压迫止血2例,门静脉缝合修补11例,自体静脉移植修补4例,所有病例均无手术死亡。结论局部组织粘连与肿瘤侵犯血管是导致门静脉损伤的主要原因,熟悉解剖、提高手术技巧是预防胰胆管肿瘤手术中门静脉损伤的关键,门静脉缝合修补和自体静脉、人工血管移植修补是治疗门静脉损伤的有效措施。
